      @@hairtransplantinindia @Darling Buds Clinic sir as you have explained in ur diagram that when testosterone levels increases by 15% and goes to brain/bone zone and increases Esterdio levels. I have taking finasteride 1mg/day for the past 10 months.
      So what tests shall I go through so that If I am not getting any side effects/increase/decrease in anything which can be harmful? So that I can be free from.mental tension like of increased esterdio levels, effects on breast or any other thing. I just want to check If I am doing good without any major issues in my body as i m on Finasteride. Please name all the tests and I will get them done & show you the reports. Please help thanks.

      @@superbsuyash1466 You are doing fine. If you do not have a problem all is well. No need to check levels. Levels after starting Finasteride will not correlate with actual situation.

    I think with regards to testosterone, whilst being on finasteride, is that TOTAL testosterone will increase but FREE testosterone will decrease. The reason for why total testosterone increases was perfectly explained in this video, but the decrease in free testosterone is because the lack of DHT means that more testosterone will bind to SHBG, hence reducing the levels of free testosterone.

      @@hairtransplantinindia So if the above comment is correct. Finasteride will lower Free Testosterone and since we have less free testosterone it should decrease muscle mass or maybe decrease muscle building? Can you shed some light on muscle mass and finastride?

    I am a 53 year old man who has suffered from anxiety and depression ever since puberty. Got off my antidepressant 2 years prior to taking finasteride. That was a big mistake. Went through hell emotionally, and ended up stopping the drug. Thus losing a good amount of hair. I am back on finasteride and prozac, and seem to be doing well. Guys, it is possible to take the drug if you have emotional issues. You really have to monitor your mood, and continue taking your med/medications. Great video by the Dr. as always by the way. Working out also really helps, and also watching your diet and alcohol intake.

    DHT is there for a reason. It’s not just some troublesome hormone. Reduce it then, it just results in chemical castration with finasteride.

      @@hairtransplantinindia it's not just a phobia it did effect me when I used it both topically and orally. My libido was completely gone and I tried it for months going off and on trying to see if it can work for me as I really wanted to try it. Had to get a hair transplant and even Dr told me to use it after hair transplant and I told him the side effects I was having. He told me use minoxidol topically only

    No matter what you day doc...I personally know many people now who had got the side-effects of this drug and are still not able to fully recover even after quiting it

      If counseling is not done properly and the 40% incidence of ED in age group 25-35 (same age as hair loss) is not kept in mind and wantom prescription of Finasteride happens, high cases of ED have been noted. They are irreversible not because Finasteride is such a bad drug, but because it has meerly unmasked existing hypogonadism/ED.

      @@hairtransplantinindia does this mean that the people in the age of 25 to 35 should stay away from it and how can one know if the Ed happened is because of finax or it was preexisting . One would certainly blame finax only if they notice sudden changes after taking it.

      @@abhinavtiwari4238 If your total testosterone level was normal and you do not suffer from hypogonadism and do not have major psychological psychosomatic issues, yes you can blame Finasteride for it. I never have had such a thing with my patients since i do a detailed assessment.

      @@abhinavtiwari4238 In such a case if baldness is not stable better embrace your baldness. If stable, you can give hair transplant a chance.

    I’m glad I finally started finasteride a few weeks ago I wish I started it years ago but I got scared off by misinformation. I read the pamphlet and the vast majority of study participants had no adverse effects and almost all had significantly more hair than placebo. It also actually increased my testosterone level, not decreased it.

    Most of the people who suffer severe Fin side effects are hypochondriacs, they are people who obsess on every little thing. With erectile dysfunction; they will take Fin - they then go to have sexual experience, think back to the Fin post they read on a forum; they panic, and that causes their erection to go down. They then blame Fin, and stop Fin - but now they have these thoughts in their mind and they still have ED - so they think "I have post-Fin syndrome" when it is all in their mind.
    One guy who was complaining of severe Fin sides such as ED, later admitted the first time he ever took Fin literally 2 minutes later, he started having bad brain fog and heart palpatations. That's when we realized it was all in his head, as the Fin wouldn't even be absorbed after just 2 minutes. It was his mind panicking - he'd be the same if he took a sugar pill.

      I fully agree with you. This has been my experience as well. With so many people petrified of Finasteride, I am planning a host of videos only on Finasteride si I can help them get over this myth.

      @@hairtransplantinindia Yes! Please do this, it will sincerely help myself as well as others. The concern I have is not that Finasteride can cause side effects (since all drugs cause side effects) but that an FDA approved drug could cause permanent side effects. That is what I want debunked.

    There are Two type of DHT
    1) Scalp DHT
    2) Serum DHT
    Finesteride just block Serum DHT .
    Which medicine will help to block Scalp DHT ?
    Q2 .
    Does microdosing like 0.2 mg or o.5mg of oral finesteride for long period of time is affective ?? And who is the best candidate for it ??
    Q3 .
    Please let us know how to stop the minioxidil and finesteride without affecting their results ??

    I 100% had sexual side effects from finasteride. I had complete shut off of Libido, loss of morning erections and about a 70% loss of erections. I had hormone testing and my testosterone was extremely low. So were my LHT and FHT levels. I am 5'9 and weigh 130 lbs so I am by no means over weight. I stopped talking finasteride and started taking an estrogen blocker(clomid) for 3 months and only then did some of my libido and ability to have an erection come back. I am slightly concerned that the effects maybe permanent. I am not an anxious person nor am I a hypochondriac. There is something going on here that you may not be aware of.

      You were improperly assessed before initiating Finasteride. You were not fit for starting this DHT blocker.

      @@consciousreverie4696 Several videos dedicated to Finasteride counseling, selection and side effects. Also strategies to use minimal dosage to nullify likely side effects. The trouble is most people do not get counseling by experts before starting Finasteride.

    Thank you for a useful and clear explanation. I've been taking Finasteride for 12 years and although I think it has helped me retain most of my hair, I do still have worries that it may be causing side effects or damage to my body - I'm particularly concerned that as you explained, Finasteride increases Estragon/Estadiol levels, and this can lead to unwanted side effects like Gynecomastia. Is Gynecamastia common with Finasteride?

    DIM and Calcium D-Glucarate can lower the excess E2 hormones from Finasteride while increasing the "good estrogen" Estriol, also cutting out Xenoestrogens from products can help keep Estradiol lower.

    Hi Doctor, great video filled with valuable information! Question, what are considered normal levels of testosterone & estrogen in males as a good candidate for finasteride use? What levels of testosterone considers too low and levels of estrogen too high to realize one is not an ideal candidate for use of finasteride? Thanks for any insight you have on this!🙏🏼

    as a paediatrician myself , I appreciate your dedication to your craft . One question i would like to ask to you ( I have even asked my senior consultant dermatologists) is that how far will dutasteride affect other dht depend follicles like beard hair? I have seen randomized albeit minor studies wherein the it has stated that it does affect minimally .

      90% DHT is blocked by Dutasteridebut by Finasteride- 70%.
      The latter does not affect beard growth therefore. However with Dutasteride I cannot say since it's been in vogue only past 5 years. We know so much about Finasteride since it is in use for 40 years. But since it does not bock all of DHT, the effect should be negligible.

    Thank you very much for this video doc, it is really nice that someone debunks finasterid myths on the scientific basis.
    I have one question that really interests me about this topic. Namely, does adding arimidex to finasterid has “steriodical effect” on healthy male person? If yes, does in that case arimidex must be taken also on daily basis? (I am aware that is not responsible to advise use of arimidex for such purpose, at least not under endocrinologist supervision, but i would like to hear Your opinion just for orientation).

      It is not a good thing to play with the hormonal milieu in your body by taking Finasteride and Arimidix. Te later has plenty of side effects on it's own. I do not use this drug for my patients.I would much rather ask him to stop Finasteride or alter his lifestyle.

    Finasteride is not for people whose Testosterone levels are in the lower range. Because their libido is highly dependent on their DHT level. When you are young in 20s then Finasteride is fine

    Thank you doctor for a VERY well explained video. Many of my lingering questions were answered. I'm not a novice on these topics at this point in my life but your diagram helped reinforce my understanding and helped me eliminate some confusion I've had with other conflicting videos and discussions I've had with others. So as I understand it, although Dutasteride increases scalp testosterone to something like %100, scalp testosterone really doesn't have much of an effect on hair loss since it's really about the binding affinity of DHT (which is 5x more influential on hair loss from what I understand) to the follicle? Even though testosterone is an androgen as well?

      Testosterone is very at binding with SHBG as well. Testosterone also affects hair, but instead of going totally bald, you'll grow very thinner hair in 50's because testosterone also affects hair on scalp but it is order of magnitudes less than the havoc DHT causes on your scalp from increasing scalp tightness to hairloss.

    Unfortunately there is still a very negative effect from finasteride: and it is decrease of DHEA hormone.
    I've been using finasteride for two years and started to look much older. Because use of finasteride decreases production of your facial sebaceous glands. Because not only your dht goes low, but also your dhae go low.
    You will suddenly feel the difference in the morning when you wake up your facial skin will be very dry. Every single day. And now I have black circles and my eyes.
    As 1 doctor told me you will feel that you got old when your face in the morning wouldn't be oily anymore.
    There are many finasteride forums were young guys over 20 years says that they are so happy that after starting taking finasteride their acne has gone.
    Unfortunately, they have no idea what new problem they gonna face. 😔

      The normal T:E2 testosterone (T)/estradiol (E2) ratio is 1.7 ± 0.12

    Thank you one more time for an excellent video! So what range of testostrone to estrogen levels that are good or bad omen for finasteride sexual side effects? How about PSA (prostate specific antigens) levels, i heared they are important to monitor and how can you interpret the base level PSA before treatment?

      PSA levels are an indicator of prostate function and a marker to screen prostatic tumors. This is affected when we take DHT blockers and hence the need to take these levels before starting on Finasteride. I do not start a patient on Finasteride unless his total testosterone level is more than 300 ng/dL. Below 500 level no more than 0.5 mg a day should be given. Testosterone levels are used in my practice to screen patients who will most likely have side effects with Finasteride.

    Thank you for showing us the physiology of testosterone metabolism. There are some physicians discussing the benefit of lower finasteride dosage such as 1mg 3x per week, with the benefit of having enough DHT blocking at the follicle, but less side effect. I have yet to find any research to support this strategy as the original study was only done at 1mg 7x per week. Have you found any research that would point to this Lower dose of finasteride as a potential therapeutic option?

      I have tried it. It works in those whose balding is stabilising. Not in a young man with strong family history and who is thinning at a young age.

    But a 15% increase in testosterone levels sounds like A LOT.. what if one does not take the full dosage and instead do 0.5 mg three times a week - will it be significantly lower then?

      It does not cause shrinkage. People who say this are uneducated about Finasteride and believe hearsay. A doctor will not advise it without telling side effects.

      The topic of libido is very complicated and a whole different field altogether. Free testosterone is mostly repsonsible for libido but a lot of men with normal testosterone levels have low libido. Both male and female hormones are implicated for libido across gender lines.

    Doctor , good content. Doctor you said it will increase the level of 15% testosterone by using finsteride that will be added to the third category which will increase estradiol , that is a female sex hormone which will give an impact to bone and brain in male. Which will impact in slight reduce in muscle and all( this is what I understood from your video). But what I didn't understand is muscle comes in other category of testosterone, until or unless finestride touches the testosterone that stimulates liver , muscle and fatcells. How the muscle is impacted? If the 15% increase in estradiol, what will be the impact in bone and brain?

      Testosterone has no impact on muscle mass. It is a Myth that it does. This is reinforced when patients on Finasteride develop puffy breasts (gynecomastia, man boobs). People will then take more of the unnatural proteins for muscle mass building like Whey protein which in turn will cause more thinning and the cycle goes on and on...!
      Estrogen is good for bones. I do not think increased Estrogen due to DHT blockers has any noticeable good effect on the brain. Even if it does, due to some reason patients occasionally complain of mood changes and foggy thought process. Patients only report side effects, and seldom do to come and tell how good the drug is!
      This is purely by observation.

      Their testosterone that is excessive also converts to estrogen but their body can handle this estrogen. Not the case with someone who is overweight, sedentary, has genetic propensity to developing gynecomastia and has low testosterone to start with.

    I have been taking Dutastaride for one year now along with Minoxidil. My hair looks great and everyone notices it. But It has effected my libido a bit so my dermatologist said to take the .5 mg every other day - not everyday. My libido is better. I have even started taking the dutastaride every third day and my hair still looks great. My question pertains to lifting weights and dutastaride. I am 66 years old. I do intermittent fasting, eat very healthy, have lost 16 kilos in 4 months BUT it seems despite my intensive upper body workouts my muscle don’t seem to be growing. I mean I look great, but I feel skinny though I am eating eggs, fish, beef etc. Question: Is the dutasteride blocking my muscle growth. Thank you ahead of time for your answer. Blessings.

    I read a lot about finasteride and you are a knowledgeable doctor.
    If you need a estrogen receptor, then I do think that Aromasin is better then Arimidex, since it is very easy to crash with Arimidex, just someting you should take into consideration.
    I do have a question, if the paitent is obese, can the rise in estrogen from finisteride long term not cause low testosterone from the negative feedback on the hypothalamus?

      I have not tried Aromasin. Will discuss it with the local pharmacology professor. Yes you are correct about the negative feedback mechanism in obese patients. Obese individuals rarely tolerate Finasteride. I am quite hesitant in starting them on Finasteride.

    Hello Doctor, I have heard that men with excessive body hair growth like thick back hair/shoulder hair chest hair etc have a much lower chance of side effects and a better response to finasteride treatment compared to men with very little body hair in your clinical experience is this true? have you noticed this observation in your patients? this observation was mentioned in a Japanese pilot study comparing efficacy of finasteride thank you

      Ofcourse since men with more hair usually do not have low testosterone which is the cause of ED. Finasteride just tips the balance in these cases. We do not get total testosterone levels done and hence these are missed.

      Finasteride does not negatively impact since it does not block all your DHT. I recommend the dosage you are on. But dosage of TRT/Finasteride has to be tailored specific to your situation.

      Because the deivery agent to take topical Dutasteride directly to the follicle without entering the blood stream has not yet been invented. However Polichem has come out with a version that holds promise.

      Yes this is true if side effects are not due to hypogonadism or high level of pre exiting estradiol. You need to get a proper assessment before starting Oral Finasteride.

      Yesand then it is fine- but effectivity of microdosing is side effects that develop due to excess estradiol will get alleviated. But taking an AI is a huge issue by itself. But people take microdoses though their effectivity is questionable. But it is your calling and that of your doctor.

      In my own experience, taking AI cause alot of issues. From pain in joints, overall feeling gloomy and lethargy. From what i have read, we tend to look at estradiol as something negative but this hormone plays alot of important roles in our bodies, including mood, energy, bone density etc. Shutting it off leads to more problems than benefits, but of course , this is all very dependant on each individual and any kind of drug use of this kind should be medically supervised.

I am a high level karate player, and I am afraid if Finasteride will affect sports performance. You said it doesn-t affect muscle mass, but does it affect muscle explosion?       Please do not worry. It will not affect extreme strenuous training. Many of my patients taking this medication are athletes, body builders and adventure enthusiasts. Your body composition is ideal for Finasteride.

    Thanks for a great video. I am 50 and I think I'm at my cognitive peak. I am considering finasteride. I'm a runner and I do heavy strength training. I am concerned with this claim - that ' the prevention of the synthesis of neurosteroids like allopregnanolone is caused by inhibiting 5 alpha reductase. ' In other words 5a reductase does more than just reduce DHT levels. So it could be that the disputes about whether testosterone or dht affects libido (or particular parts of mood) the most are complicated by the fact of 5a reductase levels affecting other neurotransmitters that affect mood. It's possible that dht or affected neurotransmitters affect the cns and focus in sports. Any thoughts? (the, reason I mention my cognitive peak is that with physical decline inevitable and mental decline following behind later hopefully, it's going to be difficult to know what might be going on).

      Yes the hormone DHT is essential for cognitive well being and general health of synaptic pathways of the brain. However we are not blocking all of the DHT. Very few people report this problem but as in erectile dysfunction Finasteride tips the balance unmasking the effect in those who are vulnerable. This does not happen in the vast majority though. If this were to happen you can stop the drug.

    I really don't understand how DHT that was present in my body right from my teenage years was just lying dormant in my body without thinning my hair till I reached 25 and moved to a different city. I see this effect on most of my friends. The ones who moved to a different city for work, have been diagnosed with miniaturization, MPB and started experiencing thinning after crossing 24 or 25.
    Can you explain this? I assume Testosterone (& DHT) peaks during our teenage years and early twenties and yet I and most people I know did not lose any hair during this stage.

      DHT alone does not cause balding!
      DHT is present in everyone in equal measure after we reach puberty. But it causes androgenetic alopecia only in select few. These are the people who have the androgenetic alopecia (baldness) gene. When the gene will express itself is also encoded in it. In some it can happen as early as 17 years of age, while in others only after 50.

      Aging is not in our control but our lifestyle is. Knowing this it's trivial to assume giving finasteride for this problem is such a duct-tape solution. It's like using kerosene as a coolant for an overheated engine. It is bound to wreck the whole car.

      Vasectomy is of no releance while taking Finasteride. There are no such side effects. Hair loss will continue even with oral Finastreide but will be much less after 3 months on it. Thanks for watching.

    Do u ever prescribed finestride to the patient in age between 23 to 35(bcaus of that ED side effect found in 30-40%).....and if a person between age of 23-35 find a sexual or ED side effect ...then if we stop taking finestride....Will this side effect will go gradually?

      ED side effects is less than 1% in my practice. The percentage you have mentioned is the prevalence of ED in normal population in age group 25-35 age. These are not the right candidates for starting Finasteride. I never give them Finasteride.

      1 mg a day is not a high dose. Your hair loss would be much worse without Finasteride. Ask your doctor to consider you for once a week 0.5 mg Dutasteride.

    I am an Natural athlete who likes to do bodybuilding. as you mentioned in the video it is the estrogen which makes you feel more weak in the gym. How can I conquer these side effects. I am currently using finasteride since 3 months. I track my progress in the gym. So at first I felt weaker in the gym and when I compared mi lifts I could see that I indeed got weaker since I have taken finasteride. So basically taking finasteride free testosterone in your body increases because of the drug inhibiting 5 alpha reductase. The increase of free testosterone leads to an increase of estrogens. My question is, can this increasing in estrogens be harmful to health? And can that more free testosterone increased by the drug work like doping? Like making you stronger and other

      DHT is the active metabolite of testosterone. Muscle building is a function of DHT. Increased testosterone levels can be alarming in those who live a sedentary life and are overweight. In others I have not seen much of a problem. To avoid all this Dutasteride should be taken with utmost caution since it blocks almost 95% DHT which is a lot.

      @@hairtransplantinindia thank you i didn't know dht is involved in muscle building, i thought it did just a small part and the big part of the job was of testosterone
