@Breathlless as shown in the video, the plaque builds up under the first inner layer of the blood vessel, so to speak.
Damaging that layer to scrape off the atherosclerosis causes thrombosis, internal bleeding and most of all a whole lot of cholesterol embolisms, we don't want that :)

It can. but that is a different procedure, called an endarterectomy. It is surgical and thus more invasive, more expensive and there are more risks involved. Depending on the situation, a PTA would accomplish the same job.
