    In order to diagnose between reversibile pulpitis and irreversible pulpitis, Dr Herb Schilder , in Boston, taught many years ago the following way to test. Place a small ice cotton pledget on the tooth very briefly. A sharp reaction which recedes immediately after removal of ice indicates reversible pulpitis I.e.a graph that goes sharply up and down. On the other hand, if the pain lingers ( paroxysmal curve) and slowly fades away, the lesion is irreversible and RT is indicated. Dr Gerald Kaplan, prosthodontist, Johannesburg, South Africa

    what is the difference between pulpal necrosis and irreversible pulpitis then? how do you distinguish when irreversible pulpitis has progressed to pulpal necrosis? thanks

      Pulp necrosis is when the pulpal tissue has died completely where as irreversible pulpitis they’re inflammed in such a way that they won’t recover and need to be removed before they cause an infection. Usually you’ll with pulp necrosis, they will present with a history of pain in the past to cold and hot which was spontaneous which is no longer present.

    A question , if u are suspecting diabetes in a patient , ll u do scale / clean or ask patient to get blood test done first to check blood sugar levels. Thanks

      If it's a routine scale and polish and the patient has had a meal recently then I would go ahead and carry it out. But also encourage the patient to see their doctors for blood tests 👌🏼

I would trust your dentist and follow their instructions. I hope everything with your wisdom tooth has been resolved or is being resolved. I know they can be a real pain

      Take an educated guess when you do you BPE👍🏼 or you can count how many surfaces bled for an accurate answer but that’s a bit long to do for every patient
