    Very informative. I stumble on your video looking for SHBG. I'm a subscriber now. Ok one thing that always comes back in most of the videos I watch is" talk to your doctor before doing anything". Fact is when you have no family doctor, in Quebec, they put you on a waiting list and if you have no severe condition it will be anywhere between 3 to 7 years before you get one. And still that dictor might be a 100 kms away from your home. So what do we do? Trials and evaluate the response by the body reactions, simple as that. I auto-evaluate everything I do and watch as many video as I can to educate myself. Thanks a lot. You're on my list now. I'm glad I speak more than one language.

As a licensed healthcare professional, it's important from my perspective that I encourage people to speak to an *experienced* HCP who knows how to use these tools and monitor you appropriately. In Quebec my understanding is that the options are certainly sparse, but continually looking for the right match has to be the goal. A biased suggestion is to seek out a Naturopathic Doctor. In Quebec there is no regulation for NDs, but the good ones will hold and maintain a license in a regulated jurisdiction (usually ON), so you can differentiate them from the less experienced and poorly trained "naturopaths" in Quebec who have had minimal training.

    Longjack has been shown to lower shbg and that is how it gives its effect. Stacking it with fenugreek is really a beautiful combo. Fenugreek can control increases in DHT, which I found can give a nice effect on mood.

    Doc - thank you for this awesome content! Med free but total is low -278, Bioavail is 111 and free at 41.9. SHGB is 46 and Albumin 4.6. I train something almost every day and feel pretty good but just wondering what to interpret from my levels. Would love your opinion. 58 and been training for damn near 45 years, DL’s, KB’s Rucking, etc. thanks!

    I know you mention that zinc may lower SHBG but wouldn't preventing conversion of testosterone to DHT be undesirable for some if they are looking to increase libido? I don't have male pattern baldness and my prostate is fine. I'd be curious to know your thoughts on this. Really enjoy your content thanks!

    Hi Doc, appreciate your work. I was given a month-long course of antibiotics which, in my opinion, wrecked havoc on my body. It also kick-started an autoimmune disease that I have finally managed to take control of. Since then, I've been monitoring my bloods. I was wondering if you could give insight to how high prolactin & progesterone levels could affect overall vitality?

      Most relevant: Elevated prolactin will shut down testicular production of testosterone.

    Hi Doc, I hope your channel gets bigger, great info overall. Fellow 🇨🇦 I wanted to ask you. I’ve been on TRT (175mg/ week) for close to 2 years, but feeling awful lately. Bloodwork came back recently and Free t is sitting at 390 pmol/L(bottom of range), SHBG is at 59 nmol/L (top of range) and Estradiol was 107 pmol/L. Albumin and the rest of my blood markers look good though. I recently lowered my dose of test cyp to 140mg/ week to avoid having to use an AI to mange my estrogen. But I’m not sure this was the right decision, after learning that testosterone would in fact help Lower my SHBG.. what do you think?

      #1: Consult an experienced and licensed practitioner before using herbs. Just because it's "natural" it does not mean it is always safe for you. #2: Definitely make sure it's root extract. #3: Ensure it's standardized in some manner, so you can be sure that what's on the label is in the product. #4: Good human clinical studies on nettle root are few. #5: 120mg is a common starting point, but I also increase this alongside other herbs and nutraceuticals.

      This is an awesome and fun question! So, yes, some studies show an (though barely statistically significant) association with ferritin (iron stores) and SHBG. This is likely due to ferritin being an acute phase reactant, increasing in the presence of inflammation: contributing to SHBG increases. So this what I call an "upstream" issue: if your ferritin levels are high due to hereditary hemachromatosis, then regular blood donations probably wouldn't be a bad option because you're managing an issue with a root cause we can't control well. But if we know WHY ferritin levels are high, we should be digging to find the cause of that, and addressing the issue upstream. So the question is: why is ferritin elevated?

    My numbers are all messed up:
    Aug 2022:
    Free Testosterone 57pg/ml
    Total Testosterone 487ng/dl
    SHBG 75.2nmol/L
    April 2023
    Free Testosterone 65pg/ml
    Total Testosterone 656ng/dl
    SHBG 91 nmol/L off the scale
    I just turned 51 . Pretty much lost all hair on legs, facial hair thinning and not much hair left in armpits. I really don’t notice loss of hair on my head.
    Energy, Ambition and Motivation is low along with low mood. I don’t want to do TRT after things read and heard. I would like to try and go the natural route.
    I really need help as to where I should start to remedy these issues.

    Hey do you have a video for steroids for people that have had a severe physical trauma, for example my core muscle ripped and i wasnt walking for years, im still experiencing lots of pain and i read a study that your hormone levels change with a severe injury, im 28 years old and i am curious if TRT or an anabolic steroid at low dose could help? But im very hesitant because of everything you are talking about, there are a lot of risks if this isnt done properly

      I don't have a video for that. But yes, that's a thing (injuries can be associated with hormonal issues). A good physical assessment for your injury is the first thing to get done - if it hasn't healed appropriately or inflammation hasn't cleared, that needs to be dealt with. Look into other options for speeding recovery like Cold Laser, while getting your bloodwork assessed.

    prolactin lowering is huge as a man and feeling great. Schizandra p5p Ashwaganda macuna puriens.
    as a man I try to lower Prolactin, Cortisol, bad estrogens.
    I try to raise Testosterone, DHT, Good estrogens, oxytocin

    Hi, I have the following situation:
    Total testosterone: 38.4 H (nmol/L) - (reference interval 8.64 - 29)
    SHBG: 83 H (nmol/L) - (reference interval 18.3 - 54.1)
    Free Testosterone, concentration: 0.468 (nmol/L) - (reference interval 0.091 - 0.579)
    Free Testosterone, share: 1.22 L (%) - (reference interval 1.59 - 2.95)
    Bioavailable Testosterone: 31.8 L (%) - (reference interval 37.3 - 69.1)
    I shown the results to endocrinologist in my hometown, he said that everything is good, that I should do nothing about that (but he left this feeling that he didn't know much about sex hormones). He just said "free t. is lower because of high SHBG. I can give you testosterone therapy" (but I don't feel like I need one, yet). On the other side, it is hard to find endocrinologist in Croatia specialized in sex hormones (even if you find you wait for 5-6 months).
    Don't have any erectile problems, but my libido is a way lover (I'm a 32 y.o., having good diet and working out regularly, didn't suffer from any serious disease).
    I wouldn't like to play a doctor, but do you have any suggestion what should be the next steps for me to do.

      Processes that are related to increasing your SHBG are likely contributing your symptoms as much as your suboptimal f:t ratio - that's the question you should be focusing on. 83nmol/L is quite high for your age, need to find out why. Unfortunately most doctors don't have that inquisitive drive - why is this process going on??

    I am a 42 years old man, working out 4 times a week, eating healthy, hypothyroidism treated with t3 and t4, 590 ng/dl average testosterone levels, 25 pg/ml estradiol average levels. Also liver, lipid, Iron and glycemic profiles in range. The shbg levels have been in 60 nmol/l average for the past 5 years that I started blood testing due to no libido and ed. I have tried with trusted brands of boron and tongkat Ali with no success. Can a microprolactinoma treated with cabergoline be the root cause of high shbg levels? Thanks in advance

      I am not familiar enough with cabergoline to say with certainty; however, prolactin inhibits LH, which would likely lead to lower T levels. Cabergoline is a dopamine agonist, theoretically it should mitigate that effect.

    I had my tests done in 2014. The doc said my body runs on overdrive.
    What he meant was everything was high.
    High Total Testosterone
    High DHT
    High SHBG
    Bio available testosterone in 2014 = 0.3% !
    Doc advised HIIT , saunas, cold plunges , diatery changes.
    I tested recently in 2024.
    Everything still high but Bio available now at 1.13%
    Still bit to go reach 3%.
    I started boron. I can feel it works but well measure in August again...

    Long term Zinc intake will lower Copper or even cause a deficiency . A low Copper situation will lead to Iron overload or Disregulation, Anemia and stop the manufacture of Collagen, ATP and other various health issues , high iron will then raise SHBG and cause low Testosterone symptoms. Long term zinc leads to low libido, weakness, fatigue and effect sexual function long term...be advised that a balanced mineral formula which contains bith zinc and Copper is best . Zinc and minerals should not be taken daily, off and on gives best effect ...

      This is why if prescribing oral zinc supplementation, most HCPs will ensure copper status and or include 1mg for every 15-30mg of zinc.

      Oooh great question. I'm a fan of using devices like Oura ring and Whoop band to track HRV and sleep staging. Looking for at least 1.5hrs of REM sleep and at least 1hr of deep sleep is a general sleep target.

    Do you have a reference study that shows the correlation between low carb and lowering SHBG? Every published study that I have reviewed indicates the opposite and just want to make sure I'm not missing something.

      Most studies I've seen show no statistical significance. (Eg. C Longcope et al. J Clin Endocrinol Metab. 2000 Jan.) And I'll also point out that a carb is not a carb is not a carb. Quality matters. So I'm realizing when some of you are saying low-carb, we may not always be talking about the same kind of diet. Also see my comments above on low carb often being calorie-deficient. Here's an old, small n study associating high carb with high shbg: K E Anderson et al. Life Sci. 1987.

      @@contrarian717 Most of the recent puiblished studies available have shown higher glycemic load equaled lower circulating SHBG. Basically as insulin goes up SHBG goes down. Most of the findings suggest more studies need to be conducted to verify though. Personally I would suggest to follow a well balanced whole food diet with about 40% of calories from carbs.

      I love Tribulus! Over the years it's become harder to maintain a consistent quality source and it's becoming quite expensive due to it becoming more sparsely available (at high quality).

      As an antioxidant and substrate leading to glutathione production and improved liver function or at least protection, NAC might be expected to have favourable (lowering) effect on SHBG. But you need to start figuring out why SHBG is high instead of just reactively lowering it, IMO.

    I have been taking fenugreek 400mg(20:1 ratio) Equal to 8000mg powder form x2 per day. Only increased lubido and i feel maybe increased prostate size as started noticing not fully releasing urine.

      Get your prostate examined, and probably worth testing DHT. Make sure someone trained licensed and experienced is working with you on this.

You are right I have symptoms of either high or low DHT because I was examined with Benign prostatic hyperplasia slight enlarged prostate year or 2 ago and started losing beard hair (spots with no hair and turn grey hair) which looks like alopecia. I also suffered with completely emptying my bladder which is probably due to the prostate. I think this all happened during COVID19 time. Thank you

    Do you ever run someone’s total testosterone higher than 1100 just so their free T can be above 12 (this being if their SHBG is elevated) and second did you ever lower someone’s dose due to having a high free T 21< even though their total was below 1100.I know some places like to “run” free T 25 to 30 range for men. Again this is considered Supra-physiological so curious of your take on it. Sorry for the questions

      No. The problem isn't low testosteorne. It's high SHBG. Further firing up T production likely leads to negative consequences and just ignores the problem (high SHBG) and the mechanisms that led to that elevation. Find the root causes and address them.

    I am still trying to find a video to tell me what is considered a good bioavailable testosterone level. I have my lab results, and it shows my level to be 332 with the usual range being 50-190. What does that mean? Is 332 too high (bad) or super healthy?

    Proviron ( mesterolone ) is very effective in the treatment of low Testosterone and is legal and available here in the UK . 28 tablets of 25mg is around £ 25

      Well SHBG binds DHT too... There aren't a lot of studies that look at this yet though. If SHBG is "used up" binding DHT, theoretically, initially, there may be more testosterone left unbound. But, it would depend on the context. If SHBG levels are high due to another underlying issue or mechanism, the body like counters by simply driving SHBG up. Increasing DHT purposefully in men is a bad idea in my opinion anyways.

      A simple google search will elucidate concern for boron as a potential estrogen agonist - It's the reason why companies in Canada that include boron in bone/osteoporosis formulae require a warning about estrogen sensitive cancers.

    My shbg is dangerously low. I am on 200mg of trt and all my clinic tells me to do is do cardio and lower body fat. But my body fat is low already.

    Lowering carbs will RAISE SHBG levels. Look at keto/carnivore adherents and check their SHBG. Fasting will also RAISE SHBG, not lower it, as SHBG responds to changes in insulin. I would question everything proposed in this video

      It's the caloric deficit that is the strongest factor to raising SHBG in carb reduction - this is why keto and carnivore diets in the short term, will have positive benefit to free T. This is also why, if done improperly (not achieving caloric adequacy), IF can raise SHBG too. You're not wrong, you just have to consider the context of the individual.

    Do we really want to lower Shbg or reduce the binding? Shbg is necessary to reserve test availability. Nettle root and boron with ZMA are cheap and great at this. I'm 63 and can absolutely experience the difference when I stay off it to long. Yes, cycle it so they remain effective. Basically, when the month bottle is empty, take a couple weeks off the hit it again.

      To maximize bioavailable T, absolutely you want to keep SHBG at reasonable levels. I've talked about my dislike for boron in other videos on this channel, but I do like nettle root (it has to be the root) for lowering SHBG.

    19.5 nmol/l range 8.9- 28.8
    Bio available test was 9.0 nmol/l range is 3.9 -11.2
    I take zinc, magnesium an vitamin d3 .. omega 3
    An I use to cycle boron for 2-3 weeks then get off it ab repeat
    I also had Ashwagandha I use to do
    I have test supplement with Ashwagandha an tongkat ali but turned off from longjack

    🤔 *I was searching for something that could increase my Free T, I was running around 70 pg/mL* I took Tongkat for a year then tested I hit 122 pg/mL, possibly a fluke.       Age 17 225-230lbs 6'3 18% bf 1993 "Doctor put me on Cyproheptadine" which helped weight gain. Was 130'slbs prior 3-4 years prior.
      212 Total Testosterone 2006 age 30 Prompted by doctor to try TRT gel. All lifts were still good and did not reflect low T. Grip strength in 200's lbs.
      183 Total Testosterone 2006 TRT Gel prescribed for 2 months my levels went down. Got off Gel and focused on diet and training. Roughly 235-240lbs 18% bf.
      =========WASHOUT========= 9 years
      122 pg/ml 2015, "TRT GEL 2 months" Did not like the feeling at all boosted anxiety also palpitations.
      7519 ng/dL Total Testosterone must have had error by ZRT Labs.

      Ashwhaganda is awful awful for the liver. I had 32 enzymes in June of 2023. Took Ashwaghanda all summer into Sept. Oct had a test. 82 enzymes. Dont do it. It damages liver. All day

      @@scottp6761 Ashwagandha is one of the safest herbs in the repertory. But, like any medicine, if you use it inappropriately or get shitty quality (this especially happens in the US where regulation on nutraceuticals is near non-existent), it can do harm in the wrong context. This is why you don't take shit without talking to someone who knows what they're doing.

    Hello and thanks for the video.
    I took blood sample to check my t-levels and here are the results:
    Total T: 32 nmol/l
    SHBG: 89,9 nmol/l
    Free T: 3,2 nmol/l
    Both my total test and SHBG are over the normal limit.
    Free T is low, but not under the limit.
    Should I visit endocrinologist?
    I’ve lost my libido and feel like shit .

      How old are you? Those SHBG levels are definitely not ideal. Yes I'd suggest you find a practitioner to investigate and discuss options!

      One of the ways mesterolone works to provide short term, weak androgenic effects is to competitively bind SHBG so that more testosterone is left free/bioavailable. But this is an example of bad medicine in my opinion - instead of asking the question “why is SHBG elevated”, you’re treating the symptoms, not the reasons symptoms are there. In this case, mesterolone won’t decrease SHBG - it will lower the impact of SHBG on testosterone, and then the body typically compensates by producing more SHBG. It’s also an illicit substance.

      @@johnnick2512 Of course it will be high if you are on keto, and it will stay high if you stay on keto, as for carbs between 120-150 grams per day is optimal and your SHBG will gradually drop with time! Good luck 👍🏻
