CPT® coding for Skin Lesions

  • This presentation discusses coding guidelines for excision and destruction of benign and malignant skin lesions, CPT® codes 11400-11471, 11600-11646, 17000-17286. The coding guidelines for each set of codes are compared and contrasted. Different scenarios are presented with coding guidelines and examples for each.

    Benign lesions include neoplasms, cicatricial lesions, inflammatory lesions, congenital lesions, and cystic lesions. These lesions are treated either by excision or destruction.
    Also discussed are benign premalignant lesions, benign vascular proliferative lesions and granulation tissue.
    Malignant lesions include basal cell carcinoma, squamous cell carcinoma and melanoma. These may be treated using excision or destruction. Calculating margins for excision of these lesions is discussed.

    Hi Terry, can I use mod-59 if I have 2 excisions of malignant lesions, 1 located face, and 1 with ears? They are grouped to 11640-11646. lets say I have 11640, 11641-59?
    and can I use mod-51 if I have 2 excisions of malignant lesions, different sites 1 trunk, 1 face, like 11640, 11600-51? (if I'm doing provider coding)

      Its interesting that this is not spelled out clearly in the guidelines. I did find some limited information in CPT Assistant that recommended a modifier 59. The diagnosis code will indicate the different sites.

Althought I know that CPC exam would be multiple choice and we won't have access to tools, I'm just really trying to know everything there is, and your videos really are very very helpful. I wish I knew about you before I enrolled in my current medical coding training school.
