  • Also known as Secondary Ventricular Standstill.
    Inherent Rate: Less than 20 Ventricular beats per minute. ACLS algorithm per Asystole protocols must be initiated immediately.
    INTERVENTIONS: based on AHA 2016 ACLS Guidelines.
    - Perform high-quality CPR
    - Start IV/IO, draw/send labs
    - Give Epinephrine 1 mg (1:10,00 solution) IV/IO followed by a 10-20 mL NS/LR flush, repeat every 3-5 minutes
    - Correct possible causes
    - Consider advanced airway
    - NO shock! There are no erratic impulses to stop

    Just so you know. Fine VF is the death warning, Agonal is the beginning of the heart's flatline, and Asystole is when the heart fails and then flatlines(dies).

    Agonal can be treated if the right steps are done. I asked a doctor from Harrison, KY and they stabilized a patient that was in Agonal.

    I have also heard of this as being Ventricular Bradycardia. Slow heart rate and wide QRS with no P waves means ventricular rhythm. This is commonly seen after V fib but so is NSR. V tach is another commonly seen rhythm after V fib is treated or is paroxysmal(rare for V fib to be paroxysmal. Usually if it is paroxysmal, it is V tach or A fib). I wouldn't be surprised if a person dying of old age had Agonal or Ventricular Bradycardia in those last few moments when they are still breathing but are unconscious.

      Caters Carrots Typically, patients will start out in Pulseless VT, then progress to Coarse VF, then to Fine VF, then to Asystole. You don't see agonal very often, but I've seen instances of patients being defibrillated in VF and then going to Agonal after the shock. As for the elders being in Agonal, I could understand that.

    A person experiencing agonal rhythm is no longer conscious, but reflexes in the brainstem are trying to keep them alive. Unfortunately, once a person's heart is in an agonal rhythm, *death is imminent*

    The heart that was been with you though all those great moments in your life, beating away with a lovely sinus rhythm has sadly had it's last beat and now replaced with nothing but the occasional wide and unrecognisably bizarre QRS complex as the heart cells die it tries and tries weakly but fails leaving nothing but a flatline.

    I think agonal is worse than asystole. If we consider that in asystole, if recent, the big majority of heart cells are still alive.
    But agonal is specifically caused by the death of almost every cell on the heart.
    There is, however, asystole that comes after agonal, and if it's that the case, my above statement is invalid.
    PostData: I'm not saying you can't stabilize an agonal patient, but the prognosis is extremely poor.

      Agonal is the last semblance of electrical activity of the heart prior to death. It represents a dying heart and has been seen on even people dying from old age.

      Yeah recent asystole is more treatable with adrenaline than agonal because it’s more cells are alive adrenaline will force blood into the heart restarting it

      I'm not a doctor, but I think that the asystole that comes after agonal rhythm is way less treatable than that that occurs after v fib (correct me if I'm wrong). Because in v fib asystole most cells are alive and can be restarted. But after agonal rhythm asystole most of the cells are dead so restarting the heart is much harder. Correct me if I'm wrong.

      Well, this usually happens from old age where a majority of the heart cells and dead and few alive cells vibrating to get the blood flowing. Asystole can happen from a cardiac arrest or a myocardial infarction, which is also different because most of the cells are still alive.

    You start feeling pain in your chest. Minutes pass with the doctors trying to correct the situation, but to no avail. The Grim Reaper walks in the room, it gets very cold.     I don't think is an agonal. The morphology and the high rate makes me think it's a slower version of idioventricular rythm. Administration of norepinephrine @ 2-3ml/mg depending on age and sex of the patient. True agonal will have much wider comples with extremely bizzare morphology and the rate almost always remain below 15.

    Agonal rhythms are dysrhythmias, most recognized as the last ordered semblance of heart activity before asystole. Agonal, ventricular in origin, is caused by weak cardiac contractions in a person's last moments before completely shutting down.

    Can you imediantly fix this agonal by fast injection of atropin or suprarenin? Or do you have to start cpr? (I hope you do understand, my English is not very well ;-)

      The treatment should be injecting adrenaline (epinephrine) and starting immediate CPR, but the chances of recovering are extremely low. I'm not a native English speaker either. ;)

      Agonal heart rhythm is considered a form of Asystole (aka a flatline), Agonal is a treatable rhythm, but the chances of these efforts being successful are EXTREMELY low. This is pretty much the heart’s last signals of life before true death arrives. At this point, a vast majority of the heart cells are dead or are dying out, while the heart cells that are still remaining are trying to keep the heart beating until they can’t anymore.

    Wide complex QRS bradi', an escape / ventricular standstill. This signifies massive myocardial death. Hadn't it been a defib's test sequence, I'd have turned the surrounding machines off, 'tmakes no sense. Nothing reverts this rhythm.

      @@arny625 based on what? Maybe if you know what to do you could save the person (at least with permanent brain, organ damage)
      I guess that you say its impossible because most neurons die in the brain. But there is a possibility that life-needed neurons are still untoched. So maybe you can stil save a person

      @@alaas1041 what you see is the heart unable to work unable to pump blood unable to send signals probably due to lack of o2 or old age there is too much of a pause between beats I feel like from the music in that video it’s much clear it’s a variant of asystole it’s not shockable & hard to come back from meditation can maybe bring them back but depending it may have caused problems by then there will be damage to the heart lungs brain etc
