    I'm VERY SAD....... Had my CAC a few days ago and it was 167. Have been on dedicated Ketovore the past 6 months (90% meat), but it appears I should have started decades ago. Lost 35lbs, but the other blood numbers just moved a little, not a lot. Being proactive, I set up a face to face meeting with Dr Cywes at his office. Let us see how this goes..........

      Dr Cywes, thank you for explaining the right healthy metabolic numbers. I been on carnivore for 90 days, my triglycerides and VLDL went up?! Does not make sense then how carnivore can heal your body? .😢

      A CAC is a measure of calcified plaque buildup over your lifetime. It is very unlikely to go down. What you want to focus on is stopping it from going higher which sounds like what your doing.     Im76 yrs. Old, am 5 foot 10 and weight 240. I started 16, 8 fasting 4 days ago and cut out all sugars. I am 20 lbs lighter today, so it's working.

      It's working if you're not hypothyroidic, have good hormonal levels and have good blood sugar levels, and I doubt you have these 3. Keto can be helpful, but it must not be a way of life.

    Key points by Bard:
    Body weight: If your BMI is 26 or above and you are 50 pounds overweight, you are at a high risk of metabolic disease.
    Blood glucose: An average blood sugar above 120 is a sign of insulin resistance and hyperinsulinemia.
    Hemoglobin A1c: A hemoglobin A1c of 5.7 or higher is a sign of metabolic dysfunction.
    Cholesterol: A total cholesterol below 150 and an LDL cholesterol below 100 are signs of metabolic dysfunction.
    Triglycerides: Triglycerides above 125 are a sign of metabolic risk.
    HDL cholesterol: HDL cholesterol below 50 is a sign of metabolic risk.
    VLDL: VLDL above 18 is a sign of metabolic risk.
    Uric acid: Uric acid above 6 is a significant metabolic marker.
    Gamma glutamyl transferase: Gamma glutamyl transferase above 15 is a sign of liver inflammation.
    Ferritin: Ferritin above 150 or 300 is a marker of inflammation.
    Resting heart rate: A resting heart rate above 85 is a concern.
    Blood pressure: Blood pressure above 140/90 is a concern.
    Physical activity: Less than 1 hour of physical activity per week is a concern.
    CAC score: A CAC score above 400 is a serious risk factor.

    48 years old, bp average 118/124 over 64/74, was 486lbs, down to 336lbs. 7 months keto/carnivore, Cholesterol went up to 269, HDL 49, Triglycerides 82, LDL 205, non-HDL 220, yes we will talk in June for during our appointment, but just joined a NYC Mount Sinai Hospital Cardiovascular screening through work Union due to high stress job. Have scheduled in March CAC scan, Nuclear imaging scan of heart, echocardiogram, peripheral arterial ultrasound, ultrasound of carotid, will be a week long screening, hope I make it through. During todays Cardiologist interviewing and consulting, rolled his eyes when I told him keto/carnivore to lose 150lbs in 7 months, but he did say things are changing how they view high cholesterol, but went into a 30 minute conversation about statins and cholesterol bi weekly shots(talking a foreign language, all medical terms). I told let’s see how the tests come out. Will bring all tests results to our visit in June.

      Oh average blood sugar with CGM 5 day is 95, A1C is 5.1. Had to stop CGM, alert when I drop below 70 annoys me, can stay around 50-80 and I feel great with energy

    Measure your waist (correctly). Divide it into your height. If the number is higher than (.5) you are at risk of heart disease.

    It takes time. What was your CAC before the 6 months? Probably higher, it takes time.... it's a life style of eating, and some exercise if your at a point where you can exercise.
    You might consider a strict CARNIVORE OMAD eating.
    I do believe that your results would dramatically take off and continue/get the better benefits that you are expecting/wanting!!
    YOU CAN DO THIS!! DO IT FOR YOU AND FOR AMAZING HEALTH BENEFITS!!

    A better measure is waist to height. Your waist should be less than half your height in inches. BMI is way off for heavy frames.

      I’ve had the initial Covid shot & all boosters, plus RSV, flu & pneumonia shots, shingles shots (a couple of years ago) & no problems. Lab tests show no inflammation. I’m a senior citizen.

    How big is person-to-person variation on optimal blood sugar? My 12 hour fasting blood sugar after a large meal is still typically 90-95, which means my average is probably 100 or so.
    I feel like my metabolism works pretty well. I've been carnivore for about 6 months now, and I can fast for 3 days easily. I'll get a .2 ketone reading after 5-6 hours of not eating, and .3 after 12. I get into ketosis pretty quickly and never feel hypogycemic. Feels like I'm metabolically healthy, but my average blood sugar is almost certainly closer to 100 than it is to 80.

    Please clarify lipid panel. Cholesterol should be above 150 or problem if above, triglycerides should be over 125 or problem if above, etc. If you could put numbers on paper and post that would be a great help. I fully understand other markers, yes BMI is the problem but everything else is ok except lipid panel since I didn't get it. Always looking forward to your videos. Thank you.

      He said total cholesterol under 150 is a problem. I tend to agree with Dr. Cywes, but you should be aware that it's non-standard advice. It's in direct opposition to what most conventional doctors say/believe/practice.
      He said triglycerides over 125 is a problem. This is not opposite of conventional doctors but it is a little more strict. Most doctors follow the "normal range" provided by the lab company (Quest, Labcorp, etc.) which say that anything under 150 is normal (and therefore presumably fine).

      High cholesterol is not bad for you, new studies show a higher level is healthy and some what " protective" ...Trig should not be higher than 80 and much lower is much better
      Like Dr. Berry says, Fire that Dr. and find one that knows something about REAL nutrition and metabolic health!!

    Concerning calculating BMI for older folks, do you use the height you were before you got old and shrunk or do you use your current shorter height? My doctor said he'd never been asked that before and couldn't answer. For me, I've lost 2 full inches due to osteoporosis. It does change my BMI depending on which height measurement I use. If anyone could answer this I would be most appreciative!

      As a medical assistant I do take patients height and weight daily and when I put it in the computer it will calculate the BMI with the height you are currently at. So if you’re 5’9” and you were 5’11” your BMI is going to be measured at 5’9””.

      Several tests can be ordered just by going online to LabCorps. I was surprised. There are other places that will also let you order your own tests. Good luck!

    Hello Dr. Cywes, I am a type 2 diabetic on a strict Carnivore diet. I am not necessarily thinking about Carnivore as a lifestyle but a tool 12 - 18 month tool to bring down a decades long obesity issue. I am 1 month in and I have not felt this great in a long time. I have lost 30 out of 130 lbs so far and I am optimistic that I am on the right path. I have started watching your channel recently and feel like you are an invaluable source of knowledge. My question is about Metformin and Gliclazide and me being on an Essentially Zero Carb / High Fat regiment ??? Do you or any of the super smart people in the chat have any thoughts on the matter before I bring it up with my physician ??? When might I start weaning off Type 2 diabetes meds ??? My intention is to reverse the diagnosis. What, if any, are the signs that type 2 meds are no longer neccessary ??? And ... am I on the right track, or what am I looking for in my blood-work that will give me cause for celebration ???

      #1 Work with your doctor
      #2 You have to measure your blood sugar regularly, otherwise you're shooting in the dark. If you don't know how to do this, find someone qualified that knows and let them teach you.
      #3 Each time you make a measurement be sure to capture the date, the time of day, and if you were fasting or not. Write it all down or put it into a spreadsheet you can print out and take with you to the doctor visit. If you're up for the extra work and have time, you could also keep a food log.
      #4 Measure the glucose in the morning when you're fasting and once in a while do a random measurement at some other time.
      #5 If your meter gives you a whacky number make second measurement. Example: you're accustomed to seeing 130 - 140 in the morning, but suddenly you see 180 or 100. Redo the measurement. Sometimes the meters can be flaky, esp if the blood doesn't draw up into the test strip correctly. Write down both values.
      #6 When the numbers are coming in normal or close to normal it's probably time to cut or reduce dosage on ONE of the meds, but this should be done by your doctor.
      #7 If your numbers are coming in too low then you should follow the instructions your doctor previously gave you for managing a low (probably eating a small amount of sugar).

      @@jmphome9793 18 is the conversion factor for glucose. Triglycerides, Cholesterol, etc. all have different factors.

      @@chaz6399 Good point, I assumed glucose especially when talking about insulin resistance. Keto IQ is used for glucose and ketones monitoring hence the conversion, we have the same thing in Canada, always have to convert when many are talking about CGM or glucose in YT videos.

    My husband is 66 and 5’10 and 250. It’s all in his belly. He’s had a big gut for over 30 years. He eats healthy but likes his occasional bourbon and cigar. He’s had a knee and shoulder replacement so he doesn’t exercise. His knee gets a tight feeling and swells up. His cholestrol and blood pressure have always been normal.

    I'd like to add a comment about the CAC score thresholds of 100 and 400.
    The meaning of the number/score depends on the age of the patient.
    I would regard a 45 year old with a CAC of 100 a 5-alarm fire, because this patient (in theory) has ~40 more years to live. Unless he fixes the root cause he's got serious problems ahead, maybe not in 10 years but for sure in 20.
    A 75 year old with the same score is much less worrisome because he's getting near end of life with a shorter runway for that ASCVD to develop. It would still make sense to address it, but the urgency would be less.

    "GGT above 15" is a sign of a fatty liver. My GGT was 63 IU/L with a normal range stated as 6 to 64. Which is right? Upper limit 64 or 15?

      For many lab tests the "normal range" is statistically calculated from a large number of measurements taken from the blood samples of a large number of people (presumably other customers of the lab company).
      If you take all the measurements, then calculate the mean and standard deviation (s), you can calculate the upper and lower limits of normal as follows:
      Lower limit of normal = ( mean + 2 * s)
      Upper limit of normal= ( mean - 2 * s)
      Having a value inside this range doesn't necessarily imply healthy and it definitely doesn't imply optimal.
      Now consider that a huge portion of the US population has fatty liver and their GGT test are included in the calculation of the "normal range" of GGT. Because so many of us have fatty liver (and probably elevated GGT) they shift the upper limit of normal to a value higher than it would be for a truly healthy population.
      Think back to getting a report card in school with a letter grade.
      Your GGT of 63 is barely under the upper limit of normal, so it's roughly analogous to letter grade of a D. It's passing but just barely.
      Dr. Cywes wants his patients to have A's (excellent/optimal values).
      By the way, there can be reasons other than fatty liver for mildly elevated liver enzymes. So while it can be a "sign of fatty liver" it's not 100% diagnostic.
      For example if you were drinking alcohol or taking Tylenol in the days before your blood was drawn you could easily have elevated liver enzymes (GGT, ALT, AST, etc.)

      I was wondering the same thing. Dr. Berry likes to see it under 10 for sure, and preferably under 5.
      Of course if it's too low then that's also a problem. Dr. Berry's cut off on the low end is 2.
      I'd still love to hear Dr. Cywes cutoffs.

    However, the keto meme has got to end guys. If it helped, fine. But you have to consume carbs if you want to get your glycemia in check. Carbs will restore your hormonal levels, hence regenerating your whole physiology. Saladino is right, look at his labs work.
