      No orthotics have helped me. No shoes help. Only some store bought orthotics have reduced daily walking pain by maybe 15-20%.

    I just had flat foot surgery and bunion removal on my right foot 10 days ago. Very little pain after surgery because I was prescribed pain killers . I'm now in a cast for six weeks. Hopping in a cast is horrible for me. And also sleeping on my back is killing me. Upon leaving the hospital I was given crutches I fell first week then I got a walker so much better. The worst part about being in a cast for me is using the bathroom. I go to see my doc two days.

    I've really appreciated your insight on various foot conditions. I had a podiatrist recommend the traditional flat foot correction surgery. He didn't really spend a ton of time with me on it, so it scared me to consider it. I've recently had surgery for another foot issue, but I've had so many problems arise because of my flat feet lately that I'm wondering if this is an option I should consider in the future.
    I'm still wanting to give the custom inserts a shot, but I keep getting them made to where they're either way too stiff or too soft and they don't include a morton's extension for my arthritis.

    I really enjoyed your crital thinking in regards to this issue, I have a collapsed arch in right foot and flat foot on left, I use both a custom orthodic and sports orthotic inserts with top of the line asics gel kayanos for pronation...iam a daily exerciser and quite athletic, yet am aging...with weight bearing use particularily running the custom orthodic ($480)is not enough cushioning so i wear the sports orthodic ($30)and I am able to withstand a 1km run, however I am finding that I am experiencing pain long after activity that begins in arch goes into the soleus, calf, medial and lateral sides of knee up into hamstring and buttocks, I am starting to get concerned that I may be causing myself early onset osteoarthritis due to over use with misalignment...from eatching your video I will keep doing the measures I am currently, aim to increase swimming and non weight bearing exercise and in a few years reassess if I want to run the risk of surgery. thankyou very much.

    I have had my reconstruction surgery 4 years ago. Now, my ankels are painful with a burning sensation along with the whole foot. What should I be doing?

  • @ostermanumbrellaco.7445

    i just saw a doctor today and he suggested that if i really wanted the surgery i could either have it done on my right leg and keep my left leg alone because i have a stronger ankle on my left or do both. I chose the do both option and the doctor said it’s a 70% that the surgery might help and it is not a small matter because i am still in my teenage years. The surgery he wanted to do was a keyhole surgery at the back of my ankle and insert a plastic-like polymer into my foot in the front. I have a SPEC scan coming up soon in 2 weeks time, i have tried OMEPRAZOLE and DICLOFENAC but every time i take it i get an extremely painful stomach ache and watery stool for hours. I have an extra lodged inside right behind my ankle in both feet.

    I spoke to the top Foot and Ankle Orthopedic Surgeons at the Hospital for Special Surgery and the don't seem to perform the stent or pin surgery but rather take them out of unsatisfied patients. I injured my PTT from Ironman Triathlon competition and what helped me was wearing orthotics, avoiding running up hills or longterm use of the elliptical. If you're in stage I or II of PTTD then start conservative, but if your malleolus is dragging on the ground then it might be time to discuss the FDL tendon transfer with your Orthopedic Surgeon. The new tendon will be weaker than a normal tendon but a win win for an abnormal or torn tendon. All injuries and pain suck so try and find a workaround.

      Yes, gymnasts do not compete or train in shoes. However, in between they use shoes as other people, and that would be the time to use the best shoes possible. Even walking at home, taking a dog out, shopping, and to school in quality shoes will benefit the health and well-being of their feet and knees! That could, over time, translate into less pain and greater strength of the athlete's knees and feet.
      If possible, we would appreciate a quick and easy rating from you, and a good rating makes a BIG difference for us!

    When little my orthotics were cast with my feet being held up slightly lifted (like if wearing a bit of a heel) while laying on my stomach. And so the actual orthotic had a tiny bit of a heel as well as going up a bit higher than usual for additional support. I had to wear mid or high tops so they wouldn’t show but that offered even more support. They broke and the podiatrist school was closed down and custom orthotics aren’t made like this and they don’t help as much anymore 😢

    Podiatrist yes to Subtalar Arthrodesis (SA), but Orthopedist no to it. Orthopedist said that SA still results in impingement (big pain) via the rod instead and so while the scans and visuals are good the pain is bad. Orthopedist said they had to remove these due to people’s pain. I want to know if I can do high impact sports with the SA or if the traditional surgeries are better?

    I have Posterial Tibial Tendonitis Disorder and I was born with no arches. Around 24 yrs ago I started wearing custom shoe orthotics and wearing different shoes due to pain caused by my flat feet. It worked wonders for my pain up until 15 years later when I fractured my ankle and was diagnosed with PTTD.
    For the past 6 years I can no long stand in one spot for too long, ride a bike or go for a jog without my ankle swelling or me limping for the next week or 2 and being in extreme pain. I got 3 different opinions from Orthopedic surgeons and a foot and ankle restoration specialist last year. All told me I needed surgery. One told me to avoid custom bracing but If i wanted to shy away from surgery that i could switch jobs and have a desk job. The others told me that the last straw before surgery was getting a custom brace which I have now. The brace speeds up recovery time and reduces my pain a little which allows me to walk a little better when its inflamed. I also aquired Plantar Fasciitis in both of my feet not long after my fracture and it bothers me often, especially in my left foot which is my good one. It's because of my bad right ankle, since my left foot will overcompensate for when my other foot is inflamed. I always change shoes or wear sandals after my 9 hour shift everyday. I was going to get some recovery type sandals but my foot was borderline hanging off the edge because of my flatness. One Dr suggested minimum invasive calf surgery for my tendon. Where they cut my fascia in my calf and lengthen it, since its believed that calf tightness is the cause of many types of foot pain.
    I'm currently doing weekly sessions of accupuncture for my ankle and its helping the pain allot but I dont know if it's a solution for long term pain management.
    I want surgery but fear that I'll never be able to ride a bike again afterwards, do the type of work I enjoy and fear the recovery process. With my situation can you think of anything that might alleviate my pain before considering surgery?
    Also I gained around 80lbs during my recovery from my fracture I'm now at 270 lbs. Would weight loss of 50+ lbs significantly reduce my pain and swelling do you think?

    • @DrTomBiernacki
      @DrTomBiernacki  Před 3 lety +1

      Unfortunately I'm not able to be specific for your case, but in general with situations like these we find that there is constant stress and inflammation leading to sore nonflexibile muscles, less weight and the best possible health definitely always help in every situation, then good shoes, insoles, bracing, exercises such as resistance training over repetitive running/walking and then controlled increases in flexibility are usually best. Surgery is sometimes beneficial, but personally we feel we are getting away from it more and more with better PT, home care/OT and better shoes and products on the market. Surgery won't guarantee pain relief Unfortunately but can help.

      @@DrTomBiernacki What are some examples of better OT, PT and home care? In 2016 I was out of work for 3 months due to my ankle and underwent PT for those months as well as resistance exercises at home. My ankle was better for 2.5 years but then it came back and worse then ever. PT only worked for so long. I've suggested PT to my Dr and he says it's only going to be a temporary fix like my last time, that it's not going work. I've also mentioned weightloss and was told it wouldn't do much. I was told I've tried everything non surgical, that most people only need insoles and changing shoes, just like you are saying. The shoes I wear are Hoka One One Bondi, these were recommended by my Dr. They help alot but only do so much.

      @@DrTomBiernacki hey thanks for taking interest into my testimony. So I had bunionectomy plus hyprocure implants on both of my feet. I have hypermobile joints hence I have flexible flat feet and that makes me the perfect candidate for hyprocure. Based on my experience, the procedure for hyprocure implants was short, simple and fast. It took about 20-30 minutes. It was done under spinal anaesthesia in my country. The incision wound was only like 1-2cm. The wound itself wasn't painful but the stretching of ligaments and tendons does. Since flat feet have shorter ligaments due to the bony structural collapse, when it was repositioned, it stretched some of the ligaments and tendons in the foot. But the pain was manageable when I took anti-inflammatory drugs. As for when I started walking, my podiatrist advised to not start walking until the stitches is taken off. The stitches was off after 2 weeks, so I started walking then with the help of orthotics/ankle guard to give support and reduced the pain. It took me around 2 months to not feel the pain and was able to drive and walk normally. I didn't even feel the implants. Although walking 'normal' like other people was a bit difficult at first because I was so used to walking with overpronated feet. Physiotherapy really helped to guide me on how to walk normally. Overall for me, my body able to accept this implants and my overall lifestyle changed like I have less lower back pain, no pressure on arch and less calves cramp. I'm able to stand and walk for a long time without having all those symptoms. As for running, jumping or doing other vigorous activities, I'm not able to comment on that yet since I just had these procedures done 4 months ago. But so far I'm satisfied with my current condition. I'm so sorry for the long explanation. I'm trying to make it short but i really hope that my testimony can help others to decide if they're advised to have this procedure.

      @@DrTomBiernacki oh and I forgot to mention that I used the orthotics only at the early stage of recovery when I started walking after the surgery. It helps to support the new arch and reduces the pain. After 2 weeks I can walk without the orthotics. But I do use them sometimes, I mean I already bought them so might as well use them. But I'm just trying to emphasise that I don't need the orthotics once I can walk normally. The arch stays in shape even when I walk on flat surfaces.

      czcams.com/video/5M7ngir46yQ/video.html Take a look at this video; even though you don't have osteoarthritis, the concepts of a biomechanical exam may be the answer for you. Generally, an orthotic is a tool that can help hold the foot straight, but if a knee, hip, hamstring, glute, thigh, or lower back is tight, it will injure those muscles. The key is being held straight while massaging, loosening, stretching & strengthening the tight joints/muscles while gradually holding them more straight. Surgery general, will not fix these issues and may lead to even bigger injuries.

I have what you could call severe flat feet, meaning i have normally zero arch, but i can fake it rather easily if i think about it, just cant hold it for more than a few hundred steps, haven't used orthotics since 18 and im now 33, they never really did anything for me. what concerns me is that i've heard many people complain that about 50 years of age i could get problems and maybe i should get it fixed now, but i really don't want to be in bed for months and the regular operation to me seems to be out of a gore movie should i fix it?

They are part of a plan, and the theory is sound, but they are also designed and touted by people that don't use them. For me it has been like bringing the floor up at the point of correction to bear a disproportionate amount of the total weigh to be distributed. My son has extremely pronated flat feet along with tarsel coalition in both feet. He is 20 years old. Is there anything that can help him and would this help him considering he is also dealing with tarsel coalition?

      In my experience while yes it is difficult to do anything to 100% stop a tarsal coalition, it is beneficial to use proper shoes and insoles, most of my patients have almost no problems while doing this and even working heavy labor jobs

    Shoes and inserts only work so much, honestly they didn’t work for me and I’m on my feet 12 hours a day at work. I had to get the Hyprocure surgery and I will admit, it really does work. My feet are aligned, and I no longer feel horrible ankle pain.

    • @DrTomBiernacki
      @DrTomBiernacki  Před 4 lety +5

      Absolutely no one is saying it doesn't work, it's just that there are real risks to these procedures and that they should not be first line treatment. Personally I see 95% success without moving onto surgery, although many people want it right away. Knee, hip and foot implants can have issue in the future as well, 10, 20 and 30 years down the road lead to reoperations and future issues.

    • @TS-yn9ey
      @TS-yn9ey Před 4 lety +2

      I agree, of course they shouldn’t be first line treatments, and no one is saying they should be. But if shoes and inserts don’t work, then other options are available that do. I know people with this procedure that have not had any issues for over 10 years.

      @DrTomBiernacki  Před 4 lety +1

      I'm really happy you are pain free, and you sound like a great candidate for the procedure that is doing well!

    • @DrTomBiernacki
      @DrTomBiernacki  Před 4 lety

      I'm a big believer in getting to the root cause of pain, and it's not just orthotics and shoes, but work optimization, muscle and strength optimization and much more, there is always other options. The results of surgery are not bad but there are always ways to get creative and find a solution for everyone.

      We know you want a more detailed response, and believe us if allowed we would love to give more specific information.
      Unfortunately we have to let you know how important it is to see your podiatrist in person to get a more detailed response.
      There are just so many variables and risks for giving information without a proper physical exam in person.
      Good luck and we hope you feel better soon. We really appreciate you as a fan!

    I have fibrous calcaneonavicular coalition in both feet; mine “popped loose” and turned to bone when I tried orthotics because they raise the front of the calcaneus; now they hit even when my ankle rolls in the slightest; I would give anything for them not to hit and swell after a long day on my feet; I have “all” the shoes and all the orthotics and nothing really works; in 4 years they have taken me from a relatively normal life to daily problems forcing my feet flat

    I had flat folt reconstructive surgery at the end of August and my dr. told me it is now ok to start wearing a shoe and walking on my foot. It's painful but not nearly as painful as it was after surgery. It seems like my foot is going outward now instead of inward like before but I assume that's because I'm not used to using the repaired foot. I'm sure physical therapy will help me walk on it properly. That will take place in about 1 or 2 weeks. Hope this helps flat footers!
