What Causes B12 Side Effects?

  • Are you having B12 side effects, like heart palpitations or digestive issue after taking B12 or B vitamins? In this video, we look at these side effects and what typically causes them. We also look at what some of the common side effects are and how common they occur. B12 Side Effects
B12 side effects are not very common. However, they do occur in a segment of the population. Most of the time these "side effects" are not a true B12 side effect. Think about it. B12 is an essential part of our natural biochemistry. To have a side effect to that is contradictory to how your biochemistry naturally works. That doesn't mean you can't be sensitive to something in the B12 or B complex that you took or even B12 itself. Because B12 is an essential part of how your biochemistry works, it's not very common that it is actually B12 causing the side effects or reactions. More common it is something that's in with the B12 that you took. Before we get into the common reasons for these side effects or triggers from B12, let's look at the. Let's look at what some of these side effect reactions etc that people have from taking B12.
Causes of Digestive B12 Side Effects
Nausea, bloating, upset stomach, and various digestive symptoms are one of the more commonly reported side effects from taking B12. In these cases, they are often taking a B complex that they swallow. This contains B12 but also has the other B vitamins other than B12. All B vitamins feed bacteria. If you get digestive symptoms after taking B12, often there is a bacterial overgrowth or possibly fungal overgrowth creating the digestive symptoms. Anytime patients tell me they have a reaction from taking B vitamins, like nausea or some kind of digestive symptom, I automatically think that they might have some microbial issue. It's not always the case but it does come to mind. If that happens to be your issue, taking your B vitamins or B12 in the form of injection, might resolve the issue. We wouldn't expect the injection to do this because it bypasses the overgrowth.
Causes of Anxiety B12 Side Effects
Another common side effect or issue reported from taking B12 or B vitamins is anxiety, heart palpitations and difficulty sleeping. B12 definitely can do this but it is more common for other B vitamins to do this. If you get a B12 injection and it stings, that means it's not just B12. B12 does not sting but the other B vitamins do. You want to differentiate which one of them is causing the issue. You could just get B12 alone and see how you react to that. If you are getting these palpitations from just B12, then you know it from the B12 only.
If you know for sure that you only take B12, swallowing or injection, and you get anxiety or heart palpitations, consider taking a much smaller dose. Take the smallest amount that you think would not create a reaction. Start with that and gradually work up to higher doses. Once you find the dose that is not creating these problems, over time your body may adjust to it and you won't have such a strong reaction. Sometimes palpitations and anxiety occur when there is a big deficient. As result, your body will get very stimulated from the B12. Sometimes the same bacterial issue that causes digestive issues from swallowing the B12 can also cause these heart palpitations, anxiety etc. The same issue wouldn't be expected though when you take an injection. If you have both the digestive issues and the anxiety and palpitations when you swallow it, we wouldn't expect the same thing when you take an injection.
Other Cause of B12 Side Effects
Another thing to consider is what's in the B vitamins too. Sometimes people think they are just taking a plain B 12 or B complex and there are other things in there, like probiotics or prebiotics and other herbs. These can cause reactions too. Make sure you check all the ingredients to ensure you are not sensitive to one of those things. Also when you take B12 as a sublingual tablet or chewable form, it will usually contain sugar alcohols like xylitol etc. That gives it sweet taste but those can can definitely cause digestive issue for some people.
If switching the route of administration and dosage don't help, you may also have like a substrate issue. B12 is used to recycle homocysteine. Homocysteine comes from amino acid cysteine and methionine. If your levels of cysteine and methionine are low, the B12 has nothing to do once you intake it. That's called methyl trapping. You can check your fasting homocysteine to see if your homocysteine levels are low. This may indicate a bigger problem going on (inflammation) or just simply a protein deficiency. However people with low homocysteine, mostly don't have these negative reactions to taking B12 as an injection or oral. Still it could be part of the problem in your situation.

Hello Doctor. This is exactly what I am suffering from. Namely, anxiety, palpitation and sleep disturbance. As I noticed, this happens when taking drugs containing methyl forms of vitamins b. Complexes with cyanocobalamin (up to 200 mcg), which I take, gave a short-term reaction within an hour, which eventually passed. While the reaction from methyl complexes lasted about two days. (My homocysteine is around 10 units.) And I found an explanation for this in Amy Yasko tolerance panel. According to this, as a carrier of the homozygous val158met mutation, I should focus on other forms, mainly hydroxo. How common is this point of view? On the net, I saw information that hydroxocobalamin also causes palpitations, and it is less common. And what form of vitamin B do the tests reveal? Also what is the antidote if caught a heartbeat from vitamin b? I think I saw you somewhere that it is glycine?

Hi- I have been getting a lot of acne and acne scarring , had been taking hydroxocobalamin shots and then now on liquid methyl (oral). Tried methyl shots too before.Acne persists in all cases and lots of clogging of pores, getting acne on body too. Weird thing is the only thing that has helped is taking psyllium husk fiber (1 - 1.5 tblspoons per day in water). I read somewhere b12 changes the skin bacteria. Know anything else to help?

Hello Doctor!, thanks for the very informative video. It's been almost two months that I started feeling burning sensation in my feet. Sometimes I feel it in both feet. Sometimes in left or right feet. I read somewhere that low B12 levels can cause peripheral neuropathy and permanent damage to the small tiny nerves in feet. So I got my blood work done to see the B1, B6 and B12 levels. The report came out as normal and the reading are within the range. Even though the B12 reading is normal at 280 pg/mL, I feel the levels should be greater than 400 pg/mL. for optimal nerve function. So I am planning to go in for a B12 injection to boost the level. Can you please suggest what is the best dosage to start with to ensure no sever side effects before going in for higher dose? I appreciate your kind help!

No bc everyone is different and most don't have side effects from B12. If you are worried you can start with 1/2 or 1/4 of standard dose which is 1000mcg.

Started to develop a vague burning of my skin ( mostly Nose) then progressed to my legs and pretty much entire body. My B12 lab work was >2000. My PCP denies that the supplement could have caused this. Everything else was pretty normal but bilirubin was 1.3. 3mos after discontinuing the supplement my symptoms rage on with normal Blood work. Possible PN from this?

I am hypothyroid and react to pure sublingual bioactive B12 (mix of methylcobalim and adenosylcobalim) by having palpitations. My B12 serum levels were low but not below range (298 on scale 200 to 900) so I am unsure why I have this reaction except perhaps it is overstimulating me as I have never tried this type of B12 before (I usually go for the cheaper supplements and never had reactions to them although I also find iodine and Vitamin E very stimulating and will be awake all night if take them, sometimes for days afterwards after just one dose).
I have tried reducing the dose and doing it only every few days but I still react even to a small drop of this stuff. Any idea what may be going on?

I dunno if I Have MTHFR, But every 2 weeks I just take 500MCG of B12 I always got an injection a few times a year, always got a kick of energy from it, I think it was my IBS causing a lack of absorption however is it safe to take that every 2 weeks and question on l-methylfolate how much can i take of that as i dot eat many dark greens for folate, its it safe to take l-methylfolate even if you don't have MTHFR? say 1000MG with that B12 once a fornight? ir can l-methylfolate be toxic I know B12 isn't toxic but I read it was best to take B12 with Folate?

How about tachycardia and elevated bp? I took methylcolabamin b12 sublingual tablets 1000 mcg and by day 3 of this I had what seemed possibly a reaction from the B12. The tachycardia though didn’t start until 4 hours after taking the b12. And the tachycardia episode itself lasted for 4 hours! We’re talking tachy, 130-160 resting bmp for a long time, along with extreme restlessness and physical discomfort/tension.
Thoughts? Could this have been an imbalance being highlighted?

I had the same issue, but I took Cyna-Colabamin at 1000mcg, day 3 like you, I ended up in Hospital with heart from 120 to 130, crazy blood pressure changes, my heart rate lasted for like 4 hours also. With in a day of taking it, I was at the doctor with pals, then hospital visit second day, then ER on the third. I still had intermittent issues of racing heart after I stopped taking it. I decided to take a B12 tablet, 2 week later to test it, and I felt anxious with hours, had lay in bed, then at night, I couldn't sleep as my heart would start racing as soon as I tried to doze off. I told the doctors this, they didn't really believe it caused it.

I had the same issue. My Dr recommended 2000 mcg of b12 because my level was 230. One morning I took the recommended dosage ( it was sublingual methylcobalamin) instantly my heart rate shot up to 170 and I had to go the bathroom but also broke out in a cold sweat. I went to the ER and they took me directly to the back and ran a bunch of tests. My blood pressure was also high 140. I did not have high blood pressure previously.
The ER released me and asked I follow up with cardiologist.
I completed different tests and they said my heart is fine. However now I have heart palpitations and feel breathless. My B12 is lower now, but I’m too afraid to take anything other than food. I even tried nutritional yeast and I get a racing heart, not as extreme but enough to scare you.
