Unfortunately, most all of these recommendations are not appropriate for those with Parkinson's Disease Dementia/Dementia with Lewy Bodies. Dopamine agonists often worsen cognitive symptoms to the point that it may induce episodes of profound delirium and psychosis. DBS is contraindicated in people with PDD/DLB, not to mention the fact that general anesthesia is also contraindicated in these patients. Anticholinergics are a big no-no because, as was said, it increases hallucinations and delirium. I find it also increases the frequency and severity of Restless Leg Syndrome. Botox is not covered by insurance for this purpose in the U.S. either. And those with advanced PDD/DLB often need quetiapine to manage disruptive hallucinations and delusions and so discontinuing meds for this purpose would be counterproductive in the overall management of patient comfort. Lastly, the excess daytime sleepiness in PDD/DLB patients would make the use of audio swallow reminders not particularly viable, nor would that help those that drool excessively mostly at night. It's very frustrating that there are virtually no options for PDD/DLB as compared with Parkinson's. Hopefully, at some point in the near future, there will be interventions available for this particular patient population.
