    Dr. Hill is a Psychiatrist that Josh had sessions with after the loss of his sisters. The first few seemed like legit flashbacks, and it shows how long Josh had been planning his "game". The later ones are hallucinations, Josh is seeing what Dr. Hill would say about the situation he is in.

      I think the first actually happened to him, but the rest I think are hallucinations. As we see with the one inside the mines, Dr.Hill follows him there.
      It seems, that the place they meet at in each 'visit' is the place we or Josh is at currently.
      In the second 'visit' it looks like it's inside the lodge, instead of the office. And as we advance through creepy stages of the game, the office is creepier as perhaps that was what was around Josh at those times.
      I can just imagine him taking breaks in certain areas, and just having to talk with Hill once more.
      I thought Dr. Hill was going to be the 'Stranger' at first when we didn't get a good look at his face, and I was pumped.
      Too many, close to bald guys.... :(

      +Thaal Sinestro I think everything is an hallucination. Josh was pretty calm at first, didn't show anything of his psycho-stuff. But then his plan started to work and Hill's office started to get all dark and creepy just like Josh's mind.
      Also: apparently there's a voice mail or something or a message about Dr. Hill. I can't remember what it says but it was something about his sessions. Josh was already on the mountains anyway so he couldn't have gone to dr Hill.

    *SPOILERS*
    I believe these hallucinations are the part of Josh's mind that tries to get better. At the beginning he really believes that playing the Game will help him, but as the time goes on he starts having second thoughts. But as he falls deeper and deeper into psychosis, and as his mind deteriorates, so does the part that tries to help him. In the end this part of his psyche starts to openly fight him, recognizing how damaging his Game is for him. And when "Dr. Hill" leaves, that signifies the death of this part of his mind - it can no longer fight the madness, and now only violent hallucinations and regrets are left.

      +Siriathion Lol I had a similar hypothesis after playing three times the game :
      First, we discover Dr Hill through the "sessions" but later in the game we learn he really exists and worry a lot about Joshua's plan and the backfiring consequences via sms. At each session and according to your choices, Dr Hill is shown more inquisitive and less friendly, he even strikes a chord with Joshua at several occasions. Later in the plot we understand Josh suffers from psychosis (with a lot of symptoms hiting at paranoid schizophrenia) altering his perception of reality and supressing his inhibitions. After the discover of his twisted plan, Hill is seen questionning Josh about his morality, his guilt and how his actions has hurted innocent people. Hill's office and the psychiatrist himself are transformed along the way Josh is decompensating. Actually, Dr Hill as he's introduced in the game could be considered as a projection of Josh' s superego trying to refrain him from losing control and coping with his inner sorrow and pain, but without the medicine it's too strong and that's why in the end Josh is assaulted by morbid hallucinations.

    His comments about the "game" are so brilliant when you learn he's been talking to/about Josh the whole time, and you think he's talking about you, the player. Also how his tone changes from the beginning, when his view on the "game" is positive and both you and Josh are just getting started, and in the middle of it when he wants you (actually, Josh) to stop and repent.

      The first session is probably the only one, the other sessions are Josh's hallucinations. First, Josh thinks he's doing good, but then he starts to regret it slowly and he aknowledges that this "game" isn't good for anybody which is why "Dr. Hill" in his hallucinations starts yelling at him and telling him to stop.

    I absolutely loved how they implemented this "analyst" stuff. The way they make you think Dr Hill is talking to you as a player, but in reality he is talking to Josh in Josh' mind.

      I always got the feeling that the fake Dr. Hill was what was Josh's desire to make sure that no one would be hurt or to stop what he has planned for his former friends, but since Josh kept neglecting that part of him for so long, it resulted into becoming a separate personality where it causes hallucinations and tries to make him stop but then he realized that the other part of himself was beyond saving.

    Judging by the state of the window, I think the first two ones are real, not only the first. In the two of them you see the window normally, like when the station was operative. But in the third one, you see the window sealed, meaning that the station is closed and everyone left, so he can't be anything but a hallucination.
    Send a powerful message about dangerous patients that should not lose their therapy sessions... congratulations to the scripters for this social message

      He was real. In the game you can find the bill for Josh's therapy sessions. Dr hill signed them at the bottom and everything.
      The sessions in the game are hallucinations, it's all in Josh's head. He's is visualizing what dr hill would say in his current situation. Sort of like his Conscience telling him that what he's doing is really fucked up, and instead of asking for his friends help. He feels he needs to seek revenge for them sort of killing his sisters.

    Um, I have a theory I would like to share.
    *"Josh is a reflection of who you are"* (I wish I could come up with snappier wording of it, but this will do.)
    Please listen to what I have to say :)
    Now, you watched this video, right? Well, we later find out that "we" are controlling "Josh" in these scenes. Even as Josh becomes isolated and alone in-game, Dr. Hill's office becomes more forest-y and gore-y as time goes by. Also, it is evident that if you start choosing things that Josh's character in-game normally wouldn't choose, then Dr. Hill will start to think you are lying to him.
    Going back to what Dr. Hill says when we make our choices, Dr. Hill says things like, "...as you continue with your 'game'." or "...the choices you've made". It's seems at first to the player that Dr. Hill is breaking the fourth wall by speaking "directly to you", as if implying that we, the player, is in charge of the characters in the game and should be cautious and mindful about our choices and make sure they all survive.
    Pretty straightforward.
    But something seemed a little off to me when the game revealed that Josh was the patient. It blew my mind. I was like, so Josh is...well, me?
    Then it hit me.
    The "game" Dr. Hill was referring to was not only about what I was literally playing, but the 'game" Josh was playing (pranking his friends to get back at them for indirectly killing his sisters).
    So with that, I also gone to the conclusion that the game is trying to imply that you, the player, are psychotic. Just like Josh.
    Still a bit confused? Put it into this perspective. Our job as the player is to control who lives and who dies. It says so in the description for this game and it's basically the whole focus of the "Choose your path" feature. (Aside from the Butterfly Effect feature)
    But isn't that a questionable statement all on its own? Josh was doing that, too. By playing this game, you, the player, are putting these seven (eight) characters at risk of dying just because you wanted to play Until Dawn. That was what Josh did--he didn't care that his "game" would possibly kill his friends and put them all at risk. He just wanted to play. Just like you.
    Trust me, I was a bit freaked out about this. So many people who played Until Dawn disliked Josh because of his mental problems and villain like nature. But to think that the character you didn't like ended up being a reflection of what you are doing.

    I liked Josh's character. I may be a bit bias because Rami Malek is my all time favorite actor *ahem*, but I really did like Josh the second he appeared in the game. He was awesomely creepy. :)

    *HOLY HELL.* When I did a playthrough of this game for the first time, this literally jumpscared me to Africa. Was not expecting Dr. Hill to do that.
    I also like the symbolism in the room where Josh is having "therapy sessions" with Dr. Hill. The room goes from looking normal, to being in total disarray- probably symbolizing Josh's mental state, as it slowly breaks down throughout the game. The windows also become boarded up, which could represent that Josh feels trapped inside of himself, feeling that he is to blame for his sisters' deaths, and he has no one to console to about the way he is feeling. Dr. Hill could also be representing Josh's guilty conscience, Josh questioning his decisions, as well as Josh questioning himself- uncertain if his decisions are right or wrong.

    Game Theory: Dr. Hill is actually Josh's voice of reason (Conscious) that morphed and took on the form of the good doctor within his psyche, due him being one of the few people Josh explicitly trusts and confides too, attempting to persuade him out of the deviant path Josh carved for himself, resulting in his friends becoming estranged from him as a consequence of his actions. Again simply a theory.

    I for one, would love to have Peter Stormare as my psychiatrist(or therapist, not sure if there is any difference), his voice is so soothing & calming, and his Eyes seems friendly & kind. I(as weird as this sounds since i'm 23) am terrified of making Eye-Contact thanks to my family, but with Peter, i wouldn't have any problem with that.

    i feel this intense contempt from Hills expressions and manner of speaking. When I first played the game he made me feel relaxed, perhaps because it was a break from the spooky, but watching these clips back, individually, they make me anxious...

    Wouldn't it have been a fascinating development if Josh's madness made him completely invisible to the Wendigos.
    They are literally a curse; evil nature spirits unleashed upon the mountain. What if they can only detect the flesh/souls of people who actually have a will to live; that those who are tormented by their own lives so badly they want to die or don't know how to live anymore are left alone by the curse to suffer and be denied the release of death.

    he is, in all these sessions, a figment of josh's inner guilt over his actions. he is the part of josh's consciousness that is formed due to the pent-up guilt he has had over the years.
    and the scariest thing about dr hill... is that he exists in some form. in all of us. maybe not by that name or appearence, but we all have done things that our subconscious haunts us over. insisting we will NEVER live down what we did or didn't do.
    guilt is a very real horror that affects us all.

    Am I the only one who noticed that the fears we chose from one of his interviews began to appear in the office towards the final cutscenes, for example the spider in the jar and the clown with needles in it.

    What’s cool is that when you make a lot of bad choices, dr. Hill will become worse in condition. His psychical appearance goes downhill, unless you make the right choices, leaving him to be fine.

      Nah he was literally just Josh's psychiatrist after his sisters died and knew about the 'game' he was planning but that was it. Every scene with him in the game is inside Josh's mind except MAYBE the first two, which would make them flashbacks.

      You can very briefly see at the beginning of the game that Dr. Hill left several voice messages on Josh’s phone. He is very much a real person who was trying to save Josh (though most of his appearances in game are just hallucinations).

    Also draw notice to the name. "Dr.Hill". The whole plot tie in is the Hill. So from this angle every scene is imaginary. Josh is talking literally to the hill.

    I like Dr Hill :D like, he is awesome, Peter Stormare is awesome! I find his voice and the whole character relaxing, though there is something creepy about him, but that could be just Josh's mind. The real Dr Hill seemed nice person, considering his worry for Josh. Dunno. But if there was ever gonna be a sequel or a dlc Dr Hill should be included!
