  • In this article we will discuss the causes of high B-12 without supplementation. In a previous video we talked about What Does A High B-12 Level Mean and some things to think about with that. In this article we will explore this topic in a little bit more detail.
The basis of this article is to discuss some of the more serious or problematic things that could be causing your high vitamin B-12 and the information is based a research paper. If you are having persistent elevated B-12 levels, this information will b helpful. So keep watching we're gonna get into the detail.
MY NEW B12 BOOK: geni.us/rTQTvd (Amazon)
The causes of high B-12 levels without supplementation are numerous. Before we get into this, we have to take first things first. Even though the title is "cause of high B-12 without supplementation," my experience helping people figure this out, they are taking B-12 but they are not aware of it. It maybe in a multivitamin, a workout drink, a protein powder, or something like this. So double-check you are not getting B-12 from various sources that you're not aware.
Now in a previous video we talked about how liver disease and cell membrane issues can be implicated in elevated B-12 levels. This still can be the cause of high B-12 without supplementation but there could be other, more serious, reasons for this as well. Most of these more serious causes are altering the transportation of B-12 via the transcobalamin transport proteins (more on this below). Both the cell membrane and transcobalmin proteins can be a part of the issue and solution. As you will see below look at some of the problems that can come up with the trans cobalamin the various transcript a'lamin transport proteins and various diseases that can cause elevated B-12 when you're not supplementing with B-12
most often when there is elevated B-12 levels whether or not you're taking be extra B-12 it has nothing to do with alterations in these transport proteins called transcobalmin. This is why most often when you have an elevated B-12 level that shows up on your laps it's ignored, pushed to the side, or explained away. Essentially if you remain having persistently elevated B-12 levels in the absence of supplementation you should explore this further. High B-12 levels could be an epiphenomenon or related to a deeper issue with B-12 transportation.
Role of Transcobalmin in High B-12 Level Without Supplementation
So how is B-12 transported? The chemical name for B-12 is cobalmin and it has to be bound to a carrier protein to move through the blood and more importantly into the cell. B-12 uses three different carrier proteins to do this TCN 1, TCN 2, and TCN 3. However only one of these transporters is actually functionally useful in delivering B-12 into its target cells. It is the TCN 2 transporter that is the main transporter for B-12. Therefore, anything that raises TCN 1 and TCN 3 will lower the functional activity of B-12 by limiting the binding of B-12 to the TCM 2. In this case all the B-12 is getting bound up by these other transcobalmins that don't really work well for getting B12 into the cell. Now this doesn't necessarily explain the elevated B-12 levels but it does explain how one can be deficient in B-12 in the presence of higher serum B-12 levels. So what is actually causing the high B-12 levels? Let's look at that next.
In the case that you've ruled out excess consumption of B-12 makes sense to explore some of these other things. This does not necessarily mean you have these things but problems that could be going on to cause elevated B-12. First thing to consider is solid tumors can cause elevated B-12 levels. Most commonly it would be liver tumors, hepatocellular carcinoma in particular. This can lead to elevated B-12 levels due to poor uptake of B-12 in the liver cells. With liver cancer some of the liver cells are damaged as a result the liver cell are are not expressing as many B-12 receptors. The same thing happens with liver disease whether it is acute or chronic hepatitis.
Hepatitis and Hepatocellular Carcinoma can also lead to increase release of B-12 from the cells being damaged. B-12 can be released from the cells because basically it is where most of the B-12 is stored. B12 is stored in the body. So solid tumors are a cause of high B12 and there are probably other cases of solid tumors outside of Hepatocellular carcinoma but this was the focus of the research paper.Blood cancers in general can lead to an increase in B-12 levels due to an increased production of these TCN 1 and TCN 3 trans cobalamin proteins. These particular transport proteins are made more commonly in certain cell lines like the granulocytes in the white blood cells. High B12 levels are found in many myloproliferative disease. These are disease of the bone marrow where the white blood cells come from. These disease states result in an increased production of white blood cells including Granulocytes.

I have Hashimotos and autoimmune gastritis. My b12 levels have ranged from 1500-over 2000 since 2017. I am on a pretty restrictive diet trying to eliminate all triggers of my autoimmune but I have not felt well, pale skin, cold, fatigue, dry hair, itching, pain in arms and legs at night . My doctors really don’t seem to put too much concern with my b12 levels but I certainly know it’s contributing to my not feeling well. My hematologist who called me and never met me told me to stop taking b12 supplements which I told him I didn’t and haven’t, he just dismissed me. Could my high b12 levels actually be a deficiency?

Hi Shar, there are multiple things that can cause high B12. It is possible that you have low intracellular B12 despite high serum number. There are test you can do to determine this. I have a book coming out in the fall (hopefully) that goes into some detail on this. If you hematologist is not concerned, I would not be either. It is hard to say though since they were "dismissive" . Hope that helps.

Could it be that the high B12 (1500) could come from a several repetitive infections, colds, otitis for 2.5 years old kid. @swintegrative  Před 4 lety

Hi Florian, Children have different reference ranges for many things than adults, so I am not sure on that level. Because of the immune involvement in infections, multiple infections could theoretically cause elevated b12. It would have to be going on for a while. Ongoing swollen lymph nodes should be followed by a doctor.

I've had three b12 tests over a period of 5 months and they all came back at >1500 ng/L, normal range is 150-750 (same as pg/ml, I believe). I also recently did a holotranscobalamin test, which came back as 150, normal lab values are 37-188. What could be the cause of High total b12 serum, with a normal/high holoTC result? My other blood tests, including liver panel are normal, except ferritin, which is low. I am not supplementing, and even tried eliminating all b12 from my diet for 2 months with no effect. • @swintegrative
      @swintegrative  Před 4 lety +1

Hi Edward, I think a better way to ask this question is when you have high B12, do you have symptoms. If you have symptoms, the second question is, do those symptoms correspond to a problem that commonly causes high B12. For instance, one cause of high B12 is making too much binding protein (TCN-3) this can lead to low intracellular b12. Low intracellular B12 can cause all the same things as low B12 in general. High b12 and your symptoms are associated but the high B12 is not the cause of the symptoms. Hopefully that makes sense.

Hi Fahim Anwar, the B12 does not cause high liver enzymes but high liver enzymes could be the cause of high B12. I doubt yours are high enough to cause this but it could (optimal is less than 25 so even at 36 it is a little high). It's hard to say if they are connected without having the full picture. You probably need to keep testing and possibly get a deeper look at the liver.

That does seem odd. make sure it is not in anything else you are taking. If you are worried about it being high then retest because the b12 is stored in body specifically in liver. Even though you stopped it, the b12 could be released from storage in the liver (theory). I would retest before getting too concerned.

Nurse for nearly 40 years long, long complicated history congenital heart disease JRA which appear to go into remission with my first pregnancy but appear to come back but was diagnosed with fibromyalgia. Now with type 2 diabetes, hashimoto's, and autoimmunity of a questionable nature . My total compliment is quite high but c3 and c4 are normal . In the last year underwent full genetic testing which found the source of my endocardial cushion defect and of my long-term open severe allergies both to foods, medications, chemicals and environmental sources. I suffered a heart attack in 2011 not diabetic related and cardiac or diabetes related and catheterization showed totally clear coronary arteries except for the LED which is where the clot was. Initially thought to be from my MVR mechanical mitral, which, on catheterization was clean and functioning as the day was put in over 10 years prior. (I am now 22 years on my original St Jude, we joke because I hold the record at Penn medicine for longest time on the original valve without a redo LOL) it was determined that because we were trying to switch from Coumadin to one of the newer drugs I threw a clock from my horrible varicose veins in my right leg. BUT immediately after the heart attack I began having worsening of my muscle function in my proximal large muscles particularly upper arms thighs and hip/girdle. A roller skater for years since childhood, my legs would now no longer hold me up for certain maneuvers and the cramping and pain was horrendous. Had a full genetic muscular workout and essentially they did not find anything genetic. Several years ago maybe five or so my B12 was low and my primary was thinking my muscle issues were from low B12 because in addition I had some neurologic things going on. We tried po supplementation with very little results. She wanted me to start taking shots and I said that there's something wrong with this and I really felt like they were trying to put a Band-Aid over this while what was really causing it would be raging on/getting worse. I realized I had switched to mainly chicken and fish because doctor says got to reduce your amount of red meat well that's precisely when my B12 started to go low. So I started eating organic meat from the Amish market boom couple months B12 is fine period stays fine for quite a while then all of a sudden when the compliment went out of whack I had to go on steroids for a while and wham bam the B12 starts going up. Now I have been off all steroids for about 6 months now but the B12 is climbing I just had one drawn Wednesday and it was over 1400 I will edit when I can go to quest and get the result now we understand that autoimmunity normally goes along with low complement, high compliment? And also a sky high B12. I have for quite some time had the nephronic syndrome, you know microalbuminuria and such but it's been pretty under control seems to be a little linear with how well I control my sugar. Right now I am totally normal at 0.2 random urine microaluminum. See a neurologist regularly suspected this is lupus however I have things that overlap with rheumatoid and I get skin rashes so now she is saying which I agreed with that this sounds like mixed connective tissue disease sometimes my lupus will be elevated other times not sometimes the rheumatoid will be elevated and other times not. I see my primary on Monday and I'm not sure which tree to ask her to bark up next period And I don't know what is the best way to approach this high B12 level. I did read however that metabolic syndrome which is where all this mess began before the diabetes may cause the high B12 and right now my cholesterol triglycerides are high but I cannot take statins with these muscle issues and trust me I've been on three different ones and could not get out of bed it made my muscles so bad. Oh latest A1C 5.7 3 months ago 5.8 before that I was a little up at about 6.6 but I knew what that was about and fixed it. I'm trying to wrap my brain around the bigger picture here like how does this high B12 relate to everything else. I also have many other physical issues, trigeminal neuralgia on the left, familial hemiplegic migraine, asthma just to name a few.

My 18 month old has high B12 and she had teardrop cells. With other symptoms like reoccurring fevers for 15 days now. Reduce energy. Reduce balance. She doesn't eat much. And takes no supplements. Had a normal WBC and RBC and normal platelets. My gut tell me something is wrong. That's why I asked the doctor for blood work. It's currently Friday so I can't talk to PCP till Monday. She was also a premature baby at 30 weeks. And has BPD so she is on an inhaler. She struggles to gain weight due to not eating as well. Has history of UTI that lead to blood infection requiring IV antibiotics. Everything I'm researching is worrying me. I hope my doctor looks into it more. If not I'm gonna call a pediatric hematologist. Any input would be great TIA.

I do take a multivitamin every day that contains 6mcg. I don’t have any symptoms and wouldn’t have known if it wasn’t for the blood work. I got diagnosed with Crohn's recently and my GI thinks I have primary sclerosis cholangitis I have to get tested for that. With some things that I've been told, it doesn't make sense to me. The high ALP if I understand correctly is coming from the bone? My vitamin D3 was lower, 20.8, but my b12 is high which I've been reading liver is one of the big concerns. Im just getting overwhelmed, for the third time already I've had to switch doctors cus they leave to another location. Hi APM, This is a good question and I can see why you might be confused. Alkaline phosphatase is an enzyme found in many tissues including liver, bone, intestines, etc. When it is high and other liver enzymes (like GGT) are high, it suggests the problem is the liver. The high B12 is probably coming from lack of liver absorption and storage and / or release of the B12 from the liver's storage. The absolute answer on cause would really depend on how long it has been like this and some other lab tests. For now it seems like it is best to stick with the tests the GI doctors is recommending. Hope that helps, good luck!
