Thank you for all your videos! I just had my 16 year old implants removed because I wasn't happy with the aesthetic anymore & I just didn't find them comfortable anymore. Initially I wanted a lift as well, but decided to let them settle first as I was not keen on the additional incisions. Your videos have confirmed it was the right decision for me to wait a little longer.

Hello, thank you for the informative video. What are your thoughts of breast lift with fat grafting, instead of implants?
It certainly can be done but I prefer to use an implant as I like the shape better, the implants are super consistent and reliable and don't change so I can get a great result every time. With fat it is a lot more variable but my partner, Dr. Careaga does a lot of breast fat grafting and has great results.

How would I know which category am i in?
It can be a little difficult to figure that out by yourself. I would see a plastic surgeon near you and see what their recommendation is.
Can you possibly do a video on a breast lift no augmentation. Because what about people who like the size of their breast but just would like to change the shape and make it perkier not bigger or much smaller either.
This video might help you. It is removing the implant in a patient that was too big and then performing a breast lift without an implant.

You can use any type of implant when performing a concentric mastopexy and a concentric can be used in patients with previous implants. It is quite common actually to perform an implant exchange and then also perform a concentric mastopexy at the same time. Frequently patients who have had a previous augmentation and then had children or just naturally with time have developed some minor droop making them a good candidate for a concentric mastopexy.

I know you talked about doing lifts with augmentations. Are lifts typically done with reductions as well?
A reduction includes a lift. We lift the breast and make it smaller--that is a Breast Reduction. A lift with no implant is just a lift (mastopexy), and a lift with an implant is called a mastopexy augmentation. All are lifts but a reduction makes the breasts smaller, a lift keeps the breasts the same size and a lift with an implant will make the breast larger.

Once a lift settles it is a very natural look.

There is no danger to have a reduction and an implant.
Does a lift and reduction also reduce band size assuming there is some lipo? Or is it not significant enough?
No that is a reflection of your thorax--your bones and is not changed. If you have a lot of tissue on your back or upper abdomen or on the outside of your breast than with lipo it can decrease in that situation.
Which incision would work if you are in the middle of a normal and type 1?
Usually a concentric works in this scenario.
Do you make the areola smaller during a lift?
Yes. Make the areola 42 mm in size.
I had wide and large breasts at a level 2-3 droop, I got a breast reduction + lift, you could not remove all the excess areola skin as some was needed in order to close the incision. Pigmentation was to be expected along the vertical incisions due to this. After healing, are there any ways to lighten that areola pigmentation along the vertical scars? I’ve heard laser can help lighten areola pigmentation, and I’ve heard tattoo can color over the pigmentation in order to match the skin color of the breast. But what would you have to say about it? Thank you
Those are both good options to help with lightening the pigment. Occasionally after a few years there is enough skin and it can be removed surgically.

Unfortunately there is no exercise or other activity you can perform to treat drooping. You don't always have to use an implant--only if you want to be larger than you can use an implant but if you want to stay the same size then a lift is all that would be needed.

I have naturally large breast 36DDD and I love having large breasts but I definitely need a lift. One I have the lift I still want to have large breasts but I’m not a fan of the “high profile” implant. Do you use the moderate plus profile implants at all? It has more projection than moderate but not as much as high profile? Your videos are phenomenal 🎉
Yes I use HP and MP implants. I never use a low profile implant though.
How do we pick the good fit implant with our size??
I usually suggest getting the implant that matches your breast. That's how you get the very best shape.

For someone who is stage 3 can you do just a lift and reduction to the smallest size that’s acceptable/safe? ❤
Yes. That's a breast reduction and we can make the breast as small as we can while still preserving enough volume to keep the nipple and areola alive and get a pretty shape to the breasts.
How long does the implants last? 10? 20? 30 years? And can you get this done with a tummy tuck? I need a mommy makeover. Thanks!
You can do a tummy tuck with a breast lift with implants and that is considered a mommy makeover. There is no exact length of time an implant will last per se but we know that at ten years about 8 out of a 100 patients will experience an implant rupture.
Dr William... my sister has a question she broke her foot but doing well just aches and some swelling now and she has a had a torn ACL with swelling for a year now goes to rehab she has no pain so opt out of having surgery bc it doesn't bother her. Can she still have plastic surgery such as Lipo and tummy tuck?
As long as she has never had a blood clot there should not be an issue provided she is otherwise in good health.

Same. I have a history of keloids and hypertrophic scars. It makes me wonder if it's worth it.
What about for pseudoptosis? Where the nipple is at the IMF line but the breast is deflated from weight loss?
For patients with significant pseudoptosis I also perform a lift. In patients with mild cases a simple breast augmentation can suffice.

Yes! If you have ample volume for a lift then you just can get a lift alone and not get an implant.
Is the implant required?
IF you want to be larger than an implant is needed. If you just want your breast higher and a better shape than a plain lift is all that is needed--just a mastopexy.
What about tuberous breasts? can you do a video like this about that? I want mine to get done but can't find the info available online to know

What if you’re a 1 with larger sized implants? But want to go for a more natural look with a smaller implant. Would you need a lift along with the new implants? Natural breast size was A cup.
You may not necessarily have to have a lift--it depends from patient to patient. I would say that it is more common to need a lift after downsizing than not but certainly not everyone wanting to decrease their implant size needs a lift.

Not required to wait until you have had all of your children before having a lift. Some patient's breasts look fine after having a lift and a child while other patients who have never had any breast surgery can experience a lot of changes after having a child.

What about a lift without implants???
Yes you can do a breast lift without an implant as long as you have enough volume of your breasts to allow it to be performed and you want to be the same size as you are currently.
