Accelerated Ventricular Rhythm (AVR)
Ventricular Rate between 40 - 100bpm faster than the Purkinjie or Ventricular inherent rate (20-40bpm). No P waves. QRS is wide and bizarre with R wave having opposite polarity to ST segment and T wave.
INTERVENTION:
Correct possible causes (severe hypokalemia, toxins, MI). Follow AHA 2016 ACLS Bradycardia Guidelines. Give Atropine 0.5 mg every 3-5 minutes maximum of 3mg. Start pacing (TCP) or if ineffective, give Dopamin 2-20 mcg/kg per minute or Epinephrine 2-10 mcg/min titrate effect.

Atrial Fibrillation with Rapid Ventricular Response (RVR)
zhlédnutí 170KPřed 12 lety
Atrium is quivering or fibrillating. Rhythm is irregularly irregular with no discernable P waves. It can be acute or chronic. If Ventricular rate is less than 100 beats per minute then it is call Controlled or Slow Ventricular Response (SVR). If ventricular rate is over 100 beats per minute, then it is call Uncontrolled or Rapid Ventricular Response (RVR). CAUSES: Alcohol abuse, Congested Heart...
Pulseless Ventricular Tachycardia (pVT) - Monophasic Defibrillation at 360 Joules
zhlédnutí 165KPřed 14 lety
Pulseless VT is treated the same way as Ventricular Fibrillation would in AHA ACLS 2016 Guidelines. CPR with immediate Defibrillatory shock is recommended. Biphasic Defibrillators: 120 - 200 Joules or recommended. Monophasic Defibrillators: 360 J. Other secondary interventions can be started if IV/IO access has been established. Medications that can be given with pVT given during compressions: ...

Polymorphic VT: Torsade de Pointes
zhlédnutí 409KPřed 14 lety
French for "Twisting of the Points," a rapid polymorphic Ventricular Tachycardia. It is usually caused by hypomagnesemia, hypokalemia, and malnourished alcoholics. Although Torsades de Pointes (TdP) is a rare ventricular arrhythmia, it can degenerate into ventricular fibrillation, leading to death without rapid medical intervention. TdP is associated with long QT syndrome, a condition whereby p...
Pulseless Ventricular Tachycardia (pVT)
zhlédnutí 319KPřed 14 lety
HIS Purkinjie Rate: 20-40bpm. Tachy Rate: more than 100bpm. Pulses must be verified to be absent before identifying this wide complex tachycardia to be Pulseless VT. This rhythm may or may not precede with pulses being present. But with this rate ventricular contraction does not allow ventricular filling therefor pulses are going to be absent at any given time. This rhythm can only last for a c...
Premature Atrial Contraction / Complex (PAC)
zhlédnutí 330KPřed 14 lety
A PAC is not a rhythm, it is an ectopic beat that originates from the Atria. The underlying rhythm is Sinus Rhythm at 69bpm with PAC. Correct the causes to avoid usurpation (taking over) of Atrial rhythms. It can occur every other normal beat (Bigeminal), every other third normal beat (Trigeminal), or every other fourth normal beat (Quadrigeminal). It can also occur in pairs (coupled) or three ...
Primary Ventricular Standstill: P wave Asystole
zhlédnutí 121KPřed 14 lety
The Sinoatrial Node is constantly firing at its inherent rate: 60-100bpm. The electrical conduction is blocked from going through the AV Node preventing Ventricular depolarizations. Immediate interventions is required following AHA 2016 ACLS Guidelines of Asystole algorithm. TREATMENT: - High quality CPR - If IV/IO are in place, give Epinephrine 1 mg every 3-5 minutes - May attempt TCP per card...
Supraventricular Tachycardia with Synchronized Cardioversion

SVT Rate: 150bpm with no visible P waves (AHA 2016 Guidelines). Unstable patients needs immediate Synchronized Cardioversion. The machine synchronizes itself to your patients ventricular depolarization (QRS) but shock upon discharge on the relative refractory period during ventricular repolarization (downslope of the the T wave). This prevents an R-on-T Phenomenon (causes dysrhythmias). SVT, At...
Sinus Tachycardia with Bundle Branch Block
zhlédnutí 126KPřed 14 lety
Sinus Tachycardia 100-150 bpm (AHA Guidelines 2005). Impulses originated from the SA node with a notched R wave (making it look wider). There is a block in either the Left or Right bundle branches. 12-Lead ECG is needed to identify the delay of conduction. Modified Chest Leads (MCL) 1 or 6 are used for continuous monitoring.

Impulses originate from the AV Junction area and not the from the AV Node. The AV Node is only a gate-keeper of impulses and has not capabilities of creating impulses. Junctional Inherent Rate: 40-60 bpm. Junctional impulses have an Antegrade (upright waveforms) and Retrograde (inverted waveforms) electrical conductions. Markers are: Inverted P wave either before or after the QRS. P waves can a...
Fine Ventricular Fibrillation
zhlédnutí 207KPřed 14 lety
The Ventricles are quivering/fibrillating and NOT contracting at all. This occurs after 5-6 minutes of Coarse Ventricular Fibrillation if no interventions were initiated. INTERVENTIONS: Based on 2016 AHA Guidelines - High quality CPR - Do not defibrillate this rhythm - Give Epinephrine 1 mg every 3-5 minutes - May administer Ami...
Cardiac Arrest
zhlédnutí 7MPřed 15 lety
The most common cause of a lethal dysrhythmia is by a PVC hitting the peak of the T wave (known as R-on-T Phenomenon). This causes a pulseless shockable or non-shockable rhythm in at risk patients. 90% of cardiac arrests in adults are shockable (Pulseless VTach/Ventricular Fibrillation). This rhythm started with multifocal PVCs to a wide complex tachycardia (Pulseless VTach). This can only last...

This can happen if you have a heart attack, an untreated cardiac arrest, if you shock fine vf and if you shock the patient too much. This is fine ventricular bradycardia. 18-4bpm
