    I am starting a new job in SNF with no experience in SNF so this is going to be a learning curve but this helped so much to know what to look out for and a foundation set to build off. Thank you!

    Thankyou so much for making this video. I like the fact that you went through it step by step it was clear and easy to understand. It has been a while since my program focused on the basics of evals so I was not too confident in my abilities to perform them accurately for my Level II FW that is coming up soon. Thanks!

      LovelyyOT I totally agree. In my program we did mock evals on one another however I don't think it was enough. I guess nothing beats real life experience. However my goal is to go into these next FW assignments with at least a basic understanding.

    If you have ever done outpatient, i would love to hear about how that type of evaluation is done.
    I have worked in snf as a OTA but now im a OTR/L working in outpatient and the population is different and they schedule them every 30 mins at this place so its very different. I appreciate any advice. Great upload

      NIKKI STAHR I did a hand therapy outpatient during my fieldwork level 1 but I was only there for a week so I didnt get into the nitty grittyof things

      I am prn there so im just taking things one day at a time..snfs are more my thing but i am trying to broaden my horizons by diversifying my job site experiences because i want to teach in the next 5 years so you know how it is. I love your channel and enjoy seeing young beautiful black OTRs (im Haitian, Dominican, French and unapologetically black.. patients love to assume once they see my sunkissed naturally dark tan skin that im the private duty or cna..i never get offended but educate the hell out of them.....until their ears bleed n fall off lol...
      we are definitely changing the landscape and im proud of that💃 so keep doing your thing and promoting your channel and sharing your knowledge/ experiences..im so very proud of you sis)

      I cant film at work due to HIPAA regulations , everyone I come in contact would have to sign a consent which is impossible cuz these settings are busy n filled with people going in and out

    Thanks for the walk-through! In a SNF setting, do you tend to have higher productivity requirements than in other settings? And how long do you usually need to write an initial evaluation vs. a progress note?

      Melissa Thayer Yes we tend to have higher productivity in SNF; Most people don’t like SNF because of this. I can write an eval in about 30 to 35 minutes and a progress note in about 15 minutes or less depending on the patient; some are more straightforward than others

    How do you usually plan your day , sequence Do you keep the evals as first then the treatments and discharge ? On what basis do you decide or any advices for it , switching from outpatient to snf so thinking a bit

      I usually do my evals first in the morning and then I will finish typing it out during lunch ( since patients are not available during that time). However, if am taking lunch, I will finish the entire eval in the am ( my brain is the most alert at this time ) and then treatments as patients are available, some patients do better in the AM versus PM.. I also like to do my morning check-ins when I get there in the morning with everyone on my schedule just to give them a heads-up that I will be seeing them that day. Once you get used to the setting, you can figure out what works best for you.

    I thought everything was computerized, oh Lord I can't imagine having to try and decipher those doctors notes that are written like a 5 y/o. And what if the SNF keeps people that will never be able to go home? Most of them just stay in there due to cognitive issues and have no family to go back to and can't live alone. This is what I was told when I applied for a job at this specific SNF. Rarely any hip or knee replacements, just patients with mainly cognitive issues who want to stay there because they are given attention and you have to give them 30 min of therapy 5 days of week due to Medicaid. How do you function as a therapist in a SNF when it is mainly upkeep instead of progress? I read that you can't bill for the time you spend writing up the eval or treatment. Does that mean you aren't paid for your time? Even transferring the patient room their room to the OT gym isn't chargeable. So you can work 8 hours a day and only get paid for 5. Am I right on this?

      You get paid for how ever many hours you are there but your productivity will be affected , if you are scheduled for 6 hours and you there for 10 hours , what were you doing for the extra 4 hours ? productivity is to minimize this occurrences. You can pick up LTC patients/ nursing home patients who live there and work on things they may need ; trust me progress is always there even if its small. if there is absolutely no progress after like a month , its ok to discharge them .I hope this helps.

    Oh and is a SNF only for geriatrics? I googled it and I saw was care for geriatrics. I would love to work with young adults.

      Yes mostly geriatrics ( older adults) , my youngest patient was late 20s. OT have a place for that as well :)

    Hi, do you have spouse or family while Evaluating? As patient can be groggy and will answer anything

      It depends , if the family/ spouse is a POA .. if I have further questions and patient don’t have the cognitive ability to answer ; I usually call . It’s not a requirement for them to be there ..
