  • I want to share another patient with elevated AFP diagnosed via routine healthscreening. AFP is a marker that we use for monitoring progress, response to treatment, recurrence for ovarian and liver cancer. We do not normally do it for the general public
I share another case of elevated AFP

Hi Doctor, do you have any useful articles that mention the changes of the afp levels from 15 to 8 range in the laboratories? I couldn’t find much information about it. Thank you

Hello Doctor your presentation was very helpful.
I have a less than common condition that as an adult turned to cancer. I was born with one undescended and one underdeveloped testis. At age ten the undescended testis was surgically brought down into scrotum. Clearly the surgery was more was done to me as my abdominal scar was far too big for this type of surgery. In addition there were indication from the staff that more was done to me during the week that I was in the hospital.
As I grew up my puberty was delayed until I was in my late teens. I tend to be asexual but did marry in my mid twenties. I had mostly a normal life with my wife. Sexually I was small and under developed a little and always had gender dysphoria like issues which made sexual relationship hard. But my spouse and I made our marriage work. Starting in my early twenties I started feeling as if I were pregnant and my wife and I would often laugh about it.
It took many years but we managed to have two children but a few years after our daughter was born my previous undescended testis started to grow. I went to the doctor right away. I suggested a alpha fetoprotein test be done and he refuse. Month after month I went to doctors and no blood testing was done finely this testis became so large it really needed to be removed. even then it took months for anyone to act. The surgeon removed the testis though my lower abdomen while I was wide wake. The moment he cut me open a 10 cm malignant germ cell teratoma poped out of the wound. This was attached to a immature small testis.
The alpha fetoprotein test I asked for nine months before was at last done and it was 10200.
Two days later x-rays found another mass in my upper abdomen and it appeared to be 17.5 cm. A urologist came down from another city and he said I was at least stage three but to be honest her thought I had only a few months to live. He also indicated he was in contact with the Mayo Clinic and a surgeon agreed to do the surgery to remove this new tumor after four rounds of chemotherapy.
At the Mayo a six hour surgery found and removed this mass. Interesting was this particular mass was a nonmalignant advanced germ cell teratoma. It was so huge that it contained many cellular tissue of a developing fetus. Eye, skin, hair, bone, limbs and such.
Interesting was the feeling that I was pregnant went away. This was something that was in me for 45 years.
The only thing that never subsided was my gender dysphoria. something I deal with to. this very day. As a child I never saw my medical records and it was that during the first of my cancer surgeries they discovered that several teratoma were remove when I was a child.
To this very day I deal with high alpha fetoprotein levels. In addition it was suggested I carry the genes for both ovarian and breast cancer. I must now go in for a mammogram.
As you can imagine Ive had extensive surgeries that cut away bits here and there. My under developed testis has never word all that well. In addition my estrogen levels are a bit high.
Deep down I would like to take a low dose of estrogen but sadly that would put me at risk of bringing my cancer back.
In as much as ive always had high levels of alpha fetoprotiens would taking even higher dose of AFP mimic the effects of taking estrogen but without the side effect. What would the effect on the body be? What would the side effects be? Do you know if anyone ever done this before? I see myself as being Bi gender in the very real sense of the word. My spouse has always been supportive of me she knows that I would do nothing to harm her or myself.

Hi Dr. Desmond, i also had my AFP tested since 2008 at 5.3 and it has been rising every year and this year it has doubled to 10.6. Doubled in 15 years. My liver function test is ok and i don't have chronic hepatitis disease. Should i be worried?
