- On 18 April 2002, at 17:48 (local time), a Rockwell Commander 112 crashed into the upper floors of the Pirelli Tower in Milan, Italy, for reasons still unclear. The crash killed the pilot and two others in the building. Sixty more people sustained injuries in the building and on the ground.
The crash raised fears of a terrorist attack because of the similarities with the September 11 attacks, which had occurred seven months earlier. A subsequent investigation ruled out terrorism as a motive.
The airplane, piloted by 65-year-old Luigi Fasulo, took off from Locarno Airport in Magadino, Switzerland, eighteen minutes before the crash. Later as the airplane flew over Milan, Italy, the pilot radioed to the control tower at Linate Airport that there was a problem with the retractable landing gear. The tower began arranging an emergency landing. The vibration of the impact caused shop windows to break and littered the surroundings with debris and glass in offices. The pilot and two others present inside the building died.
Between thirty and forty people were taken to the hospital with moderate injuries, while fire-fighters contained the fire that resulted from the crash. Immediately after the crash, the nearby Milan central railway station, metro station and the Linate airport were closed.

The similar incident was Flight 401 in the Florida Everglades by an L-1011 in the early 1970s. The plane was holding while they tried to troubleshoot a burnt out landing gear indicator light. The Captain bumped the autopilot by accident and the plane slowly lost altitude before they noticed in time to save the plane. That's all I can remember without looking it up.
I remember that ABC made a movie about it starring Edward Albert as the pilot and Adrienne Barbeau as a stewardess.
We were camping along the beach when that happened. Being from IL and a kid, I really didn't know much about what an everglade was. I assumed it was where they filmed "flipper". Lots of helicopters flying around. They updated fron time to time on the radio in the search for survivors as the day went on.
It was 1972 and the Eastern L-1011 was brand spanking new. Parts salvaged from that aircraft and reused on other Eastern aircraft gave rise to a series of alleged ghost sightings on board those aircraft which were given spare parts from flight 401. The ghost stories became so numerous Eastern eventually removed those salvaged parts from the affected aircraft.

Well, there's Eastern Airlines Flight 401, obviously, where the crew CFIT into the Everglades as they were trying to troubleshoot a landing gear issue at night. But I seem to remember two similar accidents - United Airlines Flight 173, where the landing gear light bulb hadn't burnt out, but instead a sensor had been damaged giving an unsafe gear warning, causing the crew to troubleshoot so long they ran out of fuel. And Scandinavian Airlines Flight Flight 933, where the crew was distracted with an unsafe gear indication that they flew straight into Santa Monica Bay.
You need to voice these yourself if you want to expand the channel.. that personal touch really helps
..or if not possible, get someone to voice them for you.

Sometime around 2010 a pilot in a small engine aircraft crashed into a 4 story office building in Austin, TX, and it WAS intentional. The man was having problems with the IRS for tax issues and he filled up several barrels with jet fuel, loaded them onto his aircraft, and took off from nearby Georgetown. The office building he crashed into was the regional IRS office.
Heard that the pilot managed to crash exactly into his ex office !
He was sacked 3 weeks before.

Answer to the question is United Flight 173 in Oregon. It led to CRM being implemented.
Similar was the flight to callay? or rizzo? in Brazil, based according to my poor memory of the facts. Something about a faulted beacon and improper input by pilot; they crashed in a mountain because they were distracted and confused of location.
I am wondering - with today's cell phones and the ease with which new apps can be programmed, couldn't they serve as basic black boxes? Certainly this would be straight foreward for a voice (noise, sound) recorder. The position of the aircraft would also be easily recorded as GPS is included in all modern cell phones. Attitude could be recorded by including a small gyro in the cell phone. Something related already exists in the form of drop sensors. Equipping a small aircraft with rudimentary black boxes might be as simple as plugging in an appropriately programmed cell phone into a crash-protected base station inside the aircraft before the flight
Cell phones have and can add numerous sensors, but just having coordinate data wouldn't be all that useful. You need the aircraft systems data to have any useful analysis. Once you have created an interface for the data, the recorder itself is not the real cost challenge.

It's scary that pretty much anyone under any circumstance can fly airplanes over our houses and buildings. There doesn't seem to be any rules or regulations at all.
LOL, you clearly haven’t a f***ing clue as to what you’re talking about. Aviation has some of the most stringent amount of hoops and regulations of any industry.
You can't fly anywhere anytime, but how do you enforce these limits? Anti-aircraft missiles everywhere? Fighter planes shooting down rogue planes over cities?
