Hi, Aleksei! What iol would you recommend for irregular astigmatism? Thank you!
Alexei, Thank you so much for the review! I've been waiting for something new for a long time (As you may remember). I still haven't put in my lenses, the excitement and stress about my eyesight is gone, I'm using glasses for now..... I guess I'm used to it.
Agree, I do not want night halos ……. am seriously considering the new J&J TECNIS Toric lens 🤓🤩😀
Hello sir I want your suggestion Having bilateral hyperopia(+2 and +2.5 resp) in which left eye is ambylopic (va 6/36)and other normal eye having cataract (va 6/18 with ph 6/6) So which is the best choice of iol in such patient Plz give your valuable opinion
As always, explained clearly for the non-professional but requiring some prior knowledge which can be acquired through your other videos. Thanks for sharing your expertise.
Appreciate your point regarding the benefits of Toric lens versus Laser/Mono focal also, the importance of not wearing contact lenses for several week prior to eye measurements etc …. Great information 👍

Really good information.....Thank you !! 👍
Really good information....Thank you 👍
I had surgery 6 days ago and couldn’t read anywhere. Today they looked in my eye and discovered I could see perfectly well with -1.25 cyl of astigmatism: all of his table even the line just below 20/20 I could make out. The lens was Toric and -2.25 and supposed to get rid of all of my astigmatism but here I am now 6 days post op with astigmatism -1.25. And guess what optometrist told me that at this stage they don’t even check the eyes with lenses just ask to read how many lines you see but because I had concern about focus he decided to “check” with lenses and that he doesn’t have statistics about those numbers with other people but assuring me that this is a common complaint and that it is early days and it will subside reduce!!! How??? There is no inflammation in retina and cornea, all other parameters he said are normal after the scan, the position of the Toric lens is correct as I asked him to check so he dilated me (they usually don’t do it for some reason), and still I have that astigmatism. Can that actually be true and it will reduce? One good news: during cataract not a single lens they put before my eyes produced any focus. It is a shame it’s such a lot of astigmatism still left as I paid a lot of money for this operation and the surgeon is very respected but how could he miscalculate so much? Something tells me it won’t go away. I still see a lot of floaters, vision fluctuates all the time. I’m actually getting sick with all of this experience. My second eye operation is coming with the same surgeon. Soon I won’t be able to write here anymore as I will lose the close sight forever. Also apparently I ended up even slightly hyperopic like +0.5 the machine gave the result, but the nurse said it will all change to 0.00 Going from -8.5 before cataract to +0.5 is a mocking really when I specifically told the surgeon that the biggest fear for me would end up hyperopic. I want to stay a bit myopic. I will do my best he said. What??? But in fact it doesn’t feel I’m seeing so well inside the home, it’s blurry. The optometrist also said it will reduce and that at this stage they don’t do tests with lenses. I’m gutted and can’t breathe. At the moment it feels like a biggest nightmare of my life. I wish I went with Rayner where doctor offered -0.75 and -0.5. I don’t need their zero vision. 😢
What is happening when you see patterns on the wall .It looks like wall paper all over the wall.
Hi thank you for your videos! I have cataracts and high myopia but otherwise healthy eyes (healthy retina, corneas, only minor astigmatism). My surgeon said multifocals are not an option and he wants me to get the Tecnis Eyehance lens, but he did not mention EDOF at all. Are EDOF lenses not a good option for high myopia? Thank you!
Is Italian edof lenses brands quality is good ? Or USA
I just had a cataract surgery and here it comes Christmas again. I see some kind of tinsel-type of stars around some light points. And they are massive. I just hope it will reduce as it’s only been 9 hours after operation. My eye is still dilated. Can’t see well at all - all blurry. I don’t know if this will ever improve. I just wish I never had this cataract. I was forced into this operation because of this. I wish I had my old eyes with glasses with high myopia. I will see how I’m tomorrow.
Are you good now my friend? How do yo feel about it after 2+ weeks?
At 78, I'm told that I need cataract surgery, but being myopic, I didn't need glasses for reading until age 55! Having worn progressive lenses for 23 years, I tried wearing a very old pair of glasses (no progression) to see if monofocal, cataract lenses would suffice after surgery & feel that I would not be happy with the results! So, I guess it will have to be multi-focal lenses, even though it will cost thousands more!
YES! Please do your review on VIVITY ASAP! I have had Lasik establishing monovision in both eyes in 2004. I now have glaucoma (left eye is worse) and developed some cataracts in both eyes. VIVITY has been slated for my right eye Aug 8, 2024. My Right eye is my lifeline and my best chance at functional vision for independence in the future. I am anxious to know more about VIVITY outcomes and your thoughts on this lens. My case is not straightforward so I'm trying to learn as much as possible. Thank you! Submitted 7/15/24
Excellent points regarding people who spend 8 hours a day staring at a computer screen. The other 16 hours are spent sleeping and doing other things like you mentioned so trifocal lenses are not always the right choice.
According to your estimate, how many years will it take for lenses with accommodation capability to arrive?
Hello Oleksii, my left eye sees 100% and my right eye sees 60%. I have cataracts in both eyes. It is a slowly progressing type of cataract, developed due to allergy eye drops. I think my right eye will have surgery before my left eye. What would you recommend to someone like me who is healthy? I'm 38 years old. Your general opinion is incredibly important to me.
The information I have obtained as a result of all my research: I am 38 years old and no lens will satisfy me as I do not have any problems other than cataracts. :(
My surgeon has recommended the Oculentis lens. I read there were problems with opacification with these lenses a few years ago.Has this problem been fixed now with the latest Oculentis lenses?
*Quality* of vision: this is so so so important. I have struggled with this just over the course of worsening presbyopia - I literally have not been able to see anything clearly at any distance for years now. The fact that our vision is "tested" using only black letters on a white background, as if this has much to do with how let alone what we *really* see on a daily basis is ridiculous imo. Yes it measures something which a I understand it, is labeled "acuity", but come on - how often do we encounter this level if visual contrast in daily life? Just because I can manage to make out what a letter is, doesn't mean I see it *well*. Yes I can read that last line, but it looks horrible! Yes with progressive lenses I can kinda see at all distances, but none of it actually looks good. It's functional, at best, but not "good" at all, which is hard to explain but if you've experienced it you know exactly what I mean. Yet the optomotrist is happy to usher me out the door, claiming great success with my new prescription :/ I now have a cataract and I'm very concerned that I won't be able to find a doctor who understands that I care about being able to see really well again, at any distance! ETA I don't mean all distances, I mean any of them :D I'm hoping it's possible to stretch that a little bit so I can get top notch distance vision but also comfortably cook or shave my legs with just a bit of blur, similar to how it was when I first started developing presbyopia (with micro mono vision? and/or an enhanced mono lens?) but I'm not cool with gaining a wide range of mediocre visual quality. It's what I've got now and I genuinely hate it.
Hello Doctor from Turkey. Thank you very much for the great information, but I also recommend adding Turkish subtitles to your videos. I am a 38-year-old trainer. What kind of lenses would you recommend for me before my cataract surgery?
i know someone who got monofocal recently. Sees great in day time but disappointingly she sees halos at night such that she doesn't want to risk driving at night. But then again she had nighttime halos from cataracts before the surgery and didn't drive at night. Overall she's glad she got the surgery.
Can computer guided laser "enhancement" (limbal incisions?) at the time of a standard Eyhance IOL placement for cataracts neutralize 1.5 D corneal astigmatism or is it better treated with a toric lens or a combination of both? What is considered most predictable in neutralizing this amount of astigmatism?
Which IOL will you suggest for my 60Y old Mother, who is high myopic (Even after Cataract Surgery, she will need glasses for myopia, that's what Doctors said) and hyperopia with weak retina? standard monofocal or Enhanced Monofocal? these are my options: Hoya Vivinex / Johnson & Johnson Technis / Eyhance or Zeiss ? So, if she will have to wear glasses for less number, is enhanced monofocal worth it? Any disadvantages of EDOF , like reduced night vision or halo or glares?
I am choosing mono lense in my right eye, when the other eye is done Can it be adjusted or selected to improve the closer vision if need be.
Great information thank you 🙏
Well, the first surgeon I saw said he doesn't use or feel the need for laser. Turns out, he also failed to detect that I had something called an epiretinal membrane in one eye. According to the literature, even if minimal, it is a definite no no for multifocal lenses which his office had dates scheduled me for placement until I chickened out for other reasons. My reason for changing course to enhanced monofocals was that getting multifocals with greater likelihood of night time glare, halos and decreased contrast would fail to address my chief complaint of poor vision while driving at night. The undiagnosed epiretinal membrane issue raised doubt as to proper initial treatment planning. The second surgeon is suggesting J&J Eyhance enhanced monofocal IOL implants in both eyes and laser corneal reshaping of the minimal (-0.50 or -0.75) regular astigmatism in each eye. I have an appointment for a laser scan tomorrow and will report back. Sites like this one hosted by Oleksii are of great benefit in discussing the latest updates and many options in cataract surgery in ways that a layperson can understand.
I’m having panoptix lens implant next week for severe cataracts. This information is great and has put my mind at ease. Thank you.
thank you for your feedback!
Can you do a video on possible problems with toric iol implants and how they are addressed? Thanks.
I have +6.25 hyperopia with mild astigmatism in both eyes. -.75 and -.50. The astigmatism is described as "regular." If cost is no object, can laser cataract surgery also be used to recontour the cornea (through a releasing incision?) and would it be preferable over a toric lens implant to correct the mild astigmatism? I am considering Eyhance in both eyes. Your videos are clear and concise. Thank you for your guidance in this complicated field.
What does your surgeon say? The first ophthalmologist that I consulted wanted to do the relaxing incision on one eye due to mild astigmatism (currently my prescription has -1 cyl in both eyes). He never even mentioned the possibility of a toric lens. I don't know if one solution is preferable over another. I have read that it's very important to get the toric lens placed in the right position. I'm hoping any astigmatism I have can be corrected post-surgery with glasses, as I see myself wearing glasses most of the time anyway.

I think one can wear progressive glasses
Yes, why didn’t he mention getting progressives after getting monofocal iols set to distance?
Thanks for this video and others. There are so many things that go into good vision that the lay person has not considered until cataract surgery. There is no one roadmap for achieving good vision, as lifestyle, age, tradeoffs in the choices, should all be considered. An example of monofocal toric lens not extending depth of focus was my neighbor who had cataract surgery recently. He chose distance as his target. He uses two pairs of readers, +3 (for close work) and +1.5 (for computer) for closer distances. The toric lens in one astigmatic eye enables image clarity.
thank you for your comment. that's why I launched my AI powered IOL selection assistant, which is using my instruction and set of knowledge to help
@ckp2ator389 why not just get progressives for intermediate and near vision?
@@CC-uh8gl He never could get used to wearing progressives so instead had two pairs of bifocals (before cataracts). One was computer/reading and the other distance/reading. Now he doesn’t need any distance correction, so just uses readers.

Loved your video. One of the very few with an unbiased description of the Lal lens. Thank you!
Thank you!
alcon IQ monofocal was my choice after much discussion with my doctor. Was -16 left eye, -12 right eye. wore contacts 30 years. Excellent vision. After surgery, could not be happier. 20/15 left eye,20/20 right eye. just use -2 readers. Am 68, had surgery 11 years ago. Had one Yag procedure. Amazing
Long distance is great, what about medium distance?
what's about tecnis Odyssey multifocal iol
not released yet, limited market release data is not full enought to tell anything. I'm monitorind the information and will release the review once gather the data I can trust
Can you please explain the difference between Mono-focal plus and mono-focal?
done in the video about advanced and enhanced monofocal IOLs - loos on the channel
Thanks so much for posting this. I was trying to decide between two different lenses. After seeing this video, I decided to get Envista Toric monovision.
Your insight is always appreciated. Is there any difficulty getting best visual acuity when using advanced monofocal lenses when combined with progressive eyeglasses? I read the following: "The other issue to consider is that if you use the Eyhance lens it will compromise your ultimate vision with eyeglasses. Eyeglasses can make a correction when the diopter amount is off. However, the Eyhance lens has a progressive power change with the radius of the lens. That is how it gets the EDOF effect. It smears the focus point over a range. A good aspheric monofocal like the Tecnis 1 will bring all the light to a single focus point, and will be easier to correct with an eyeglass lens. The contrast sensitivity or MTF will also be higher with the true monofocal." per Dr. Ben LaHood
Interesting, I was considering the Eyhance since the first ophthalmologist I saw was touting it, in that with distance selected as the target, I'd get some intermediate without glasses. The second ophthalmologist suggested I pick intermediate as the target distance, and her office uses Alcon Clareon for standard monofocal. I'm going with her. It took me awhile to come to the conclusion I'm simply going to replicate my current situation, nearsighted needing progressives, only without the cataracts. Progressives are expensive but I figure I won't be changing my prescription often, if at all, with the cataract afflicted lenses gone.

Thank you very much for your information. The best expert in IOL consulting.
many thanks for your feedback!
Thanks for your AI IOL advisor. It validated my research so far that a monofocal lens would be right for me. Are you planning to add a recommendation for optimal target refraction range based on the user's data? I liked the "key points to discuss with your doctor" as I have an ophthalmolgist appt next week.
many thanks for your feedback! sure, it is in the pipeline. a lot of things to be added.
Dr. how could I get a price list of your IOL procedures?
I am on the younger side, have cataracts in both eyes, but one is much worse. I am considering getting clareon multifocal in bad eye and waiting on advances in technology for the other. What is your view on this strategy and expected advances in accommodating and other iols in the coming few years
Dear Oleksii, you wrote JnJ Symphony ( -> PureSee). Does it mean that PureSee is a new version of Symphony? Still don’t know if PureSee Toric is any better than RayOne EMV Toric. There is no review from people regarding PureSee. I found just one thread online that’s it.
correct, PureSee is a new replacemend for Symfony. Not awailable in all the countries yet, but the initial resutls I have from the markets where it has been released are very promising
PayOne is another class of IOLs - it is not presbyopia correcting, it's an enhanced monofocal. Watch my video about IOL types to know more

Many thanks for very useful and actual information
Glad it was helpful!
Considering the dysphotopsias associated with multifocal lenses and concentric circles, would you consider J&J Eyhance monofocal a reasonable compromise?
Yes, if you prefer dysphotopsia free vision and ready to use reading glasses or distance in case of myopic refractive target shift.
@@iol-adviser Is there any complication in fabricating progressive spectacles to correct for the decrease in distance visual acuity if using Eyhance in non-dominant eye to match the clarity of the standard Technis monofocal IOL in the dominant eye?
