And when Megumi talks to Yuji he finds that Yuji had the box but not the finger and assumes that's what he sensed. Megumi also questions if Yuji has heavenly restriction like Maki who's also a twin.

Do you know how the death painting required three people; the curse, the mother, and kenjaku. In Yuji's case the curse was sukuna therefore even before birth Yuji became a vessel for sukuna and hence on consuming the "first" finger he did not die. This also relates to the fact that Yuji was in a way always close to a finger in his school.

Gege did state that the ending of JJK is going to be either only one surviving on the team, or only one dead on the 4 of the characters which include Yuji, Megumi, Nobara, and Gojo. Since we have slight chance of Gojo, Megumi, and Nobara returning, only Yuji dying can be a possible ending.

I think you nailed it. Elden Ring has this same plot device. The Triqueta, endless knot, yuji and sukuna form one of three, vesica piscis. Yuta and Rika, Yuji and Sukuna, tengen and the star vessel, it's the basis of these stories and why as we read the endless knot makes it unsolvable. Sukunas malevolent shrine and cleave are these concepts of just cut the knot solution Alexander the great uses in the Gordian Knot story. The idea of the soul has been pushed heavily since Mahito’s introduction and was also restated in recent chapters. Yuji and Sukuna have also fought inside Yuji body before which is what lead to the binding vow that Sukuna made to take over Yujis body whenever he wanted.
I know this might be a long shot but Yuji being the last finger is sort of like Voldemorts horcruxes. There was one horcrux where it showed the last of his empathetic qualities. I thought it was just similar to Yuji being selfless as he is sort of that “good guy horcrux” while Sukuna is selfish. I just saw a little parallel but I don’t think it’s anything 😂
