  • Tools are useful when it comes to improving the quality of your walking movement. In this video, I give the top ten “tools” that I use in therapy to help decrease circumduction, stop the foot from rolling, improve foot clearance, increase weightbearing on the involved leg, and improve overall walking efficiency.

      Absolutely wings and halo 😬sadly I now know my spasticity won’t just magically disappear Also love the exercise suggestions so I can do something about my hemiplegia even though I currently am not working with a therapist your time and thoughtfulness is very much appreciated Weaning myself off of my quad cane will be necessary eventually yikes like going from a tricycle to a two wheeler w/ training wheels 😬: affected leg not there yet Is walker with hand attachment piece a better bet than cane reliance for enabling a hemiplegic to stand and move around With an aide but no current therapist ( am starting to look into local neuro PT options since sitting in a wheelchair accomplishes nothing so love your exercise suggestions Affected foot is supinated so AFO is needed thank you for your You Tube pieces ( am subscribed and belled😬 a recommend your CZcams pieces on stroke- related sites e.g., Motus Nova when people are mainly seeking information re likely course of recovery where they could get the information they’re seeking from your You Tube pieces e.g., on spasticity ( what is it)

    As always thank you for what you do and taking the time to put these series together for everyone. Also wanted to let you know that since my love one saw you on TV, you got her attention. Thanks to that, you have help both us but most important he on understand how to get back. She admires you and is always checking and digesting to later on do things.     I couldn't agree with you more on the hemiwalker & the nonuse of the affected hand. I learned the hard way. It's been 3 years now but I realize that now.

    #gaittools Thank you for your videos- they are very inspiring. My injury occurred at 12yrs old, I am now 38 (tbi/brain aneurism/stroke/brain surgery) Your exercises inspire me to keep going! I even want to take a trip to your clinic to see you in action! These videos are just what I need at this stage in my recovery. Keep them coming!

    I know that much of your videos stems from working with stroke issues, but I have to say that I have all of the things with vaulting gait, drop foot and a bad knee. I've learned a lot. The main thing I learned is how to learn how to walk again. I didn't realize I had forgotten how to walk because the knee pain is so bad. The knee pain is still bad, but I am concentrating on bending the knee when walking and stretching. Thank you for sharing. This is life-changing.

    Thank You So Much!
    I just did a 5 km Race and ending up falling down! Luckily just the left side of my face and knee was bruised and scraped.
    I will use the Duct Tape Method since I did have a little catch on the ground. That is what caused me to trip. I will also get a booty strap.
    I have problems bending my knee they want to go straight legged. They also sometimes have a swing to them. The Duct tape and the booty strap is for the left leg.
    My next 5km is next month. This will give me some time to practice my new tools.
    Thank You again for the Great Ideas!
    The next day I had another Race and I was a little scared. I lifted my knees to make sure I got the clearance. There was a little slow down. Not the most effective Running Style but it kept me upright the whole time so it was Great.
    I am Very Excited to take your advise and use the 2 tools!

    Thanks for the good ideas, my disorder is not big, so I manage to improvise, like tool No. 8. I taped the shoe around the shoe. It doesn’t look nice, but it helps as I walk around the house and yard.

    Thanks a million Dr Tara , I manage to stop my elbow spasticity by just holding my walking stick behind my gluts to walk. Its gone tomorrow. I do stretch the elbow straight to loosen it too

    I can only grip a walker on my left side with my thumb and index finger. My wife came up with the idea of wrapping the left side grip with double sided tape. I use a baby wipe(or wash cloth with just water)to clean off the tape when it looses its stickiness every day or so. She only has to change the tape every two or three weeks.

    0:42 your face when you said "tools" brought a smile to my face and I had to go back and see it again. 🤣
    5:40 a simple plastic bag from fruit/grocery store would probably also work.
    6:40 maybe a length of the elastic band. It would also have a but of "give"

    #gaittools. Excellent video Dr. Tobias! I'll be looking into the 1st item you mentioned. My primary walking issue is my knee pops out to the left when I go into swing phase with the uninvolved leg. This results in sharp glute pain. After about 20 mins of walking the hip pain is so intense I can barely move. I'm sure this is partly due to leg-supporting muscles not firing and also to the uneven leg length resulting from the AFO I wear. I notice at night when I am walk without shoes or the AFO, I don't have this problem. The involved leg is straight when I plant it. Thanks for all the suggestions. Love your channel!

    I recently bought the resistance band that attaches to a waist band- oh my goodness! So helpful! I put it on and do some exercises - then get on the elliptical- it makes a difference

    #gaittools thank you. Your videos have so much information that have given me understanding, "tools," visual guides and encouragement that I didn't have prior. Since I'm not receiving any therapy and the pandemic has hindered my ability to get moving more, this training from you is all the education I have and vital to getting me back into rehab eventually. Thank you Tara! My two questions rn before I can jump in are your recommendations for some good women's footwear that works with custom AFO's. I haven't used my brace in almost two years because I can't find any shoes I can put on myself ( am affected on left side). Help! My shoes keep coming off my foot and I'm dragging a lot. Canes without four feet prongs are of no use to me, as are walkers cause if I seize I end up just falling. Lastly, I'm confused when you say involved and uninvolved limb. Sorry, my cognitive issues make it hard to follow your instruction unless I can keep it straight or you have a good visual for a particular lesson. Otherwise, I hope you keep doing these amazing videos!! Your tone and patience and easy manner make them a pleasure to watch!

    Love the ankle skii. I made a hand splint by using my large makeup pallet and some large elastic bands, it keeps my hand open while I sleep. Happy Easter Tara. #gaittools

    Thank you so so much for another video . I keep waiting to get more from you . Very informative as I’m working a lot to improve my gait . I have drop food and wear brace to walk . I was never suggested to wear night brace . In of your videos you had mentioned to wear night brace and I notice my foot turned inside while sleeping. Do you think it would be worth to wear it now after about nine years of stroke .

      Thank you for watching and for sharing your story! Generally speaking, a night splint will keep the muscles lengthened on the back of the lower leg and/or prevent them for getting shorter. I hope that helps

    I particularly like the Walker attachment which I think will help me on the treadmill. My affected hand sometimes shoots forward switching it off unexpectedly.

    My problem with height is because of my AFO, that made my involved leg longer and that caused knee and hip pain on my uninvolved side. I went to a store that specializes in foot problems. I was able to get sandals with Velcro closures, extra wide width, and a removable in sole. That evened out the length and made it much easier to walk.

    I am a parcel quad and have C3-C7 fusion. So many things to work on. Knee won't extend on swing through for a heal strike. I have spasticity and don't know if it is the hamstring stopping it or quad straight. I love your videos and appreciate your assistance.

    Thank you so much for your informative videos. I had a stroke 3 months ago and am ambulating without an assistive device now. I walk with my arms extended and use arm swing. I am very fearful of falling because I don’t think my affected arm will extend and my hand hit the ground to protect my face. Does the protective extension reflex come back on the affected arm?     I had a hemorrhagic stroke 3 months ago. I spent 8 days in ICU then 3 weeks in an inpatient rehab hospital. I am currently going to outpatient rehab twice a week and do my exercises diligently at home. I was diagnosed with foot drop and my foot rolls to the inside. Can you have both foot drop and spasticity? I have normal sensation in my foot and ankle but my foot is paralyzed. I am slowly beginning to move toes just a little except for big toe which stays angled upward. My foot is becoming more contracted. Your video gave me insight to more exercises and tips.
