  • Today we are going to talk about some medications that make your beta cells release insulin, the ones that we call Sulfonylureas. The most commonly used Sulfonylureas are Glipizide (Glucotrol and Glucotrol XL), Glyburide (Diabeta, Micronase, and Glynase PresTab), and Glimepiride (Amaryl).

    Thank you so much Dr. Carvalho! I appreciate your time and work in making these videos. I'm on glyburide. Recently got to lower my dose. Am working to get off of it all together. I had wondered what it was doing to my beta cells. I love your way of teaching. I'm nodding along the whole video. :)

      You are very welcome. And thank you for all the encouraging comments as well. I'm glad you said you're working to get off them and didn't just decide to stop them right away, because any medication is far better than high blood sugar. But if you can slowly get your blood glucose down with other things like diet, exercise, metformin...than you can consider stopping this one (but make sure you ask your doctor first, because it's illegal to give medical advice through CZcams🤭).

      Thanks a lot. I lost my dad and my grandmother last year. It wasn't an easy time. but we are doing well now. I'm back to making videos 🙏

      Thanks. And I'm sorry for your loss as well. May God comfort you. These few years have not been the best for many people. My father and my grandmother both had diabetes as well, and when I received the bad news it really got me down because I thought I had a lot of time to take good care of them. But I'm back and hopefully I will help others with these videos.

    so how could you know if you have a glucose metabolism problem and theres not enough atp to close those channels?

      That's a very good question. There's no noninvasive test that you can do to let you know for sure (even though we know from research that some people have it). But You can guess pretty well. For example, if you are taking a very low dose of Sulfonylureas and it's working consistently without the need to increase the dose, it shows that your beta cells are not too bad. But on the other hand, if you are taking a low dose and after a few months you find out that your blood sugar is out of control again and you need to keep increasing the dose, then you are probably destroying them faster with these meds. There are many other things that can be considered, so I suggest that you talk to your doctor about the state of your Beta cells. Thanks

    Sir, I’m taking Glycomet trio 1mg, has glimpride and sitared D 50 mg i recently tested cpeptide :-3.9 and fasting insulin 8.11 is it because i am on medication the bcells are working and if i stopped taking medication the bcell will not perform am i right?

    Thank you so much for these videos I understand more of my type 2 from your explanations than when I speak with my doctors who seem to know nothing really. Your videos are so simple , scientific and informative. Please could you do tell me or make video on the pros /cons of taking metformin as I am currently placed on 1500mg daily.

      Thanks for the positive comments. I don't think your doctor lacks knowledge, it's not easy to have enough time to explain everything to patients in the hospitals. And yes, I'll make a video on metformin one day, it's one of the better ones.

    I take Glipizide and Metformin 500 milligram but I've been on there since 2005 and they seem like it's not working anymore because my sugars are being high all the time

    I think some info about Metformin would be awesome. I've managed by BG through diet and I've been taken off Glipizide in the last year. Metformin is my last med... wondering if that is worth keeping, or go work my way off that one too. There seems to be some upsides to Metformin and I'm over the side effects. Getting off meds is generally a great idea, if you can manage the BG through other means, but Metformin seems pretty benign.

      Thanks for watching. I'll talk about metformin soon. And yes, metformin is one of the medications that actually help people with Type 2 Diabetes (it doesn't just reduce the blood sugar).

    Wonderful video as usual. Yes, I have taken this category for medication before and it works well. And for me it seemed to give me a false sense of glucose control. It allowed me to cheat with my food types and intake. I don't take it now because I started Ozempic. I still take Lantus but smaller doses. Diet is the key but honestly, it's not an easy thing to control. There seems to be so much more than discipline. It's almost like something is programmed into my thoughts about food. Thanks Dr.

      Thanks a lot. And yes, these meds can encourage patients to cheat🤭. I also agree with you on diet. I prefer diet and exercise to any medication. But I it's good that patients have the option to take medications as well, because as you mentioned, many patients find it difficult to change their diet. And taking medications is far better than not doing anything at all and just wait for complications.

      Can't seem to find the sweet spot that ties everything together. Food + exercise and rest = energy just doesn't work for diabetics. At least not for me. Always fatigued.

    Many thanks. I take one milligram of glimepiride. My problem is high a1c and the only Med that helps my a1c is tradjenta. Unfortunately, I cannot afford it since I don’t have insurance. So, I take glimperide once a day with metformin 500mg. Gimpiride @ a m, and metformine @ pm. I don’t have any side effects and will find out in 2 months how effective the combination works for me.

      That doesn't look like a bad combination if you can't afford the newer ones, because metformin can take care of the insulin resistance and counteract the weight gain from the Glimepiride. But don't forget that a good diet and exercise can help a lot. Thanks
