How is this different from many implementations where a business processes is managed and monitored end to end with KPIs measured along the various parts of the process to resolve issues and improve the process? Has been there forever and no one called it Digital Twin.

Yes, that's part of it - and usually called business process management. Problem is to combine this with other parts of the organization like people and what they do, projects in flight, etc. A holistic view of the organization will be much helpful.
