    Thanks a lot for your advice. I have gone for trifocal lens by ALCON (PanOptix), my surgeon preferred it. Finished the surgery for both my eyes this week.

    Please do a detailed video on the Real-World results your are achieving with the Synergy IOL. Synergy by far has the best defocus curve, but I don't see how you can get down to 33cm close vision without sacrificing Contrast Sensitivity, vision in dim light and/or increased dysphotopsias. I would like to know what results you are seeing with this lens in comparison to PanOptics TriFocal and Symfony Plus (if it is ever commercially available). Especially interested in seeing simulated dysphotopsias between those IOLs.
    Also I greatly appreciate how you show dysphotopsias as many Ophthalmologist are overpromising and not informing their patients of the negative effects.

    Hello Alex…just discovered your great Channel. Not many people seem to address my concern….I am having surgery in a few weeks and I have refractive Amblyopia (no strabismus) in my L. Eye. I have cataracts in both eyes causing a real problem for loss of contrast and blur especially in my ‘good’ eye. I spend much time needing my intermediate vision (as a musician and for computer and tasks at home). My surgeon assumed automatically that I want monofocal lenses. And will correct intermediate and near with glasses. He is concerned about using multifocal in my one worlking eye. I read about Tecnis Eyehance (hoping for intermediate vision for music especially) and also about the Vivity (EDOF). Not many ophthalmologists put videos up about amblyopia and the new lens technologies. Please can you offer an opinion. Other than amblyopia and cataracts, I have very healthy eyes with no underlying conditions. I am 72 yrs old….but feel like I am 30 :). Thanks so much if you can answer this question.

      Hi ! It is pure monofocal IOL, so you will need a glasses to read. The only advantage is you do not have a risk of wrong IOL calculation. To be honest with you, modern formulas and accurate diagnostics and good surgeon make this risk really small. It may have sense to pay extra for that technology if you have high hypermetroria or myopia, or refcactive surgery before, as these cases are difficult to calculate. For normal eyes I do not think it makes a lot of sense

      The true EDOF era started by AMO with Symfony, than Eyhabce and now number of companies manufacture that. I am sure you will have limited choice of lenses and manufacturers, so let me know your options and I’ll comment.

    Hi, can you please give me your opinion about glistening, which IOL has more postpone to have more of glistening Alcon Acrysoft or Johnson Tecnis?

      The Johnson Tecnis vs Acrysof almost do not have glistening.

    Had my left lens replaced in March to give me perfect near reading vision (love it). Now facing cataract removed in right eye I'm considering having it made farsighted but there's so little information. Your opinion please?

      Hi! If you can, try with contact lenses first, to see will you adopt or not. If there will be high difference in dipoters, you may loose stereopsis and depth of field perception, but many patients are ok with that.

    Hi i had Synergy lens in my non Dominent eye. The only reason was to eliminate glasess.
    My distance vision is not that great after 3 weeks surgery but my reading and intermediate is good. My surgon said im out by +.50 diopter for distance
    He put for temporary +.50 contact and my distance is perfect now.

      It may happen, seems that you have refractive error after IOL implanted. The reason might be not correct calculation, it may happen. I will create a video explaining that later. The option might laser correction to adjust that issue.

    I took your test and maybe answered wrong on cm distance. I have a scheduled cataract surgery in 2 weeks. I do not want glasses for close or intermediate vision at all. I prefer limited glasses at conferences and restaurants. I drive a lot at night, so glare and haloes are not acceptable. But, I am okay with keeping glasses in my car for some night driving. I use my phone 6-8" a lot and a laptop 15-16" away, a lot, and I look at documents all day from 15--16" away. I am on the go too many places to keep glasses with me, other that the 1 set keptcin the car. My surgeon suggested Vividi Extended vision and said with mini mono, I would need glasses at the ranges I stated. I have read that mixed mono does not require glasses at close and intermediate and my step mom confirms that,,as she had the mixed mono. my surgeon is board certified, but I am worried he does not know mini mono, but can't afford to wait and start over. My optometrist has referred patients to him for 20 years, so he must be good, but he tried to talk me out of mini mono, so I would like your thoughts. thanks!

    Quick question: is there a way to minimize undesired effects such as halo or starburst if you use a set of glasses during the night (while driving for example?)

    Hello Alex, great videos but so difficult to make the right decision on which lens to select.... Vivity lens seems to offer the best overall performance on all levels however, this lens is not yet available in Dubai, UAE. Would you know how to get it ordered and delivered to the local ophthalmologist? and does it require any special tool to insert it? many thanks. Michael

      Hello ! Thanks for your support ! Vivity is not a holy grail or so on. It is a lens with certain cons and pros, as any others. Regarding idea to bring the lens to surgeon forget it, as lens implantation with any new to surgeon lens requires a lot of details to know and so on.     Oh i forgot to mention my dominamt eye has a + 1.750 contact and its not the best for reading but the synergy lens makes it ok And compensates. The symphony lens I understand is not the best for reading but better for sharper distance. Would the synergy lens work well a symphony lens or should i have both synergy lens in both eyes.
    Can you explain your advise with pros and cons.

      Hi again :) Well, normally dominant eye is supposed to be used for far vision, while non dominant eye for close up. You are right, Symfony IOL. is the best presbyopia correcting in terms of far vision, so theoretically it is a good combination in your case i.e. Symfony for dominant for far + Synergy for non-dominant for near. But, I have a limited experience of a surgeon from EU, who observed more halo effect in this combination. So, I may assume that it will be better to try more precise Synergy calculation for your dominant eye.
      ps: I will contact that surgeon to double check if he has any additional data on Symfony+Synergy combination.

    I am curious about the comparison between Eyehance and Symfony IOLs for nighttime glare (night dysphotopsia). Is Eyehance substantially better in that area.? Does one of your videos go into that?

    Hi again.Its been a couple of months since my synergy lens implant.Im due for surgery for my second eye in one month. I was wrong to tell you it was. +50 it is - 50 out in the synergy lens. Im sure my eye has completly stabilized. I do notice that the distance is good but not that clear outside in sunny days. Inside my home it perfect.
    Is it because im out -50 diopter or is that just this type of lense comprimises when im outside amd the distance is greater than being home.
    Or would a sympony lens be good to implant in my second eye.

      Hi! Well, the Synergy needs the most precise calculation and placement in order to give you the maximum of performance. If you have -0.5, it means that your distance will be slightly impaired, with no added value to near (as Synergy has the best possible near any way). The most important question is your -0.5 sph whas measured in a proper way (so called maximum plus technique, or manually - you had to be blurred by trial lens or phoroptor, and then doctor has to find a set of lenses where your vision comes to maximum results). If it is your case, ok, however quite strange that at sunny day you feel not as clear as at home. Regarding mix Symfony IOL + Synergy IOL I will reply under your next comment.

    Hallo an thanks for your report. What lense would you prefere in my case. I want to see a very good picture with highest contrast from 70cm to infinity. I think of this lenses: Eyhance, Vivity and LuxSmart. I have +7dpt and astigmatism and i am 50 years old without cataract. Thanks a lot!

    If you regret the lens you've chosen is it possible to replace it with another lens and is it a riskier procedure that the normal cataract surgery?

    Hi, I am writing to share my experience and hope others may add input about theirs. I am not 'seeing' the results I had hoped but have been told this is temporary.
    Like another commenter, I too hold a commercial pilot's licence and enjoyed eagle-eye vision my entire life. In my mid 40s I noticed having to hold object farther away to read them. I was finally wearing glasses full time by 50 (I am 54 now). However, I could not adjust to wearing glasses. My eyesight deteriorated so quickly I was going through one or more new prescriptions annually. Further, progressives were horrible since I could not get the range from near to distance vision in one lens, so I typically carried two pairs of glasses around. I finally learned of IOLs and took no time at all deciding on having the procedure. I relied totally on the advice of the surgeon to get the Vivity IOL as opposed to the monovision option as not requiring glasses to use a computer was a priority for me. There was an intermediate option I didn't explore much as the surgeon did not recommend it to anyone that needed good night vision or drove alot. That left the most expensive Vivity option.
    The biggest improvement was the change to colours. White is so much whiter. I am surprised I haven't heard others exclaiming this. However, I am concerned about my choice as it has been 3 weeks since I had my first eye done and 2 since the second. While I can wake up, check my phone and make coffee and watch the news without wearing glasses, nothing beyond 20 feet is crisp. It reminds me of when I was a kid and wore grandad's glasses and everything was blurry. My distance vision can change dramatically throughout the day as well. Usually by mid-afternoon, I am all but blind to details at a distance and driving at night is bordering on dangerous. The surgeon told me this would pass and could take weeks plus it has a lot to do with the 4 different drops I am using 4x per day. However, I am skeptical as I am seeing no improvement on a day to day basis and others I have spoken with state their vision was as good as it was going to get in less than a week.
    What are others here that have had the procedure experiences?

      I have Vivity in both eyes with left eye biased more towards near vision. Perfect vision at all distances and no issues driving at night for me.

    My Consultant has recommended placing a Symfony EDOF in my dominant eye and a Multifocal (+3.25) in my non-dominant eye to improve near vision (to N5). What is your opinion about this?

      @@iol-adviser Hi Alex, thank you for taking the time to reply. I have now had the lenses fitted and have wonderful vision (near, intermediate and far). I have a Johnson & Johnson ZXR00 in my dominant eye (right) and a J & J model number ZLB00 in my left eye. I am having flickering (juddering/jerkyness) at the moment, but I think this is temporary, and also some residual dull pain in my left eye, which my consultant has said not to worry about (I am one week post surgery, so I guess it just takes time).I have the usual halos, glare etc, but I am happy in the knowledge that my brain will adapt in time!

      Hi Michele! thanks for sharing your experience! I am glad that this combination works for you well.
      Halos and glare has to decrease, and some patients experience different amount of decrease - it depends on brain adoptation and can not be predicted. Flickering is another issue, and related to individual anatomical specific of the ligament of the lens, and might be present over time. Hope it will not bother too much.

    Should he over correct my dominant eye to adjust and compensate the eye that was operated 3 weeks ago or treat it independently.
    What do you think? Also should I wait longer before I go for surgery on my second eye so my operated eye that was done 3 weeks can fully heal.

      Great question. Well, to be absolutely sure that your non dominant eye refraction is fully stabilised, it is advices to wait for at least 4 weeks, 2 month is absolutely safe.
      And the proper dominant eye IOL power has to correct the issue. You will have so called “micro monovision” which is not really necessary with Synergy, but in your case you should have a good result for all distances.

      In one hand I wait like you said 2 months and not over correct the dominant eye and then deal with laser to correct the non Dominent eye.

      Or I do mico mono vision. He did explain laser correction if i choose not not the micro monovision but said for the small amount of. +.50, is it worth it if my side effects are dry eyes.

      My dominant eye needs +1.75. So for the corection should i add the +.50 to this. Maybe i will get +2.25 contact and see if this is something I could deal with. What would you do?

      Good question. Yes, laser correction definitely may add more dry eye symptoms. If your other eye need correction, you definitely may try contact lens to check visual comfort. Actually, monovisoon lean you leave one eye as is, another correct in a proper way.
      Do you need +0.5 to get the best distance vision, am I understood you right ?

    I wonder what your experience is with the relative new Tecnis Eyhance IOL. Is it true that you sacrifice a small amount of distance vision with the Eyhanace vs the ZCBOO? Pro and cons of each lens please? Thank you!

    In minute 4, the image froze, but the audio, (and captioning ,) continued.
    Could be our signal, but thought you might like to know.
    Only one animation, in the beginning.
    Seems problem started the second time you specified the animated image.

    Is it possible to use monofocol lenses to have very good near and medium vision without glasses and bad vision for far vision? I would like to see clearly without glasses up close and would be much more inclined to have bad far vision without glasses.

      @@iol-adviser I have actually tried it but I'm not sure I would dare to try anything other than monofocal. I've read so many different side effects from multifocal and trifocal lenses. I feel like choosing monfocal lenses that give me clear vision for near sight and somewhat for medium distance is best for me. Do you know at what distance glasses would be needed with this approach? Also thank you for taking your time to help me with this, ut means a lot.
