    Low-dose oral minoxidil is slowly becoming a mainstay in the therapeutic ladder pattern hair loss both in men and women. However in my practice, the first line remains topical Minoxidil due to its well-established safety and efficacy profile. My protocol is explained in detail in this video.

      Hello doc. I am using 1.25mg minoxidil for a year. I see no side effects but no new hairs either. Should I increase it to 2.5mg daily? I am scared of side effects. I also have too much dandruff so topical is not possible for me.

    My question is the combination of Oral Minoxidil and other medications like Cialis 5mg daily and the complications this could cause?
    Would using topical have the same complications?

      Thanks.
      Topical does not have any effect/complications since amount absorbed into blood stream is miniscule.
      If you are in good cardio health and your doctor approves of you taking OM, taking it with Cialis is just fine.

    Great insights thank you doctor. Your last point, you mention stabilising the patient on Minoxidil and discontinuing Finasteride, is that correct or is that supposed to be the other way around? I find comfort knowing that I can stop the minox after 6 months or so and maintain the gains with finasteride.
    PS can I take this at night to reduce blood pressure drops during the day?

      Oral Minoxidil with Finasteride when the hair loss due to androgenic alopecia is rapidly progressive. Minoxidil can be stopped once hair loss reduces when these medicines exert their effect.
      Minoxidil when taken orally is best consumed at night before bed time.

    Hi Doctor, if you have low sulfotransferase enzymatic activity, does Oral Minoxidil work? I know that topical wouldn't work, but not sure if Oral would overcome that issue? I was tested for my sulfotransferase enzymatic activity levels and it came back confirming I would be a non-responder for topical minoxidil, so I'm hoping Oral minoxidil will overcome that issue. I would really appreciate your feedback on this.

      Yes I have had 2 procedures for my grade 6. One FUT in 2007and FUE in 2012. I am on Finastride to stop progression but since it is stabilising now I will stop it very soon

      Thee sides are not fear mongering they are real. However healthy people can take it without much concern as long as they are being followed up by their doctor. Not to be taken OTC.

    Hey doc, can you consider making a video on the use of robotics (ex: ARTAS) for the extraction process? Have they come a long way since first introduced? Is there a particular reason why you have yet to incorporate it into your practice?

      I cannot comment on ARTAS since t is trademarked. However robotic hair transplant never took off. many machines are lying vacant. Several adverts can be googled where clinics want to dispose it off. Also this aside, will you pay USD 8 per graft?

      @@hairtransplantinindia That would explain why I see more and more robotic hair transplants being available at new clinics. I would never pay $8 USD/graft. I have seen clinics offering it for much less.

    Hello - thank you for a very informative video .
    May I ask your opinion of a relatively new product developed by dr Andy goren - it is for people who lack the sult1a1 enzyme and who do not respond to minoxidil .
    The product is available through Daniel Alain - and is to be applied before applying to minoxidil to help make the enzyme needed to process the minoxidil .
    I just started using it - other than a lot of shedding - it’s too soon to know .
    Would love your opinion .

    Thank you so much for your highly informative video. I was prescribed 2.5 mg of oral minoxidil as a 37 year old female who cannot use tropical due to dermatitis. I also take 100 mg of spirinolactone. Does 2.5 mg seem too high for a female (5’9”, 140 lbs extremely active) re side effects? If so, what dosage makes sense? Thanks!

    I am a 22year old female. I have had hair loss for over 10 years. I have been using topical 5% minoxidil for approximately 4 years. I started on oral minoxidil 2.5 mg recently and I have been having amazing results. Seriously amazing results. My part is very tight anywhere I part it on the top of my head. That has been unheard of for me. The top of my head always looked like a sparse forest, and going out in the sun, you could see my whole scalp. It was devastating. There's so much more to my story, but I'm a believer in oral minoxidil.

    I started 2,5 mg of oral minoxidil and 0,5 mg finasteride once a day in October ( thanks to your advise I will start taking at night the finasteride )
    -Do you think is the right dose ?I am 41 years old with spread general thinning and hair loss
    -when should i do my first finasteride break ?
    -and also I would like to know when I can start swimming in the beach after my hair transplant?
    -when I should stop finasteride and minoxidil ?

      Please stop medication 7 days before procedure. You can swim after 3 weeks. The right dose can be assessed after blood tests and consultation by your treating doctor.

    I was for a couple of months on 2.5mg of oral minoxidil, but I had elevated transaminases maybe due to it. I also had lower than usual leukocytes and platelets. I think the long term safety still remains to be established overall

      I agree. It has not been in use for any indication for a significant period of time and hence has to be prescribed only occasionally.

    Hi doc!! It’s really urgent so please do reply. I’ve been on oral finasteride for about 4 months but I don’t know how to tell if my hair fall has stopped. I lose like 8-10 hairs when I take a shower and might lose a couple when running my fingers through it. One difference I’ve noticed is that before my thinner hair would fall out. Now its the thicker ones.

      We do lose 100 hairs every day since the hair cycle is ongoing. These are very few hairs you are losing. You should be worried only if your hair are thinning or hairline is receding or a bald spot is developing.

      Oral minoxidil has to be prescribed by a doctor and it is best to get your BP checked beforehand. Hence, I would advise you to consult a doctor who can check your health and decide whether you can take it or not.

      You're on the right medication advised intelligently by your treating doctor. You need not use anything else. Serums have questionable utility.

    Hello Doctor, my hair is so greasy that I have to wash it everyday. I tried topical min for about a month (twice daily) and stopped as it was making my hair greasy. So my doctor now prescribed me : 1) Minogain (1.25 mg a day) 2) Dutacet 0.5mg a day.
    But I am also concerned about the potential side effects and wondering if I can take topical min every night before going to sleep. So that I can wash my hair the next morning. Do you think this will work? Thank you

      Yes that is the best way to apply Minoxidil at night and wash off in the morning

    Hello Doctor. Just curious to why you may or may not use minoxidil or fermasteride? I’ve noticed your hairline has receded (which gives off a look of intellect) but have you not attempted to use these products you prescribe? Are you comfortable with the natural progression of hair loss? I have debated whether I’d like to get treatment to hold onto my hair or just allow nature to take its course. Thank you.

      Thanks for this question but I have answered this a 100 times over! I have had one failed FUT procedure in 2007 and a second procedure using FUE . Now due to the first proceudre I have run out of grafts. I do use Oral Finasteride 0.5 mg on alternate days but not Minoxidil. At 58 I do not mind some recession and thinning at all.

    Have been on 1.25 mg oral minoxidil daily for 1 week. It works very fast as I noticed less hair loss now. However, since I started 1 week ago, I have been feeling tired and low energy. Will this persist or it’s temporary ?! I don’t want to stop it but I’m concerned about this fatigue. Any advice ?

    Hi doc, great video, I've a question: After a full year using 5mg OM(2.5mg Pm and 2.5mg at Pm) along with Dutasteride, can I stop using OM and just maintenance my hair with Dutasteride because the main reason of using Dutasteride(or Finasteride) is not justr grow the hair but keep it, am I correct??       You are right but Finasteride should be the first line of defence.

      As far as i know, hair grown by minoxidil will fall out if you stop taking it. Fin/dut would work on DHT affected hairs only. So be mindful of that.

    Hi Sir, I had a question, I am not sure if this will reach you given the video was published 2 years ago. I want to know if the efficacy of minoxidil decreases with time? For eg: I read somewhere that a person who was a great responder to minoxidil lost all the hair gained from the medication 2.5 years later. Do you have any patients to whom you prescribed this medication and is still going strong in 2+ years time frame?

      That is not true. Minoxidil will never stop progression of baldness. The course of baldness will continue with Minoxidil. The function of Minoxidil is to keep your hair thick. To stop progression is the function of Finasteride.

    Doctor if someone is changing to oral minoxidil from topical minoxidil because of the clinical evidence of its superiority....will he have any problem and lose ground because of the scalp getting adjusted to topical minoxidil?

      Yes Oral Minoxidil will be in place of topical Minoxidil. Get your doctor to do a detailed cardiovascular examination before he starts you on OM.

    You lost me on that last statement "once stabilized you stop finasteride"? Don't you need to continue with the finasteride always as the "anchor" (the DHT blocker) , even when taking the oral minoxidil?

      What I meant by that was that if you have been taking finasteride for a long duration ie 1 year or longer, you can stop taking it for 15 days after every 3 months to assess whether you have hair fall or not. If there is no excessive shedding the dose can gradually be tapered and eventually stopped. No shedding would mean that your baldness has stabilized. This is especially in those above the age of 35 years as the baldness gene gets suppressed.
      Both minoxidil and finsteride have a different mechanism of action. Oral minoxidil is an antihypertensive drug that causes vasodilation and hence increses blood supply to the scalp improving hair growth. Whereas oral finasteride is a DHT-blocker and it blocks the action of the hormone responsible for baldness on the hair follicles. Hence, oral minoxidil is not a substitute of oral finasteride because if your baldness has not stabilized, after seeing the initial effects of oral minoxidil, hair will eventually begin to thin out over a period of time.
      Also, please do not start oral minoxidil without a doctr's prescription.

    I started oral finasteride two 9 weeks ago( two months and a week). In the two first weeks, I felt like my Shedding has decreased since I was already shedding a lot of hair before I started the treatment. But considering from the third week, my hair my hair was Shedding aggressively and I can say More than ever. According to your knowledge, when will this Shedding be halted by the drug? And when am I supposed to see the efficacy of the drug in matter of regrowth or at least in hair improvement?
    Also in 11:38 doctor says: he puts the patient on Oral Minoxidil for 3 months to stabilize the hair loss then moves to topical application! What if the patient sheds from all over the scalp ( due to telogen effluvium which can be chronic or due to another reason), the logic says if you move to topical, the other Areas will be affected since topical is generally applied on the top. Enlighten me please regarding this point too.

      Shedding is a good sign after starting a newer medication for hair loss or after changing brands since different brands have different polymorphic forms of the same compound. Shedding stabilises by 4 weeks in most cases.
      As far as telogen effluvium is concerned, it needs to be treated a little radically and hence the recommendation. Oral form will affect hair on top and back of head unlike topical form which is traditionally applied to the areas that are prone to thinning i.e. top of the head.

    I was taking 1mg finasteride 2 years back,I stopped after taking it for 3.5 months. Once again I started taking it after 1 year but I didn't find it effective. So from the past year I have been taking dutasteride 0.5mg 2 tablets a week. Since I was a bit irregular in taking these tablets. So I never able judge efficacy of these tablets correctly on me. Now can I take 2.5 mg oral minoxidil (daily) along with 0.5mg (weekly once)?!. All doctors are not aware of oral medication of minoxidil , many doctors don't do experiments , they go with only fda approved medicines.

    In your 50th point you said something about baldness genes turning off, how is that possible? I'm no doctor but as soon as anybody stops finastride their loss progresses so that means that baldness gene is still active. Also wouldn't stopping finastride be a bad idea since dht will harm the follicles again?

    hello doctor , I have been using topical 5% minoxidil (morrf 5%) with finasteride 1mg for approximately 3 years.after getting results with some side effects like dizziness,dandruff,itching . when i applied it in summer due to sweat minoxidil was flowing on my forehead .after applying twice daily my hair was not looking good ,combing was so difficult. Twice daily application on scalp at now in bussy days is impossible. so 15 days ago i went to the doctor she prescribed me oral minoxidil 2.5 mg night time(after dinner) and 1mg finasteride at day time(after lunch). now the problem is after taking om 2.5 mg and finasteride 1mg for 12 days, my hair fell out . is this normal? is this hair fall or shedding ? If it is shedding then how long will this shedding continue? PLEASE reply doctor i will be always grateful to you .thank u

      Shedding will happen till 4-6 weeks. It is a good sign- that the medicine is effective. Nothing to worry. Just stay the course. You are taking oral Finasteride at the wrong time. See my video on "100 fact about finasteride dr bhatti"

    1. Are all brands of oral minoxidil made from the same minoxidil? Or does each minoxidil brand have a different quality? 2. The use of topical minoxidil in men is as much as 5%, if converted to units of mg, how many mg is that? So that it can be equated with its use 2 times a day.

      1. Is there a link between extreme itching in the head and DHT accumulation? Because I experienced that even though I didn't have dandruff at all.
      2. Is dermaroler on the scalp useful for forming lost/shrinking hair follicles?
      3. Can I replace my 1 mg finasteride consumption with 0.5 mg dutasteride every day? Because during the 5 months of using finasteride 1 mg, I didn't feel any change in my hair loss. Why did it happen? Is my DHT really too high that it can't be treated with just 1 mg of finasteride? And are there any major side effects if I take dutasteride 0.5 mg every day?

    If he thinks that this growth Is enough for him ( Thinks to stop finestride but scared of losing hair gained by finestride)

    I live in EU where i could not get minoxidil in solid form tablet. I'm using topical solution Regaine drops oraly, slowly bilding dose to 5mg in 2-3 months. I would prefer tablets but it is not possible for me. What is your thoughts about the difference in efficiency between solid and liqud form of the drug. Do you think ingesting with or without food matters. What about the time of the day?

      As mentioned in this video, the best time is at night so if a person is prone to low BP, the effect is not felt. Take the medication after dinner. I do not have any experience with ingseted topical Minoxidil. That is not how the drug is to be consumed.

      Minoxidil ingested in topical solution form definitely works on every hair on the body. Longer eyelashes, eyebrows, and tiny little hairs are growing everywhere. Now I'm ingesting 10mg around noon and 10 mg at midnight. Swelling in the feet is happening from time to time but it is not really a problem. I get some heart palpitations here and there. This drug is not for hypochondriacs or people with weak hearts. I'm over 50 and I started this treatment much too late.

    Doc, I have been taking topical minoxidil for 3.5 months, it's effective ( I see thin hair on frontline area).but itching and white powder flakes.( Adding 120 drops tea tree into 60 ml minoxidil solution and using just 2 days. My itchiness reduced very much ..but not go away white flakes . should I continue this for 1 week) or switch into foam or oral minoxidil.whats your opinion..if I use oral ..what will I do , taking 1.25 mg minoxidil and topical solution simultaneously..and if I get result completely remove the topical one.. please reply🙏

      Yes there are reports that it may not be reversible. Oral form being more potent will cause greater hypertrichosisi.

    I'm using fin for almost 2 months now .. primary shedding phace completed befor 3 weeks .. nd very less hairfall now max 3-6 hairs .. before any medication i used to have 30-40 hairs per day or more !! I also started to use minoxidil for over a month now but noticed acne nd blisters on my temple nd hair also appears oily... I'm planning to start on 1.25 oral minoxidil along with 1 mg Finn on alternative days ..

      The acne might have been caused due to repititive itching which led to infection. You could consider switching over to the foam formulation of minoxidil which has lower or no alcohol content. This could eradicate your skin problems.
      Talking about oral minoxidil, please remember to obtain a prescription from a doctor after he has checked your blood pressure and taken proper medical history.

    Topical minoxidil caused a major rash on top of my head of forehead. Had to see a dermatologist to get if taken care of. Switched to oral M and no issues. Too early to tell if its making a diff (only on 2 months)

    I have been on topical minoxidil 5% for the past two years using it religiously both morning and evening and have seen really no results.
    I am looking to start on oral minoxidil and oral dutasteride. What I have heard is that the oral Destri is better than the oral finasteride and has less sexual side effects is that true? Can you please provide me your thoughts and recommendations?

      Dutasteride blocks all DHT and hence cannot have lesser side effects. There is less talk about this on the internet since it is not FDA approved for hair loss. Hence competitors cannot sully it's image like they do for Finasteride. It's the murky pharmacology industry to blame.
