      I was born with a pronation deformity. I assume surgery is my only option. My pronation is x3 worse than the examples you showed. I still walk and work 8hr+ per day

    My achilles tear from five months back now makes so much more sense. I recognized years of poor posture, tight ankles, and chronic tendonitis, but never really considered that it was my feet that were generating the issues leading up to it.     You Are RIGHT! I honestly didn't realise that my overpronation was causing my entire lifes' strange fatigue, pain in body whenever I walked about. I need to get it sorted.

    For anyone who is struggling with pain and who is the US, please look up a physical therapist who is trained in the Manual Therapy Institute. El Paso PT has a great you tube channel. I am working with a PT now for hip and knee pain, she is retaining/strengthening my feet (fallen arch, pronation) and glutes mostly. She recommended stability shoes and something like a Hoka for long walks (optional) and said I would never be able to be in barefoot shoes because of how I’m built (wide hips). So everyone is different. Good luck everyone and sending everyone healing xo

    OMG I had all these symptoms for years. Finally went to a podiatrist and an orthopedic surgeon to get treatment. Proper orthopedic shoes, custom inserts, weight management and massage therapy helped tremendously.

      @@majestymoon9276 Professional inserts in some good quality boots or shoes improved my posture and the way I walk, also made me look taller ( bonus). I am still considering getting the newest surgery to correct over pronation of the feet, because the inserts have to be replaced and are costly.

    Hi from the UK🇬🇧 and thank you for this video. I now understand why my ankles rolls over to one side(always hated the way it looks), worst on my left foot. My feet are extremely flat, looks like a flat fish. I have bunions, again worst on my left foot. I get sciatica and pain in my lower back. Always have to wear shoes that grip my sides where there should be arches.
    Even though I have visited a chiropodist at the hospital and they made me a pair of insoles my flat feet and rolling of feet has never been explained to me. This is one of the best explanations I have seen.👏🏿👏🏿👏🏿

    I simply don't understand how as a podiatrist and someone that is supposed to understand feet function, you can still recommend insoles and arch supporters at 2020... I think it is very clear today, and a lot of professionals are gradually shifting to this understanding, that our feet were meant to be free and naked and not ruined buy the tight and rigid shoes that you offer as a solution... If anything, those shoes are the origin of the problem. Then fact that we are introduced to the from before we can even walk is what causes are muscles tendons ligaments and bones to just weekend and loose their natural ability and function. The shoe wear industry is nothing less than an epidemic in my opinion. Not saying flat feet is not a problem. But strentging them, reconnecting to our feet and developing stronger physical awareness of our bodies, while actually spending more time barefoot and active - is definitely a better treatment then orthotics that quite literally destroy our feet function.

    This is helpful to understand how overpronation works. I have a bunion forming on one foot and want to correct it. I'm changing the way I walk because I noticed that, among doing other things that caused it, yes, I do overpronate on that ankle/foot. So I'm making a conscious effort to walk straighter, and with my weight distributed back to the outside of the foot inside of towards the inside. Edited to say no, I don't have particular pain from it.

      my ankle moves inward it’s been years (when I was a kid 9-12 y/o ) once my foot is injured in an road accident (I wasn’t able to walk for about 4-5 months) since then my ankle rolls inward I noticed then but ignored but It cause pain now(I’m 17 now). when I walk but it pains sometime more while sleeping & its pain when I walk straight but It feels better & isn’t painful as before until I over walk. Can you suggest me any ankle brace.

      I know the post is a year old, but another thing to consider is your shoes. Modern shoes are not foot shaped and they push all your toes inward to the point of the toe. Your toes being forced to push inwards is also a major cause of bunions. Focusing on walking the way you are is great, I'm doing the same thing for my own overpronation, but if you're wearing orthotics or cushioned shoes with a "normal" toe box, you're not going to really get anywhere unless you spend a lot of time barefoot. Luckily, there are barefoot style shoes (don't worry, they've evolved since the toe shoes, they look normal now), and wearing these will allow your toes to spread apart naturally, and they will force you to strengthen your muscles and walk properly because there's zero support and the material beneath your feet is only a few millimeters thick. It takes a long time to get used to them though, at least if you have weak feet, like me. Our feet take a beating once we leave our house so they won't be an all day shoe until your feet are strong enough to support themselves.

      Often a bunion forming on one foot is due to having one leg longer than the other. The longer leg has to overpronate as you walk so the bunion occurs on that side

    I've found recently my right foot doesn't line yo with my knee.
    What would you recommend to correct this as I think it's causing a slight gait in my walk along with some minor issues I feel might be increasing.
    I've heard the term "duck foot" for what it looks like.
    I think it's my Shin or Tibia based on what I've seen on it.

    Hey doc i have done my ultrasound done and want to know if its possible to get strong ankle and arch again . The report says
    For the lateral part Hypoechogenecity with thickening noted at the insertion of posterior tibialis tendon. No evidence of tear noted. No evidence of calcification noted
    Spring ligament is showing flattening with altered echogenicity in the fat pad of medial arch
    Deltoid ligament is showing thickening with partial tear at the level of talus ( tibio-talar component) . Tibio-calcaleal component is showing dense calcification.
    Ganglion cyst noted from talo-navicular joint space extending medially and displacing Posterior tibialis tendon with mild impingement on the Posterior tibial nerve
    For the medial part
    Rest of the underlying bones are showing no evidence of cortical breach . Surfaces are irregular with significant osteophytosis and reduced intertarsal joint spaces

    Yet another outstanding, informative video!! What is your feeling about the use of “Carbon Plate” inserts used under orthotics? These can be used in multiple shoes! This also stops the bending, but a which is better? The shoe, or an inflexible carbon plate? Thanks 🙏

      Personally I love the carbon fiber insole for thinks like broken toes, in general a deep heel cup (like a pair of power steps that are linked in the show notes) and support is more important than rigid inflexible insoles for something like over pronation. But always see a doctor if worried about a specific medical problem!

    Thoughts on dry needling the poster tibialis, gastroc and soleos to release? I had it done a week ago and my calves have never felt so fatigued in my life after for the next 24hrs, I feel like that's a good sign that they are being constantly over worked?

    I have what is now identified as Ehlers Danlos Hyper Mobility of the Joints, so any advice about what shoes I should get? I was turned away from the podiatrist who said he could not make me an orthotic as my foot would just melt around them and hurt even more. I cried when I left his office. I need help, I now have a whole host of other diseases due to my bad feet. Thank you so much for any advice

    I have custom orthotics but heel slippage is problem!I fix orthotics 2 times and heel slippage again but its better after 2 fix times.My doc says the orthotic can be fixed for 3rd time.What your opinion for this problem and ow can i fix it for 100% natural heel feel from the shoes without splippage?

    Thanks for the gr8 knowledge, it's very good for the people whe does not have a yime to train the feet, How about train you feet? I believe orthotics are temporary solution, it's like training wheel so I am 42 yo, so been living with over pronotion, last 2 years I have started training my feet and the result in the time frame is amazing, it's really hard at the beginning to train you pronotion feet but you get the permanent result..

    My Dad had flat feet his whole life and I had flat feet as a child. He took me to a shoe store that had a fellow with training in the Thomas heel. I wore shoes with the Thomas heel for five years, and got good arches over that time. I remember Mr. Sines pointing out how the wear on the heels moved slowly over numerous pairs of shoes from the inside to the outside as my pronation disappeared. Still have good arches. The Thomas heel was invented by Hugh Owen Thomas, a Welsh orthopedic Surgeon in the late 1800s. We've had a cure for flat feet that long! I can attest to the effectiveness of the Thomas heel. And the shoes were attractive, normal shoes. Just the shape of the heel was different.

    Hi, I have pain & swelling in the ball of my left foot & also pain on the top of my foot. My toe next to the big toe is bending over the big toe its happened more over a few weeks.

    I can’t locate the link on what you think about sinus tarsi implants, particularly in adults. Can you post that again? For reference, my issues began about 10 years ago during my pregnancy. I am waiting on my custom orthotics to arrive
..but I’ve spent so much money on over-the-counter orthotics and they have not helped. Thank you for your time, education, and videos! I’m a subscriber!

    Motion control shoe with a custom orthotic will help some. I also use a AFO but they are bulky and a bit uncomfortable but it all have until I decide if Want to risk surgery since my feet are so bad.

    Never thought I'd be looking at these videos but I have a 11year old who has been in pain for years. Physical therapy, orthopedics aren't working. Two orthopedic surgeons both said *just buy some inserts" it's so frustrating. Trying to find a podatrist or foot and ankle doctor for peds is a pain

    I have knee pain although I'm young and not athletic. I didn't know what caused it. I have Crohn's and I blamed the disease (could be a reason too not sure) but damn. My shoes is so overpronated and it is obvious I have overpronation when I look at my foot from the back. I can guess that the reasons are that I'm overweight, used the wrong (too comfortable shoes like sketchers weakened my muscles) and lack of flexibility. Will try the shoes and stretching

    I have a 7 year old with one foot one size smaller and calf slightly shorter. The bones of his toes center foot fused resulting in a good arch. But his ankle is rolled in. If he wears a brace will his bones(talus and Calcaneus) grow to support his ankle correctly?

    What if you were born with flat feet, I was born with them and I’ve tried everything it makes me really insecure how much my foot folds in

      Same. If this is your biology in my research and life long seeking treatment I have found that no amount of exercise can fix you. Exercise can strengthen supporting tendons and wearing support can provide some comfort and prevent excessive worsening thats it. Nothing except surgery is going to fix your flat foot and even then the corrective surgery is not always very effective and it limits some ankle mobility.
      I know a coworker that had this surgery to correct her overpronation and extremely flat foot. She ended up staying off of work for a year and was forced to retire because her Dr kept extending her order to stay away from standing jobs.
      She never felt 100% again.
      Don't get me wrong she feels good and travels and looks great but she couldn't do a job standing on her feet for almost 2 years. I spoke to her about 2 years afterward and she said that her running ability is zero.

      @@greataustin2834 No she cannot jump or rather I'm sure that she can jump but prefers not to do so due to discomfort and slight stiffness of ankle but she says that she feels better than before and she even wears high heels to events.
      She is not the kind of person to work hard or test her limits so I don't know.

      I had the pin insert. It wasn't long before I heard/felt that foot pop. The pin appears on the xray as being in that right spot. However, the problem remains.

    I apologize if I'm not understanding what my question is about. I've watched several Podiatry videos and this is never addressed. In my case I have Posterior Tibial Tendon Dysfunction. My Podiatrist recommends me using the orthotic inserts. My question: Is the orthotic inserts supposed to be worn for the rest of my life? Will the inserts correct the problem or just work like a crutch? Thank You in advance.

    him showing me all the shoe options!!!! i never knew they had special everything shoes. i need an orthotic shoe guide with a bit of a higher ankle ( sides) because the custom inserts I received are too thick for my current shoes XC.. lol I definitely know where my pain is. " some people don't even know" he said I'm in a decent amount of pain during standing for long periods of time and mostly after the fact when I'm sitting or laying down, just bending my toes hurts ( the issue is in the sole of my foot into my two middle toes) the pain has gotten a bit better with the inserts but not really sure yet well see. i have a follow up this week :)

    I'm asking myself why is there no brace that grabs the heel bone and rotates it from that pronated inclined pisition into an upright neutral position and provides resistance against pronation there? I feel lime all braces attack places that help, but that arent the origin. The very heel bone should be preventet from falling inward.

    why the talus is planterflexed in extreme supination diseases as talipes when it is supposed to be dorsiflexed as it is over supported by internally rotated calcaneus ?

      Thank you for pointing this. I’ve been spending months now on CZcams learning about weak feet and flat feet/Fallen arches. Every other video I’m watching on foot mechanics, intrinsic and eccentric strengthening seems to follow with the idea of correcting strength imbalances to avoid all these interventions mentioned. It’s as if he is acting in bad faith to scare people with tip 2 being surgery and then follow with tip 3 to recommend custom footwear and orthotics. No bias there right? What a shill. What a joke.

    I am sixty now.100% flat foot.Went through peripheral neuropathy when I was in school. Overweight and no habbit of walking at all. Walking two hundred three hundred meters is getting difficult now.Cant match speed of friends with whom I walk due to uneven balance of body weight on foot .All I want to reverse it.Once I was one of the best athlete as youngster.This video will help in my workout.

    Dr, Tom, wearing a D width for years. Suddenly sever pain, bunions etc. Yes. I aged and gained weight without adjusting the shoe size. Went to an extra wide shoe to eliminate the pressure of bunion. It helped. Today while watching one of your video, the suggestion was don’t get need extra wide shoes. Yikes, I’ve been wearing extra wide shoes for 20 years. Plantar Fasciitis showed up. Have semi custom fit orthotics. Ready to buy better shoes.
    It is too late to force my foot into a standard width shoe, from a 4E?

      With good supportive insoles the foot should pronate less, but if there is arthritis it may not reverse that much. The answer is... it depends

    Thank you so much for the video! I'm an overpronator and I've worn asics in the past with custom made insoles, but I've been getting knee pain on my runs and i feel I need a better shoe. I just bought a pair of brooks adrenaline gt 20. I've researched they're good for overpronation. But I wanted to you see what you'd recommend for those with flat feet. I saw on your site you had the brooks ariel and the altra olympus 3.5.

      With shoes it's really about how you feel. Even talking to the best shoe designers around, some people just feel better in different shoes for different reasons like sweating, bulk and ease of use

      Personally we find best results in a good shoes in the 8-12 mm range from Brooks, Asics, new balance, Mizuno, Hoka etc. All the extra stuff depends on personal preference and stuff that's tough to give general advice on.

    As a nurse on my feet 13 hours a day I have developed extreme pain in my right little toe. I started realizing while wearing flats my left foot turns in. I am assuming this is causing the pressure on my right little toe.I bought pro foot pads to wrap around which used to work, but now they're not. I wear a size 5-5.5. I bought Brooke's Addiction shoes that were supposed to be for overpronation but still having pain. I did just buy a toe separator for bunions. It's not on the side of my little toe, though; it's on top. Are there orthotic inserts for small feet?

      Find out from a chiropractor if your hips are out. if they are out it will affect the way you walk and it will affect the way your knees and legs work all the way down to the ankle then to the way planter fascia works and in turn it will slowly screw up the know system with how you walk, yep found this out also, still working on trying to get it back in alignment

    What about strengthening the foot muscles through exercises and going barefoot a lot? Plus, switching to barefoot style shoes? Wearing orthotics and stiff shoes weakens our feet.

      Depends on whats going on, if you are injured and barefoot walking makes it worse, then barefoot walking is not the answer😀, would you barefoot walk after a stress fracture or broken ankle?

    Flat and over pronating feet all my life. Have all the stuffs & done it all except surgery. Several doctors & specialists but nobody explained how simple the surgery is. If I go again and they tell me this & I can get a surgery tears of joy will flow and flow

    I have flat feet. I supinate on my left (hallux varus after bunion surgery in 96) and pronate on my right (bunion with no surgery). I just recently had a toe joint replacement (Keller) on the left and in recovery from that, but I bet I’m still going to be supinating from the years of dealing with the hallux varus. My left big toe still wants to go varus (my second toe has actually moved over to take its place) and my instinct is to tape my big toe in place until I’m fully healed (I’m 5 weeks post surgery). I think for sure I will use orthotics for the arch support but it almost seems like I need a narrow toe box on the varus toe to keep it in place and a wider one on the right. You didn’t mention anything about shoe width in the toe box. Do I need completely custom shoes? Help!

    OMG thank you. I have bunions my left is much rose than my right and my family says it’s bc of heels but I’ve never been big into heels. Now I know why. I also have accessory navicular syndrome. Just had surgery to remove it.

    i was told i have overpronation from my podiatrist. i tried custom orthotics. they sucked. like didn’t help at all. i am suffering from inner knee pain on left leg. and outter hip
    pain on both hips. worse on the right. went to dr. i was told i have hip impingement, but that sounds like inner groin pain. i suffer from outter hip pain. should i consider these insoles? find a new doctor? how do i find a doctor that’s gonna actually help and not just throw over priced custom orthotics at me!? really need relief cuz i’m

    your foot needs to be strengthened. orthotics and surgery will just make your feet weaker. proper movement patterns fix this kind of stuff.

    Thank you for the video. I had custom orthotics made and my right foot is still completely collapsing. My foot is turning externally and so has my femur. Extremely tight calves and hamstrings (no relief from stretching). My podiatrist is zero help (didn’t investigate anything you mentioned) any advice? Thank you!

      Generally in a situation like that I would evaluate how tight the foot, ankle knee, hip and lower back is. As this gets evaluated the tighter areas are evaluated and the root cause of tightness evaluated. Orthotics help but they are like a bumper on a car without working breaks if the root cause is not corrected.

      Initially if too sore, massage and pain relief as well as support, then as things loosen up stretching and exercises can help. But always check with a doctor to evaluate for the root cause and problem if possible!

    Since this time last year I begin to have really bad pain in my heels, what work the best now one year after are these:
    - Change shoes (cushion and pronation ones, for a regular job I'm a teacher so I'm usually standing up all day) I used different types of Adidas Boost and use softer insoles (only if you know you will be seating or walking very short distance or time)
    - Everyday massage, movements, stretching in the morning as you wake up - at night before you sleep
    - Buy a massage gun for your feet and your legs, at the end you need to stretch all your tendons not only your feet
    - Go regularly to foot massage (i live in China so this easy and the biggest improvement) you will learn how they do it, you can do it to yourself or ask someone to do it for me (my GF do it a lot before)
    - Never walk barefoot unless is grass or the beach, or nature then do it as much as you can
    - Do regular exercise (focus double on stretching)
    Hope it helps, it was a hell of a process, I would say a total year, now I've used again flat sneakers or even dress shoes, with some special insoles or heels, for hours, I mean like parties (being dancing and standing up for the whole night and stuff.... and don't get that much pain as before.

    I just got my costume insole today and I noticed that it doesnt really prevent my ankle from pronating, I mean it doesnt align my ankle very well as u explained in the vid.     I’m certain that this was a bio mechanical problem I’ve had most of my life. Im getting both knees replaced February 6th, I have severe mid foot arthritis in my left foot and I believe I have some in my left ankle on the inside right where my ankle and foot join. My left foot has no arch in it anymore and my cuboid bone has descended down on the left side of my foot. If I try and walk barefoot it hurts my left foot. I’ve also got osteoarthritis in my hands. My mom was riddled with this stuff and according to her it was prevalent on her mother lside of the family. I’m not sure if there’s much I can do to correct this because the cartilage is pretty much gone in my left mid foot. As long as I wear supportive shoes I manage but I do have pain when I walk very far. I’m hoping that when I recover from the knee replacement surgery that it takes some stress off my feet.

      @@pete9688 13 days post op, This is honestly the hardest thing I’ve ever done in my life. The first week was really rough, lot of pain. A person can’t imagine how much trauma your entire body put through from this kind of surgery . Your legs are black and blue, you have a tremendous inflammatory response and swelling all the way down to your ankles and feet. The opioid s they give you help some but not completely and they constipate you terribly. You must make yourself eat because you just lose your appetite. Your whole self feels abnormal. Its very difficult to sleep well. The therapy is painful also. I’m getting around ok with a walker but my knees are both stiff and the hurt when you get up and down from sitting. Your just under a tremendous amount of stress every moment. I’m hoping by the end of the week the pain levels subside and I’m feeling better and some what back to normal. Once i get through this I’ll probably be glad I did them both. But, I would not do this again, I would do them one at a time. These first two weeks have been miserable. I am making progress on the rehab, right leg is already at 111 degrees flexing, left is a little less at 105. Thanks for asking.

      In general softer orthotics are better for older less athletic people (they focus on cushioning the feet more than holding the knees/hips and back straight), but a good orthotic can be a firm shell with a soft topic - this can give the best of both worlds. A good insole like Powersteps can be a great starting option for 90% of people.

    I have flat foot on both feet but lately I have been dragged one of my feet .... I haven't noticed but my family have ..... Is that normal for a flat foot hopefully I get a answ soon I have a app on the 10-6-22 for see a orthopaedic surgeon due to dislocation twice on one foot

    Why my toes tingle when i stand still for a few mins ? Then it kinda spread towards the heel. I wear arch support because of pain in heel. No more heel pain but now im getting the tingles and at times numbness. Havent found any videos regarding the tingles and numbness. Please help !!
