    I had lower back problems for for 30 years. Life long truck driver and just dealt with it. Tried lots of different things to relieve pain. Six years ago I woke up in the truck in SEVERE pain. Took me 2days to drive 500 miles home. Had to go to hospital in an ambulance. Diagnosed with spinal stenosis. Had surgery where 2 titanium shims were installed in lower back. Woke up to absolutely no pain. Drove trucks for 5 more years. Just retired at 75 ! In 2000 I had acoustic neuroma surgery at UM Jackson memorial. Dr Roberto Heros.

    Thank you so much for all the information. I would love a video on recovery from some of these surgeries especially the one for spondylolisthesis. It sounds pretty major. So things like pain levels time to recover, etc.

    After 3 years of chronic burning Sciatica pain in my groin,thigh, calf and feet visits to three doctors,a Chiropractor,CT scans,MRI,X-Rays was diagnosed with Spinal Stenosis.A laminectomy operation was discussed , researched studied at great length.My right hip surgery will be performed first .I don't suffer from tingling or numbness,in legs.30 years of CP Rail riding box cars & engines,poor seating on diesel engines for years, truck driving working underground in mines slouching on couches,playing hockey definitely contributed to my sciatica pain

    Thx for ur videos. I can no longer walk longer than 15mins. Leaving Kaiser bc for just 1 example: 1 Xray only. Im getting a shot & was hoping for an epidural yet Im getting cortisone. Ive got a friend who got her disc shaved & bad side effects for her. Guess that's 3 but I'll stop at 3. OK 4th broke my shoulder so they told me I dont need to use that arm. Kinda do, Im rt handed & rt leg gets sciatica.
    Spine surgeon in Tx scared me more than any Stephen King book.
    Glad ur mom is top priority. She must be so proud! STAY SAFE & HEALTHY💜🙏💜

    Thank you for sharing this information. It has helped me with understanding what is going on with my L4-L5 spondylitis. The surgery I have had injections and will have another next week. Surgery has been discussed and I have been dragging my feet due to my husband’s health issues. I now think I may need to rethink the timing of my own quality of life. I can’t help him if I am not physically able.

    Thanks for the information. I did a fusion surgery years ago. I was pain free for one year. Since then I’ve suffered with sciatica and now I’m having numbness and burning pain in my toes, calves, buttocks and genitals.

      A patient came to a spine doctor in a wheel chair, he had surgery and the doctor promised he could walk again, the second time he went back to the doctor still in a wheelchair and asked the doctor, why he couldnt walk again, the doctor said, he did everything but he failed, the patient was in disbelief, then pulled a gun shot the doctor to death then killed himself in the doctor's clinic.

    I had surgery on my L4-L5 discs in 1992.(no fusion) I had a great outcome. By 2009 I was having quite a lot of pain again and had epidural steroid injections twice at Hospital for Special Surgery in NYC. They were like magic and even cured my restless legs syndrome.

    Thank you for taking your time to educate us on all the treatments available. I just had complete fusion from L6 down to my hips. I'm 3 weeks out now. It's starting to feel much better this week. I had caught aquena prior to this fusion too

      I had Cauda Equina Syndrome in 2018 and ended up having emergency spinal fusion the day it was discovered. Anyone that says anything derogatory towards people in pain, truly don’t know what it feels like to be in pain. It’s so frustrating and debilitating to suffer chronic pain. Mine stated at 27 years of age, and I am currently 55. It has finally beat me down, especially after my recent fusion surgery. Take care.

      @@user-ny7cs2dx1g I have been having the same issue since 2009-2010 and I am 39 years old . I am just afraid to have the L5-S1 fusion but it seems like this is the only option I have left as PHYSICAL THERAPY and other conventional treatment does not seem to help.

      I've had 20 surgeries 4 back surgeries the last one was a 2-day surgery 9 hours and then 7 hours in the operating room, I have 16 screws in my spine , then one screw in my left wing pelvis, then one screw in my right Wing pelvis to hold everything together then 4 Discs replaced they had to cut me on the left side to replace the four discs, and they had to move organs out of the so they can get at replacing the discs . They also had to put two 14" Rods to hold all the sixteen screws together and there's more I have four to six screws that are loose now especially ones that they put in my and now no surgeon wants to help me they're afraid to touch lots of pain

    Unfortunately you are not in my insurance network! Baylor sent me to a Neurologist who surprisingly I liked what he had to say. Had MRI Friday. Step by step 🎶

    Dr. Donnally, thank you kindly from a Western Canadian. Your presentation was very informative. I had 3 cages inserted at the C4-C7 levels in October 2015 and 3 at the L2-L5 levels in July 2016. Due to ongoing pain I received a Medtronics stimulator in June 2018. The device has provided limited relief and I experience chronic pain across the lower back, down the left leg and into the left big toe. It feels like I have had a permanent,Charley horse since 2015. I would like to know what my problem is and can anything be done about it and was recently referred to another neurosurgeon for a second opinion. He ordered X-rays , CT scan and MRI. He has since moved on unfortunately. The Xray showed degenerative changes , the CT scan shows severed neural foraminal narrowing from T12-L1 to L5-S1. I am on the MRI list and am told that the wait list is 8-9 months.

    I had a Spinal Fusion a year ago (11/2022)
    1. Bilateral L4-5, L5-S1, S1 Pelvis posterolateral fusion 2. Bilateral L4-L5, L5-S1, S1 Pelvis posterior instrumentation,
    3. Bilateral L5-S1 transformational lumbar interbody fusion
    4. Right sided L4-5 transforminal lumbar interbody fusion
    5. Left L4-5 synovial facet cyst excision.
    6. L4-5, L-5-S2 posterior column osteotomy for deformity and foraminal decompression.
    7. Bilateral Spinopelvic, fixation, 8. Application of biomechanical interbody device x3.
    9. Use of moralized bone autograft.
    10. Use of allograft
    11. Use of a surgical microscope
    12. Intraoperative neuromonitoring including SSEP, triggered and free run EMG
    13. USE of fluoroscopy for level localization &hardware placement.
    Modifier 22 applied in this case due to presence of extensive epidural adhesions which required extensive meticulous dissection. The presence of a high grade II spndyloidthesis & high pelvic incidence also required extra preparation &dissection in order to decompress & stabilize neural elements. In all this added approximately 2 hours of additional time to the operation.
    1. Pre-op Diagnosis;
    Lumbar spondylolisthesis
    2. Lumbar degenerative scoliosis
    3. Lumbar foraminal stenosis
    4. Sagittal imbalance.
    Post op: same
    Blood Loss: 400cc
    Now: 1 year later, I'm suffering from severe pain especially on the right side but some on the left side running just below my waist area down through my butt cheek and leg to the knee. Below the knee on both legs, it is numb. I feel pressure but no feeling to the touch with a pin or finger light running down the leg. I am on a cocktail of gabapentin 300 twice a day, Flexeril 10mg 3 times a day and Norco 10/325, 1 every 6 hours as needed. I also take Ropiniole 2mg 3 times a day. The pain is really handicapping me ... I'm not getting any relief unless I keep myself in a drug induced stupor. My surgeon just had an MRI and X-rays done. Everything looks good except, I have "two screws loose." Those are the two that are anchored in my pelvis. Could they be causing this much pain? On a scale of 1 - 10, I'm 15! I have always had a very high tolerance of pain but this is really kicking me down the road! At 72 years old, I don't feel that anyone should have to feel this much pain. I am desperately hoping you may have some ideas. I listened to your presentation and was impressed...so I'm reaching out for your thoughts. Thank you!

    Doc, your ok, good man, a question from the gallery! 30 years of low back pain, traumatic accident, symptoms have increased tremendously in the past 5 yrs, 30 years of pain , just had a new mri losing ability to walk more than 50 feet , leg weakness, heavy pain low back with radiating pain down legs, also some numbness in the pelvic floor, and you know what I mean, just don’t want to say in this forum, stenosis all over the lumbar region, have had many injections with short term relief

    6 years ago I had severe spinal stenosis.... after failed physical therapy I went to a neurosurgeon who operated and did a laminectomy and inserted a coflex device between L4 and L5......I was up and walking the next day......solved my problem IMMEDIATELY......still have some arthritis in my lower back but it is nothing like I had prior to the surgery......

    I've had a multitude of injections, several rounds of physical therapy, radio-frequency ablation, traction, 3 different fusion surgeries. NOTHING has helped. Spinal stenosis, spondylitis, spondylolisthesis, etc... So tired of being in pain. I've lost my life and am now on ss disability and medical retirement.

      I feel your frustration. I’m a 28 year chronic pain sufferer. I have had 6 back surgeries, with 3 being fusions. I highly recommend getting 3 opinions if you choose to get surgery. I’m in the worst pain in 28 years after recently having spinal fusion surgery. Do your research.

      After having taken humongous amounts of steroids from an early age, I now have a systemic problem. Steroids are poison… they eat all of your cartilage. I have found just recently a very fantastic physiotherapist who understands anatomy extremely well, and it’s not afraid to get in there, and he has made a big difference for me already. Do not give up hope and keep looking. I also have PRP injections in Mexico, where they don’t cost so much for my shoulders, which the doctors have told me I need to have either both of them replaced or Cortizone shots for the rest of my life. PRP has helped immensely. Good luck!

    Thank you for this video, I've not been able to feel my legs from just below the knee for going on three years, I have spinal stenosis, mid to severe, a herniated disc, and bulging disc, my latest MRI says I have legion on my liver, scoliosis, spondylosis, and my first nerve test, where they poke all the needles in you from spine to toe, show/ states all kinds of scary never damages. Was given a home PT, she said she's never seen someone left in my condition, ever! and she's I think a bit older than me, or at least close to my age/ early 60's. I was a caregiver till my spine went out, healthy as a horse, my primary says surgery is not off the table, and the surgeons in my town think they are God, and don't like my insurance, I'm to heathy to give have to live on the floor anymore, yes I can't sleep on a bed, it has to be as solid as the floor, if my spine deviates at all, the pain is to powerful, I can't more past it, I have to surrender to being stuck, most of the time, so I've had to sleep on the floor for years, I want to go back to work so bad I didn't apply for SSI until 5/6 months ago because I saw the results of my all my recent test, it's permanent as it was let go to long, this was probably one of the reasons SSI immediately awarded my SSI, I've been like this since 2020/2021, furthermore I don't understand why this is a ongoing thing with services making me have to prove every 6 months that I qualify for assistance, these prescriptions are destroying my liver.     I am 78 years old and had Spondylolisthesis 45 years ago when I was only 33 years old. I had a spinal fusion in the coccyx area. Apparently I had a vertebrae missing and had bone harvested from my hip and this was used to fuse my spine. I lay for 9 weeks on a spinal bed and then had to wear a special corset for some time. I now live in Canada, Scotland was my home at time of surgery, and I have now been diagnosed with neural foraminal stenosis in the lumber area. I am awaiting a consult with a spinal surgeon in Toronto. Am I too old now for spinal surgery considering I previously had spinal surgery 45 years ago. What would you advise. MRI diagnosed this condition.

    I have spinal stenosis, degenerative disc disease, osteoarthritis and ank-spondylosis and levo scoliosis. Not a fun back at all. Had L3-5 laminectomy (L-5S1 was gone just destroyed no bone) had so many bone spurs they didn't need to take any hip bone to repair my spine just used the bone spurs. I experienced a couple years ago almost a paralysing experience standing and it felt like a tens unit turned to 1000 an it was in my middle spine and I almost hit the floor at work very scary feeling to lose your feeling in a second of your lower body. Now at 62 my neck is shot and my levo scoliosis isn't just levo it's both left and right in my mid back. I've learned to tolerate the pain of my bones and burning of muscles being pressed upon. It also involves my ribs as well. Now with neuropathy in my feet and throughout my body I litterly feel the electric shocks and burning of nerve endings especially in my feet an neck area. An believe me when you actually hear your nerve endings zapping and then litterly just hear zzzzzip and they litterly burn an die it's something I wouldn't wish on anyone.

    MRI laying on my back , the result was severe stenosis of C5, minor stenosis in C4. No tingling , no sciatica pain. Hurts to turn head to the right. Scapula area on right side feels like it hits a rib or adhesion is there. I also have several gall stones at 33cm or mm can't remember. I learned that the pain in shoulder and right side of neck can be caused by gall bladder pressing on the phrenic nerve causing my pain. I am afraid of neck surgery.

    I have seen a surgeon in Florida that uses an instrument that can enlarge the opening around pinched nerves. It removes bone, but does not damage the nerve. I have not been able to find anyone in Michigan that can do this. Have you heard of this ?

    Pain management told me i had broken piece of vertebrae on my nerves. Surgeon said no do injections and physical therapy

    I have had three low back surgeries with a fusion at L5S1 and after the surgery right after it realy affected my ability to walk long distances.Where one time I could walk four miles six days a week until the last surgery L5S1 fusion where I couldn't hardly walk a block without experience severe low back pain.What is the problem is it because your body was never made for rods and pens screwed into bones to stabilize your spine.

    Just found your channel and am quite impressed. I think your patients are very lucky. I am a 74 y,o.retired podiatrist. I was diagnosed with cervical stenosis C4 thru C7 in my 30s. Had several injections, which worked quite well. In my 50s, I was diagnosed with lumbar spinosis without nerve entrapment. Had injections that were beneficial. Now I am in my 70s. My lumbar spine hurts only when I stand for too long or after I have been walking without support. I can live with that. The question I have is about my cervical spine. I can not raise my arms above my head, R>L. Also, I have hx of osterarthritis in Rt shoulder. If it turns out that the shoulder lack of movement is coming from the neck and not shoulder arthritis, do you think I might benefit from physical therapy to improve range of motion? I realise without seeing xrays or mri, you can't say for certain, but I was wondering in general if PT will help movement of shoulders caused by spinal stenosis. Thank you!

    I had a wicked herniated disc level L4L5 in 1998 and pain from basically my tailbone through my hip and outside my leg around and into my foot. We tried every conservative treatment known but it disabled me so bad that I found a good neurosurgeon who did a microdisectomy and within a couple months I was dancing with my daughter and moving to a different place in Minnesota and then to Alaska and back. Now I have the spinal stenosis in my neck and I suspect my thoracic spine after a fall off a ladder while scraping snow last year landing flat on my back. Maybe lumbar as well tangling anonymous in my behind and then to the urine area and down the back of the legs but nothing like I had before the previous surgery. Going in for a lumbar MRI this week but can't even talk anyone into getting me set up for cervical MRI which I have had previous surgery on and haven't had scan of my neck since the last April which seems like a long time to me and my symptoms but bouncing from doctor to doctor they seem to be playing a copy and paste diagnosistic approach. I was always under the impression with the Mayo clinic here that Minnesota was a good place to get health care since people come from all over the world to be trained here. Can you recommend MRI or x-ray from there? I was going to pay out of pocket but the MRI Center said I still have to have a referral from my doctor. Lastly can you fix necks due to botched neck surgery?

    Hello Dr Donnally, are you still a fan of using an ultrasonic scalpel? I'm having surgery in mid Dec where the Dr will be using an ultrasonic scalpel for my spinal stenosis at L4 and L5. From everything I've read it's much better that the laminectomy or fusion the other Dr's I've seen have suggested.

    I have lumbar pain, numbness below left knee and calf and foot cramping which all get worse with standing/walking. After no relief from epidural and PT I saw an ortho surgeon who examined me and referred to my mri and X-rays. He recommends decompression surgery without fusion. I saw a neurosurgeon for second opinion. He didn’t evaluate my mri or X-rays but only used physical exam, saying it’s better to treat the patient than the imaging. He said try another epidural. Is it common for ortho docs and neurosurgeons to have these specific, differing philosophies?

    Thanx for all the information Dr. Donnally, I am 38yrs post Op, T9-L3 Harrington Rods. I am looking @ revision surgery, to correct an L4 pars defect, so they are going to remove my rods, which are not compatible with GE, Phillips MRI. My team are part of the U.M. Project to Cure Paralysis, the surgery for upgrade/elective upgrade is 5-6hrs, when I had my accident was in 1985, it was 10hrs. I have a lot of pain bilaterally down both legs. I had RFA, Nerve blocks and pain will not subside, so I opted for a last effort with CT Guided PRP injections @ the L5, S1 nerve root, would LOVE your thoughts and expertise, if possible. My Lil brother lives in Houston, my 4 nieces were born there too, LOVE the Lone Star State Happy New Year 2024.

    I have had two epidurals. Neither did anything to relieve the pain. The second one actually added a new source of the pain. It seems to me that the injection even if it eases the pain it is temporary. The Doctor said three should help. Scary stuff when they are poking around your spine with the larger epidural needles. I will do my own physical therapy.

    My surgeon recommended me a laminectomy but I would like to know another opinion. Degenerative disc changes L3-4 and L4-5 with high intensity zone annular tears and left greater than right annular bulging at both of those levels. Disc bulging has regressed/decreased in size since 5/9/2021, though there is persistent left lateral recess stenosis at both of those levels, potentially irritating the descending left L4 and L5 roots in the lateral recesses.

    Severe Spinal Stenosis L4-5 here, it has taken múltiple CT's, MRI's, X-rays and doctors over 15 months to decide on surgery. Laminectomy on 8 December. I lost feeling/movement in left foot/toes in August 2022, hoping for a fix! Thanks for your input!

      I’m 80 years old and I had c5-6; l 2-s 1 done this summer and have gone downhill ever since. More pain, lost use of left leg and now I’m scheduled for hip surgery in January. I live alone and I’m miserable. They tell me that I’m not too old . Good luck to your friend.

      My mother was a healthy 83 year-old...our neurosurgeon said she was a good candidate for lumbar fusion based on her symptoms and diagnostics. He said she could have a good quality of life for many years. She is 90 1/2 and doing GREAT! She lives in a senior community in her own two-bedroom apt. She walks nearly a mile a day with her rollator (at my insistence); otherwise, she uses her cane. Quality of life is always worth the risk, but only each of us can make our own informed decisions in concert with our trusted physicians. I always ask, "If I were your wife or mother, what would you recommend? And at what hospital? I am facing the same decision myself now at 65. 22 years post-ACDF at levels C4-C7. I wish you well.

    I have horrible pain ever since I broke my C2. Just keeps getting worse. Nothing relieves the pain. I don't take any pain meds and never will. That shit is worse than the pain. Excellent information Doctor. Much appreciated.

    I just had a second fusion which turned out to be a revision of my prior fusion and another at L-3-L-4 due to adjacent segment disease and an additional laminectomy. I’ve always recovered from surgery well but it’s been 1 month and while I no longer feel pain when putting weight on my R leg & I am now able to stand straight, I am having severe pain on both sides of my lower back that is actually getting worse. Additionally, my incision became infected & I’m on IV Zosyn bid x 7 days. My surgeon is well aware but his office is not close & said I still have a ways to go. I’ve also been diagnosed with Avascular Necrosis so not sure why I’m not recovering more quickly or why I’m having pretty bad back pain and very sensitive raw feeling skin. Anyway thought I’d inquire. Thanks

    I had acdf surgery back in 2016 unfortunately it didn't help. i had another mri about three weeks ago.the surgery that i had was c3 and c4. the new mri show a mass at c4 an5 and 5 and 6. also scoliosis, spinal stenosis foraminal stenosis, spondylosis and other issues.the orthopedic doctor that i saw three weeks is referring me to pain management.the orthopedic doctor says that pain management may send me back to the Neuro surgeon for another surgery. I am scared and not sure what to do. I am having headaches and it goes around behind my eyes.i wish there was a cure for this. I am fifty years old and have been dealing with my neck for thirty years.i also have lumbar spinal stenosis also it just ant as bad as my neck. thanks for the info

    I’m having now barely able to walk much with walker. 2 fusions… neck And lower back. Surgeries done 2006-2007.
    Now in 2024 injections no longer work. Pain is 24/7
    Understanding from a surgeon that because of lower back area he doesn’t really expect much help?
    All numbing feeling through butt down both legs. Also neck pain and bad headaches. Losing bowel control and bladder control.
    I have had spinal stenosis many years. Is there hope from here?

    I have stenosis on multiple levels. I just went through an over an hour MRI and I am messed up from a motorcycle wreck where I was run off the road and down a cliff. That was a $600,000 accident. Thank God for insurance. My back is destroyed and I think my damage is beyond your repair. I.m Tied in with Kaiser and now have metastatic advanced cancer. Perhaps it is my time...

    Hi Dr.Donally, when I walk and stand I don't feel pain. However, the moment I lay down flat on my back to sleep, strong spasm seem to pinch my left armpit and middle back for 2 years now. I still don't understand what causes it. I can barely sleep either on my back or sleeping on left side is worst. I've L4, L5 and S1 bulging disc long time ago and narrowing canal at T11. Also had trigger points injection on my left shoulder in July 2023 and now has worn out. Pain in the left shoulder travels to the lumbar to the left sacrum and left hip.     What can be done to stop Si joint pain that only responds to nerve block injections for short periods. The Si joint pain has been only increased over the years despite many types of treatments. The pain breaks through ketamine infusions after 4-5 days. I have already had a discectomy and fusion of l4,l5, s1.

    I am a 65 year old woman with history of anterior cervical fusion of c4-7 in 2011. I currently have large blocks of osteophytes at C2/3 & C3/4., making my neck “unsteady”I am scheduled for a further fusion above my current fusion.
    I also have lumbar spondylathesis with severe pain down my L leg for a year. My neck needs to be addressed first. Am I making a reasonable decision. I live in FL. My first fusion was at UM. This surgery is scheduled in Palm Beach with a reputable neurosurgeon. 🙏🏻

    I had sciatica on the inside of both thighs. I was laying on my stomach propped on my elbows on the floor playing with a tiny dog and when I got up, BANG, severe sciatica for the first time.
    It was diagnosed as arthritis in my spine which caused calcification of the cartilage where the nerv exits the spine.
    At first the remedy was described as simple clean out calcification. Every time I saw the doctor the surgery needed got more and more complicated.
    When the surgery was finally done, a lot of nerves were damaged leaving me in chronic pain.
    While I was convalescent, the surgeon retired and his office closed.
    A pain management clinic took over and did 5 different proceedures trying to correct the problem. Nothing worked and ultimately, they installed an SCS which was fairly effective at first.
    Now, 5 years on, the SCS it not doing much and I am at this point condemned to life long pain meds and even with all that, I can only be on my feet for short bursts totalling perhaps 2 hrs a day.
    My question at this point is fairly general:
    Can a failed lumbar spine surgery be truly 'unfixable', or is it a matter of finding the right surgeon?

    I’ve had two spinal fusion surgeries.. first one bottom of L3-S2. After that surgery I couldn’t walk without a walker. Second surgery 5 years later top of L3-T12. Better pain management now but still can’t walk without walker. Will I ever get my walking abilities again?

    I have facet joint disease in my lunbar with osteophytes my L2 thats causing me pain espe ially whem twistin i had a lumbar laminotomy L 4/5 in 2017
    Could bone spurs be causing my pain ? Also the top instep if my right foot is semi numb
    No sciatic pain after my laminotomy sunce 2017

    I have spondylosis, scoliosis L1-3, stenosis L5s1, I've herniated most of my discs, and DDD , I had injection and ablation and it feels like the top of my top spine is not supported by bottom. Can mis help?

    I had a trans thoracic fusion L2-3 and L3-4 for CES, fusion at L5s1 and a fusion at C5-6 possible new pinning of C2-3 . Will my spine ever function normally with this articulate ?
