  • Do you ever wonder if cracking your neck or getting a chiropractic cervical manipulation is good for you? Dr. B shares her decades of expertise as an orthopedic surgeon.
What happens when you crack your neck? Your neck contains blood vessels, nerves, and complex, sensitive structures that can (with documented cases) be damaged, particularly by high-velocity neck cracking.
But what about when you visit the chiropractor and get a neck adjustment? Is there any benefit to that in medical studies?
Finally, Dr. B explains the difference between high-velocity neck cracking and when you're stretching or exercising, and your neck just pops.

Agree. I used to work with an emergency room MD. Oh, the stories about people who had their necks cracked.

My cousin cracked his neck while walking down the stairs. He passed out, fell down the stairs, and broke his ankle... he does not crack his neck anymore.

As a Chiropractor... may i chime in?
I have really valued the information Precision Movement is putting out there. As a chiropractor I thought I would just chime in. We as a profession do... or at least most do... recognize that there is some inherent risk with any procedure... including cervical manipulation. There is at least a correlation between neck manipulation and stroke/dissection. The manipulation is certainly causative in some of those cases... but it is also true that patients who are already experiencing the early symptoms of dissection, neck pain and headache, often in the absence of any neurological lesions, are presenting to chiropractors for relief. The adjustment may worsen the already existent dissection, but its also possible that that patient would have progressed to stroke/neurological injury/death had they not gone to the chiropractor. We also know there is a strong correlation between VBAI (neck artery stroke) and patients going to their MD. The MD didnt cause it... but there is a correlation. We know correlation and causation are very different things. I do understand the fact that the mechanism of injury is present with a chiro manipulation, so certainly causation... or at least contribution to the stroke, is higher with chiro than the primary care doc... but I use that to illustrate that these patients are presenting to clinicians due to dissection symptoms, suggesting the dissection occurs prior to their presentation and its driving them to the clinician. There are of course cases of asymptomatic patients being manipulated and suffering a VBAI and in these cases the argument for causation over correlation is greater. However, there are genetic factors that play into this condition and we know dissection/stroke can occur with other routine neck movements (hair washed at the salon, normal rotational movements of the neck like looking over your shoulder while driving, sports injuries etc) and also spontaneously. Due to collateral circulation from two vertebral arteries its possible that someone with adequate circulation routinely but with a congenitally narrow artery on one side suffers transient ischemia from a neck movement. Additionally, agenesis of one of the vertebral arteries can increase the potential that somebody has neurological injury from a neck movement (chiro or otherwise) due to transient ischemia. So its possible that the asymptomatic patient that gets adjusted and has a stroke, may have suffered the stroke at some point from some other neck movement/injury in the future due to underlying genetic weakness of the arterial wall, agenesis or congenital stenosis of one vertebral artery etc. The number of incidences also needs to be considered in the context of the shear volume of cervical manipulations performed by chiropractors. Most chiropractors will adjust 10's to 100's of thousands of cervical spine joints over their career and almost none of them will see a single case of this kind of stroke. Now if youre the one that has the injury... it is devastating and life altering.... but we need to be fair in how we measure and talk about these things. The amount of improvement in quality of life experienced by chiro patients getting their necks adjusted for most of them makes the relatively low risk of complication worth it. To be fair... if we applied the "there is risk of bad things happening" argument to orthopedic surgery (your specialty Dr Boynton) or for that matter any single medical intervention, prescription or OTC medication, we would have to equally say... "Stop! Dont do it" to nearly everything a clinician does to try and improve the health and quality of life of their patients... possibly even Precision Movement exercise programs.
So I say all this that the discussion can be measured and fair and another voice on the subject might be heard. Ultimately patients need to be made aware of the potential risks and decide for themselves if the potential benefit is worth the potential risks.
I do very much value the information Precision Movement is putting out their for the health and betterment of people. So thanks for that!

Have you done a video on AC joint injury? I injured my right AC joint from bench pressing in 2020, and it’s still not quite the same. Then I injured the other one dumbbell pressing a year later. My hunch is that my posture shifted to a rounded shoulder. It would be interesting to see a video discussing why people injure the AC joints. Thank you.

Look into the history, into how chiropractic started.... do yourself a favour, and see experienced doctors, instead. ✌️ Thanks doc for your insights... I'm curious what inspired this episode.

My neurologist made it crystal clear that no chiropractor should do that, a clot can be thrown leading to a stroke.

I went to a quack chiropractor in attempt to fix my neck pain and I got zero practical advice, all she did was full force lunge at my neck and she tried 2x and my neck would not crack. She was puzzled and said come back next week and another $60 for 10 minutes. It was a reputable place, well known in town, so I was shocked. I went 3 times and I actually prayed then I felt really scared to go back and I did some research on the inventor or creator of the chiropractic technique and I was in no way going to go back to that. Cracking your neck with high velocity looks violent and barbaric. I’m so glad God kept me safe from that quack. 🦆

Literally, doctors know Jack about the spine, they only were taught how to cut and to give drugs. I have 11 years of experience with that 🤷‍♂️

Well this one is obviously different. She doesn't want you to get surgery or take drugs for pain. Changing movement patterns is the better solution.
