  • I had an appointment with my new Endocrinologist (Physician's Assistant, but close enough), so I thought I'd film another "Day in the Life" video!
    Watch me go through a day of meals/blood sugars, debrief on what I thought of the new Endo, go low quite a few times, take my puppy for a walk, and decorate my apartment for CHRISTMAS! :D

    went to a Diabetic Support group last night felt really bad afterwards learning about how much went into maintaining my blood glucose and how much of my life will need to be devoted to that.... When i woke up i saw your new video was uploaded..... it helped a lot.....

      As I mentioned above, diabetes is a one day at a time disease, mess up today do better tomorrow. Diabetes is your new best friend so get used to having it around. Do you manage by injection or an insulin pump? Having the proper basal or background insulin level is crucial to good control. I was fortunate in the mid 90's to meet the person that wore the first insulin pump. He stated without having the correct basal insulin delivery you will never be in control. Our new fast acting insulins behave life slow, long acting insulin in basal delivery profile. John instructed us to fast for 6 hrs and watch/record our sugars every hour. When the test starts make sure no bolus or insulin injection was given 4 hrs before that. This is 10 hours for just this 6 hr window. We then repeat 3 more times so we have a record of how your body is behaving over a full 24 hour fasting time. Watching only how basal insulin is acting is key to excellent control. Now for the fun stuff. You have made a record of what is happening to your sugars. Now if sugars are dropping at 10 AM you reduce the basal insulin delivered 4 hours before. Your adjustment would be at 6 AM for you to see it at 10 AM. Apply this over a full 24 hrs. If you are crashing THIS IS DANGEROUS!!! STOP THE TEST AND EAT.
      This is a laborious test to perform. What you will get out of it is adjusted basal insulin that will keep your sugars nearly flat line and a basal profile that mirrors your bodies need for insulin. It is not constant over 24 hrs. Early morning you require less. Around 4 AM HGF hormone released while you sleep will make you less sensitive to insulin requiring more.
      Factors are: 1) correct basal delivery 2) insulin sensitivity 3) correction factor
      I wish I could talk to you about this and how to perform each of these tests. Many clinics are not even capable to understand, perform and interpret the data.

    I love those steamer meals!!!! I've found that they don't mess around with my blood sugar readings as much as other frozen meals do. :-)

    I have changed Endos because my amazing lady has moved to the public sector along way away :-( So I'm meeting my new one in Jan so a bit nervous but i hear shes awesome so a bit excited too. I'm starting on the animas vibe pump Feb 3rd!!! Feel like its Christmas!! PS. You're pretty awesome too. I was diagnosed a t the beginning of my uni degree as a RN March last year as Type 1. Very lucky as I learnt about diabetes at uni so put my symptoms to my endo (also have hypothyroidism) two weeks later at age 27 diagnosed. You vidoes have helped me a lot! So thank you!!!! :-) x

    I love your videos and your attitude towards t1d.. i wish i was in the US, here in Spain things are so complicated and always one step behind..anyways I'm looking forward to getting at least a CGM..who knows maybe a pump.. There is a lab that sells the Dexcom CGM .. cheers!

    I'm in nursing school (just passed my first semester!) so it's hard at times to get doctor appointments in when they are needed. But I'm going to see a new endo next year. Pretty excited about it with the only downside being that the wait time to get in was three months. Kind of worried about being in your situation next December when I graduate with having to move for a job and setting everything up again.

    Good for you in "connecting" with your new PA. I am T1 for 53 yrs. My Endo is extremely good and we think along same lines for management and problem solving. A1c is 6.1. Used to be 5.8 so WE decided to back off a bit and raise target Bg to 105. Dr Jones also got me approved for Dexcom G6 which has been a game changer.
    Watching your food choices made me cringe. Bread carbs & jam? If sugars are high use correction in place of carbs for that meal. ALWAYS have protein and fats with carbs, no hydrogenated fats. Use butter, natural peanut butter, cheese, oils as your fat combined with carb. This lowers glycemic index reducing spikes Eat the fat on your meat as this helps support nerve & hormonal health. My cholesterols are normal. Trying to manage with only insulin I was running 330 years ago. VLDL are transitional lipids created by insulin storing glucose as fat. They are plaquey, nasty lipids and over time cause serious problems. KEEP THAT # LOW!!
    Diabetes is a day at a time thing. Don't let it beat you up. Good spirits, clear thinking and good choices make US the WINNERS.

      Apparently cholesterol is actually beneficial for diabetes management. Insulin receptors require cholesterol to function. If you have high BG then unfortunately you most likely had glycated LDL and probably triglycerides. I am a type 1 diabetics in a really low carb diet using Tresiba and R insulin and my HbA1C last I check was 4.8. I recommend you listen to Dr. Bernstein on CZcams he has great tips on reducing diabetics complications and achieving a metabolic balance. Proper lipids and normal BG!

    Even though we have little in common except T1D (I'm 73), I appreciate your positive attitude about treatment. I put a lot of the info you publish into practice. Since you started using the Animas Vibe Pump I've wanted to rid myself of the Medtronics pump I've been using for the past 3-1/2 years as it doesn't feature CGM and I've been gathering a lot of info on the Dexcom CGM built in to your pump (one of my daughters has a good friend working in Portland at Dexcom). As a recipient of Medicare, it's impossible to get CGM without paying cash out of pocket. They pay for the pump but not CGM. However, after absorbing this latest video it seems that you are continually having to re-adjust the CGM to your own finger stick readings. My impression is that the CGM is very far from accurate. I've been averaging A1c levels of 6.8% for the past two years without CGM. Congrats on your new job!

      +Larry Post The CGM only has to be calibrated every 12 hours, I just calibrate every check because I figure, why not? The day that I recorded this video, my CGM was off more than it usually is. CGM's aren't perfect, but I would never go back to not using one! It has helped me so much!!

    How helpful is the cgm? It seems like it is way off from your actual blood sugar numbers.
    My daughter is getting a pump soon so I'm trying to prep:)

    Hello Danica. I'm looking for a Endo. I'm a T2 and mostly in control. However I think it's time I step up my diabetes management. In one of your videos, you were walking around DC with a friend from South Africa. I don't know if you live in DC or just visiting. However if you are in the DC area, what Endo do you recommend? Thanks.

    Danica, how can you have chocolate and take insulin into account, i didnt think you could convert sugar into insulin or is it like carbs? I'm very confused, by the way im a type 1 myself.

    I'm amazed at the carbs you eat, you really don't seem to gain weight like crazy like I do. I have to keep it under 100g carbs a day or it gets a bit much on the weight gain side. Like I haven't had bread in so long. Any tips? Thank you!

      I know you were asking Danica, but what works for me is exercise. I try to exercise at least 3-5 days a week.

    Hi Danica! I don't have a CGM but I was surprised that it was quite a long way out on many of your readings during this video. Is that normal? I thought they were supposed to be pretty accurate?

    I got Diabetes Type 1, Minimed Medtronic 780G insulin pump with Guardian 3 sensor. And I have free jobs at home, but no paid job because I live in an extremely economically depressed area. And i have an Endocrinologist Dr. Pol Darras and he is very knowledgeable about Diabetes and how to help them. And my sugars are a lot better now. My sugars rarely go above 15mmols and it comes down really fast. God Bless you. May we pray for the cure for it to come to us soon really soon.

    Im so confused when your sugar was 194 & you took a unit of insulin. i dont have a insulin pump, but I do have diabetes. i have CF (Cystic Fibrosis) related diabetes. (CFRD) but i would say im type 2... I NEVER take insulin unless Im eating, cause I drop super low.. Can you explain why you still took that unit?

      +AmbersRoses That's a little high for a type 1. As a type 1 myself, its really normal to take a unit or so if you are high or getting higher probably. If I had that number and was going on a walk then no insulin cause I would probs drop. But if I was home just chilling on the couch I would correct. As type 1 diabetics we kinda have a rollercoaster going on with diabetes daily. Hope this explanation helped.

    I don't know much about type 1... your Cgm seems to be incorrect a lot. how do you know what meter is right??

      I have a cgm and it doesn't have to be exact but if it is in 100 numbers its pretty good.

      +Elizabeth Ritter CGM tests sugar in the interstitial fluid. Glucose meters test sugar in the blood. So the glucose meter is treated as the accurate one, and you calibrate the CGM off of that. The CGM was less accurate this day then it usually is for me.

    DD, Wow! you are brave beyond brave.... this is the kind of day none of us want to talk about. And it feels to me like being in a fog and being smothered. So this is what is the double bad about lows. When going low, I also get stupid(er) and can start to "stack" mistakes. You illustrated that so beautifully in this vid..... Well done!

      Because your a1c is too high so it's not in your best interest. You have to earn an insulin pump.

      +Jose alberto Herrera How long have you had diabetes? I'm so sorry, you must feel so crappy all the time. If your A1C was that high while you were on an insulin pump, consider these two likelihoods:
      1. Do you experience pain or skin issues when using an inset? Which kind of inset do you use to deliver the insulin? How often do you change sites? Do you experience heavy bleeding, blood in the cannula, or other issues that make injecting an inset difficult?
      2. This is far more likely and unfortunately the norm: your doctors and diabetes educators are doing a poor job teaching you how to manage your diabetes and use an insulin pump. There are many nuances to using an insulin pump that require at least periodic experimenting with a continuous glucose monitoring system to fine-tune. I've heard the mantra that you need to earn an insulin pump before, and it's more of a cop-out excuse diabetes 'teams' use for slacking off in educating you. I'm not trying to disagree with the previous poster, chances are a professional told them about needing to earn a pump, but if you have type one diabetes and your pancreas does not produce beta cells to regulate blood glucose, that earns you the most efficient way to manage your diabetes, which is an insulin pump. If your doctors and educators get defensive when you ask questions, don't answer emails or phone calls, or give you advice that doesn't make sense, ask your parents (or do it yourself if you're over 18) to find a new endocrinologist and diabetes educator that your insurance company covers. If your doctors are causing you to experience poor health and not doing their part you don't need to listen to them. There are very good doctors and diabetes educators out there that care about their patient's health and you deserve to be treated well. It is your body and your health.       +courtney courteau but there are major major differences. He doesn't take insulin because his body still produces insulin. A type 1 diabetic doesn't produce any insulin.
