That's a good question. Fusions are really hard. Do you have an AMA PCS book? Look to see if there is an Appendix M with coding exercises and answers. If not, see if there are other CZcams videos or online questions.
What is the code for one nodule with four biopsy taken using FNA? Please guide

It depends on whether or not all the biopsies are performed using the same modality (without guidance, with ultrasound, with fluoroscopy, with CT, with MRI). If all use the same modality, list one code and then the add-on code x3. For example, 4 biopsies without guidance --10021, +10004x3. If two are without guidance and two are with ultrasound guidance--10021, +10004 for without guidance and 10005, +10006 with ultrasound guidance.

Where can I find the guideline for video 10:24 where you mention difference between abnormal findings on test results inpt- dx only if test result are documented by provider as clinical significance. Outpt- dx may be assigned base on radiology or pathology report? What pages?

Hello for the bypass section for table 021 can you explain the correction note?
See Guidelines for Medical and Surgical Section, B3 Root operations, Bypass procedures, B3.6b. The body part is number of coronary arteries is bypassed to, qualifier is vessel bypassed from. This is the opposite of the guidelines for other bypass procedures. There is an example following the guidelines.

What type medical conditions would you include for substance use codes for?
for a medical condition due to substance use, abuse or dependence, report a code for the condition and a code from chapter 5. The guidelines give the example of alcoholic pancreatitis due to alcohol dependence, use K85.2, alcohol induced acute pancreatitis, and F10.20, alcohol dependence. See guideline for chapter 5.b.4).

Can you explain CABG in PCS
That's a great idea for a video. Basically, body part value is number of arteries bypassed to; qualifier value is body part bypassed from. This is different that other bypass codes and I apologize for being confused about this one. I am working on a video about this. Use different codes if different devices and/or qualifier are used on different vessels. So if a different character for device or qualifier value, use a separate code and the number of arteries with that specific device or qualifier.

I feel like a clown for having not understood this for so long, and then here you come and make it crystal clear in like ten minutes.

Everything I can find, iced-10, cms, bibs and ACOG is stating to code for the reason for admission but I am being told that the delivery note needs to stand alone and that we can only code from the delivery note. Is this true? Do you have any supporting documentation? OB is confusing for a lot of people but OB delivery codes are not like other surgery codes because they encompass prenatal care and post-partum care. Any help would be greatful
When a delivery occurs the condition that prompted the admission is the principal diagnosis. See guideline for chapter 15 b 4).
And being billed monthly for this is ridiculous!

percutaneous vs natural or artificial opening endoscopic.
Percutaneous requires an incision through the skin. Natural are artifiical opening endoscopic means the instrument is inserted through an opening already there.
