Grades and level of Baldness ( Norwood Scale )

  • The Norwood classification or Norwood scale is the most popular classification of grades of baldness worldwide. It was first put down on paper by Dr. O’tar Norwood in 1975.
    In brief they are:
    Grade 1/Type1: There is no hair loss. This is the adolescent hairline or the juvenile hairline which at times persists in adults. The hairlines sits on the uppermost brow crease. The frontotemporal expanse shows minimal hair loss which looks natural and pleasing because it is appropriate for age. I would say this is a normal hairline. Many would say genetic baldness begins at Grade 2.
    Grade 2/Type 2: The hairline along the tuft is normally situated but s one fingertip breadth behind the upper brow crease of Type 1. The recession is appropriate for age and defines the masculinity of an individual. The mid coronal plane is the reference point. If you draw a line in the coronal plane 2 cms in front of the external auditory meatus (ear canal), the fronto-temporal recession should not touch it or go beyond. If it does, it enters Grade 3 territory. No hair transplant is advised if this is minimal but if the recession gets deeper (hairlines staying normal), hair transplant may be needed in selected patients.
    In Grade 2 a/Type 2a: when the whole hairline recedes uniformly along forehead till mid-coronal point. (we shall discuss this in the individual video for grade 2)
    Grade 3/Type 3: The recession happens beyond the coronal plane 2 cms ahead of the ear canal. This is the first clear cut grade where hair transplant is always recommended when the patient is keen.
    Grade 3 (Vertex)/ Type 3 (Vertex) indicates baldness primarily of the crown area with or without features of recession along the hairline which can be of type 1-3 (but not exceeding features of type 3 ) on Norwood scale.
    Type 3 a is like type 2 a but touching the MCL.
    Grade 4/ Type 4: This denotes further balding and a bald crown larger than 3 inches in diameter. There is an intervening swathe of good quality hair between the fronto-temporal recession and the crown balding. If this region has thin hair or good quality hair but width less than or equal to 3 inches, it becomes grade 5 baldness.
    Type 5 a denotes effacement of the frontal peninsula of hair till beyond the MCL
    Grades 5 / Type 5: The hair loss has now reached an advanced stage around the temples and on the crown but there is a bridge (less than 3 inches) of hair thinning or otherwise separating the 2 zones areas of balding. It appears that this bridge is breaking up and effacing.
    Grades 6 / Type 6: This is slick baldness. Horseshoe pattern of baldness. Hair remains but it is like a wreath around the back of the head. No bridge of hair separates the 2 bald areas of balding. Both zones have coalesced. The crown has expanded to the edge of the horizontal and vertical confluence of the shape of the skull.
    Grade 7/ Type 7: Men with such advanced hair loss can be candidates for a hair transplant, but with such a restricted donor area the transplant may be limited to restoring the hairline while the crown remains bald.
    There are 2 other patterns:
    1. DPA- diffuse patterned alopecia
    2. 2. DUPA- diffuse unpatterned alopecia
    The lesser talked about grades of balding.
    This is a challenge and especially during online consulation it can be actually missed out due to proper lighting issues. If missed in the young it can be disasterous if a hair transplant is done.
    We shall talk about them later in an individual video but the essential difference is that DPA patients make good candidates for a hair transplant , whereas DUPA patients do not.

    Very well explained sir. I'm Norwood 2 (age 23). I've lost a lot of hair around my temple region and your description about 2 explains it perfectly that my hairline has receded to a certain extent and not become like 3.
    I've a widow's peak which has become somewhat separated from the actual hairline do the hair grows there separately and doesn't mix with the frontal hair.

      The widow's peak at times is very obstinate to effects of DHT, but eventually it gets effaced with time.

      @@hairtransplantinindia Sir, I understood about the obstinate part of widow's peak but can you explain effaced in a simpler way?
      When I was 15-16, my widow's peak part was the most curly part in my hair region. I think so due to that, since last 3-4 years, my frontal hair has also become curly as the widow's peak started getting mixed with the hairline.

This April, I'd shaved off my head thinking there might be an improvement in my hair once it grows back, but I've noticed that the right side of my hair, the side hair, it looks it has less volume and I guess it's becoming thinner as it's showing my scalp there while the left side seems somewhat okay not yet affected by anything. But overall, I've noticed that my hair doesn't seem thick anymore and scalp is visible most of the times (except back side).

    Sir i have thinning over my top evenly and the temples have receded at somewhat grade 2 type..approx 1 1.5 cm face side of ear canal vertical ..i am confused as to what my stage is..because though theories suggest that due to recession it is grade 2 but the thinning has been severe as when i cut short it is apparent

    So from his description I feel that I am in between 5 and 6 And I do have a pretty large donor area on the back of my head So I'm wondering how many graphs that would be approximately. And truthfully if I could just get the back of my hair replaced with a conservative widow's peak in the front I would be okay with that. I'm not looking to have a fully restored hairline just maybe something like Bruce Willis which is what I used to have a few years back.

      I have done a procedure on a 88 year old male! Age is not a barrier. If you are in good health and donor hair are available any age is good for a procedure.

    i believe this video does not cover the hair loss topic completely , because in my early 20s my hair started to grow v shape on my forehead and started growing on my sides , i believe thats just the natural hairline change

    Hello sir , I am nw2 at 22 , kind of like a more nw2 on right side and more like a nw1 on left side . You said mw3 is ideal stage for HT , can u predict when I will become nw3 assuming that I started balding ( had slight temporal recession ) at 19

Have u seen cases where people retain nw2 hairline for long ? I am saying this bcoz my brother who is 23 has not receded at all , he is like a nw1 . My father also enjoyed good head of hair till his late 30s so I just don’t know how I got this baldness gene

      the length of the nose. But there are a lot of variables involved (grafts available, facial contour, age and caliber of hairs) involved. You will need to consult. Usually said 4 finger breadths but its not that simple either.

    Why does no one acknowledge or explain the A variant much? I had that type, which I heard is a rare type and starts young, I was 16 when I got type A and by 19 I was a completed 5A when I got 1st hair ttransplant

      These are baldness grades. Unless you get bald further, you remain at Grade 2. But surely thinning does predict your eventual point of final destination without medical treatment.

    Have you ever seen someone who held a Norwood 2 hairline or Norwood 3 hairline since 20 yrs old and kept that there whole life?
    I'm Norwood 2 and I'm 27 now, still full hair with zero thinning, it went back when I was 20 and hasn't changed. Narrow v shape hairline like the director Christopher nolan

    Hi doc,
    Why can't we able to produce a medicine which has no sexual sides.yes DHT is the culprit but is there any other backdoor pathway where we can approach it from. Finasteride is scary to me. It feels like a zero sum game.
    This baldness shit is taking too long to get fixed for generations. This has to come to an end. So that we can move on to solve something else.
