Presbyopia cataract lenses and use of glasses

    Cataract lenses for presbyopia correction or intraocular lenses for refcattive lens exchange of multifocal or EDOF type such as Alcon Panoptix, Zeiss AT LISA, Tecnis Symfony, Synergy or Alcon Vivity are intended to get rid off glasses.
    However, it may happen that even with the most advanced IOL type, a patient will need a glasses.
    Why it is happening, what is possible to do to fix it, and answer to the question what is a defocus curve, a main characteristic of presbyopia correcting intraocular lens.

    Wow...I have watched MANY videos on this topic. What you have presented is a MUCH more comprehensive discussion than what I have been able to find up to now. Your explanation about the defocus curve and how it relates to IOL selection is something that I have not seen in other tutorials. Thank-you. I look forward to watching your other videos.

      Will i ever need glasses or does lens inplant never changd in perscription.

      normally, once stabilised after 8 weeks it is stable

    I just got my second eye done. To follow up, synergy in both eyes. Im very happy with the results. One thing I have is sometimes I get flickering which I did not get in my first eye. Why is this and will it go away.
    Also now I have no need for glasses at all.Im 50 years old. Are these lens implants going to keep my eye sight this good forever so i never need glasses agian.
    The reason i got these lens is with age my reading was going bad and then was distance,this started around mid 40.
    Now its near perfect.

      I am glad, that you have good results! Regarding flickering, I am afraid it might be problem with cine ligament, and it might not be improved. Some surgeons are skilled enough to suture the lens, but it is not easy. So if it is disturbing too much, I suggest discuss with your doctor possible options.

    I have slight ghosting down and starburst at night. I had cataract Toric surgery Last week. I find now if a put a line on my glasses to black the ghosting down it helps a lot watching tv. My invention.

      @@iol-adviser glare and blury vision outside in daylight. Last night went to dark live theater. I think if I restrict hole of vision I look thru that will help in that setting. Invention works great watching sports where focus is on competitors on the screen

      May be caused by prior problematic lasik surgery

    Hi.. I'm Dinesh, 20 years old.
    I had corneal sutured surgery over the pupil before 3 months due to injury. Now the doctor prescribed for cataract surgery.
    Which Iol I have to choose?
    I have more work with laptop.

    When is the most suitable time for exchange of IOL after implantation if I find the IOL very untilerable with the side effects?

      the earlier the better.. however some surgeons can do that even 1 year after, but it is more traumatic and complicated

    Yes wonderful video..thank you! I do have a problem. I had a Macular hole mostly resolved (not perfect...some parts of letters missing but overall good). I had a Vivivity put in for the cataract I developed after the vitrectomy and have very poor mid range and close range vision (I thought the mid range would be as promised). Anyway now considering having lens exchange with Panoptix multifocal. What are your thoughts? Am I better off with glasses (they are helpful only for reading). Thank you for all your videos!

      Hello again :) As I explained in my other comments - I do not suggest any multifocal IOL as any multifocal technology decreased CS by design, as well as some EDOF designs. The best options for you is Symfony.

      Which lens is the worst for CS? Since I already have the Vivity would it make much difference to get the Panoptix based on what I am used to already?

      I have no data yet about CS loss for Vivity to compare to Panoptix, however I would not suggest any of these lenses for your case. But, final decision is up to your surgeon for sure.

    Does the Lal iol , get around the refractive error calculation

      LAL may give precise refraction, however it is still a monofocal lens with all its compromises

    Hello Alex! I am 64 with presbyopia and I am quite farsighted (+7.5 and + 4.0) and my vision corrects to 20/20 at all distances with glasses. Most people are nearsighted with presbyopia--- So, my question is what should I expect with a PanOptix IOL? Will it affect my "near" focal point and push it to a longer distance on the defocus curve? Is it possible to wear contacts for "near" vision correction after implantation. Thank you!
