    I appreciate that you make a point to be inclusive in your videos. As a Black woman I find that us darker skin folks are often left out a lot of these videos. We like laser and ultrasound treatments too! I recently started following you bc of that. You have another laser video and you talked extensively about laser treatments for people with darker skin.       It is really scary. When you buy or lease a device directly from the company, its will be the most updated platform with updated software. It obviously more expensive but worth it for patient safety and superior outcomes. Because some of these devices can be expensive (North of 100-200k) offices will instead will buy a used model from a third party laser broker. Instead of 300K its about 20k but its old technology with parts that may not be original. This is the side of the industry that usually hidden. Aesthetics is a very leucrativeindusty and everyone is trying t make money. Laser brokers are no different. But to laser specialists who know better, we refuse to buy old or used devices because we know the value of the newest models that come straight from the manufacturers. I try to give the consumers insight to these things that go on in this industry to help protect you. Don't be afraid to ask a provider where they got their devicefrom. Hopefully they will tell you the truth and hopefully its from the company directly.

    Ladies, listen to our friend Dr. Kappel. Knowledge is power. I purchased an Ultherapy package from a MedSpa in Newport Beach, CA. I was numbed and it still hurt like hell. I was in tears towards the end of the treatment 😭
    I feel like I lost some volume/fat. Never again!

      Any time anyone numbs you for an energy base device- that is a red flag. The companies say not to numb for a reason.

    Its the first time I’ve heard you mention Tixel, and would appreciate your further insight.
    I had an accident many years ago and now have Titanium bolts and screws interfacial, so have steered clear of Thermo or Energy devices that could potentially cause damage, then landed on heat only - Tixel, so far so good, with this year being my first treatment... What other options would you recommend for scarring and skin treatment taking into consideration internal hardware?
    You have taught me so much, but I need to be aware of my limitations. Thank you.

    Thank you so much for the informative video, Dr. Kappel! Your video raised some alarm bells for a n upcoming procedure I have. I have three sessions of Secret RF with a double board certified dermatologist’s office (performed by an FNP) to help fade two puncture wounds on my cheek from a dog bite. They prescribed me a topical triple anesthetic cream and told me I will be further numbed on the day of the procedure. Would you say that is a red flag and that I am supposed to have the biofeedback of the heat?

      Heliocare will not block the conversion of Vit D from UVB light as it does not block 100% (like sunscreen). Great question! And welcome to our family ;)

    I had JUST Fraxel Repair (dual) and I lost so much weight in my face that people didn’t recognise me so this doesn’t seem to be true. Sooo many people are complaining about fat atrophy due to skin lasers that should be just for texture

      Did your face feel very warm during the procedure? Fat atrophy is likely caused by heat, heat destroys fat cells. Not sure if the laser itself damages the fat but the heat from laser certainly will. I wonder if ice/cooling packs prior to laser treatment will help? Too cold temperature will also cause fat loss by the way, so don't go too cold with cooling/ice packs.

    Hi Dr. Stefani. I am Skin Type V and I am nervous about having a treatment with an energy device. But I am noticing some minimal lower face laxity (neck/ jawline) and want to try a one & done treatment for tightening for my skin type...before an actual neck or lower face lift. I just turned 50 and want to get ahead of any further laxity. Do you have suggestions on what energy device (Thermage FLX?)     Hi, there. Great video! I am looking in to home RF devices like The Lumo. I live far from any salon. I am also afraid of fatloss. If a person use a RF machine (themselves) and they feel ok during the procedure, does that mean the temperature from the device is not going to cause fat loss. ❤ And if a person have the RF device a little too long over an area, will that case fat loss or is it more over time using it wrongly?

    I got lidocaine gel before CO2RE. It still hurts and the tech used a fan type device to help with the smell of burning skin. Was it because it is a stronger laser? It was a full face resurfacing with double passes on my cheeks where my acne scars are. It is normal that there is a lot of swelling which means it is strong but doesn't using numbing cream depend on how strong the laser is?

I've been offered Pico for this but I've also been told by others Pico should not be used on the arms and isn't the best for spot treating

    Hi Stefani, from your fellow Beauty-Nerd, that was great information! Claudia who is quite a sweetie from @claudiaglows mentioned she had a CO2 laser and also had three Radio Frequency Microneedling sessions, which she believes left her face quite gaunt. You are right you need to go to the right provider and use that right devices. Fortunately she shared with everyone how she tried using Sculptra for several sessions without much success, then she used Lanhuma to restore fullness to her face, I am so happy for her. Claudia face does look fuller, and of course she looks quite lovely. Thank you for taking time to share with all of us❤

    I'm wanting to try thermage for my neck but have a phobia about throat tightening. Will the swelling cause that sensation? I'm overwhelmed with options but most concerned about the after care and effects. Thank you so much for educating us.

      Don’t do it. That’s one of the main treatments that has caused issues. There’s another CZcamsr, Dr., Rajani, he says that 80% of practitioners use these devices are incorrectly. If it’s more likely that the therapist is going to use the device incorrectly, because these devices are too complicated, it’s not worth the risk.

    We're not supposed to be numbed even with ultherapy? The display doesn't provide enough feedback of depth? I've had both ultherapy and thermage done, and all my consultations stressed the importance of not numbing and needing feedback with thermage, but is necessary with ultherapy.

      I get micro needling with PRP several times a year, and I do not think so. There’s no laser or heat involved in micro needling with PRP.

    If we ask a medical office/medspa if they use the most current software, or how they dilute their injectables, are they required by law to answer truthfully? I feel like if they're not required to be honest then a lot of places wouldn't be truthful

    Hi - can you explain the difference between V Beam and Cutera for redness? My dermatologist uses Cutera, but you seem to primarily reference V Beam for redness.

      The Cutera ExcelV has a 532nm wavelength which targets redness. The Vbeam is 595nm which also targets redness and in my experience is more slightly more effective for redness, although both a good ;)

    HELP.. I am reall concerned, I got CO2 Laser treatment and my Doctor told me he does not use glooves in the eyes for protection because CO2 Lasers does not scatter. I had my upper eyelids and m full face without the under eyelids done. Now I see some praticioners do use protection even fro themselves during the treatment. Can you please tell me what is right? How can I recodnize if my eye sight has been damaged? After the traament I had a kind of blurred view, but it seemed just because my eyelids were very stretched from all the crust and dry blood over them. Could you please tell me if it is true that protection is not always recesarry and CO2 Lasers really do not scatter? Thank you!

    Hi Dr. Kappel, wondering what laser you use for spider veins on the legs. I had some treated with a laser by a doc but it hurt soooo bad. I mean I was holding my breath and it really was ouchie. I wanted to know how your patients hand the Pain? Also does it cause some scabs because I got a few bruises and scabs over some the areas.

      NEVER use transcutaneous lasers on the legs to treat vessels. It will scar. Vessels on the face can be treated but NOT on the legs. Did you scar. Sclero is the option for leg vessesl with the exception on endovenous laser done by vascular surgeons.

    I always get numbed before microneedling with PRP (just basic Microneedling not with RF). There are some instances where it makes sense.

      I got numbing for CO2 and it still hurts, I even got numbed for 30% salicylic acid peel. Some people have sensitive skin and low pain tolerance.

    Hi Dr. Stefani! I’ve read that red light therapy devices can be used for fat loss, while they are also used for building collagen. I’m apprehensive to use my red light therapy machine consistently on my face as I know fat pads contribute to a youthful and healthy face. I’d love to hear your thoughts!❤️

      I'd be careful because although red light has been shown to stimulate collagen, at home devices may not have the control to make sure the temperatures do not reach the subcutis or fat. Just proceed with caution and the fat you are thinking like a scientist/dermatologist is amazing. Also, red light is heat without a handpiece that cools the epidermis like Thermage has. So, unlike Thermage that does not cause hyperpigmentation, at home red light devices can cause melasma and PIH.

    I want to do my eyes. I'm 64 and have wrinkles. I had an upper blepheroplasty about 4 years ago, but my eyes are still hooded. I think he was very conservative. I would like to smooth out my upper and lower eyelids. I don't have dark spots or acne, large pores, or anything else. I'm fair skinned. I do use Skinceuticals A.G.E Eye Complex and Truth Treatments Vit C Balm over it at night, and it really helps, but I'd like big results. I have had lots of basal cell carcinoma removed because I was stationed in Florida and the South Pacific, plus I used to use a tanning bed. I was a sun worshipper. I use sunscreen daily now and wear gloves when I drive. I do have brown spots on my hands. But, I'm focused on my eyes because I love to wear makeup. I've asked you for recommendations on Dr's in the Annapolis area, but the doctor you gave me isn't doing procedures. He's teaching at Georgetown. Maybe I have to go to Baltimore but I can't believe there isn't someone in Annapolis or Columbia because the demand would be higher in those locations. I'm seeing my dermatologist this Tuesday for my bi-annual screening and will ask her, but they have a medspa, and I'm sure she'd recommend them. I don't know anything about them, and I don't think they are a good fit. I've never had botox or fillers, so I've got no experience with a medspa.

      @MyMaria1224 the first time I used the Skinceuticals A.G.E Eye Complex my lines softened. I use it in the morning on its own and at night with the Truth Treatments Vit C Balm as a sort of slugging and my eyes look so much smoother in the am when I wake up.

    Dr kappel! i’m saving up to make an appoint with you for a clear + brilliant treatment and I was going to ask my questions when I see you but i’m 21 years old and have very dark circles and sunken under eyes, i think it’s because i’m 100 pounds and don’t have a lot of fat on my body so my eyes look more sunken. i’m okay with it now, but i’m nervous for when I get even a couple years older they will become more sunken, is sculptra for my under eyes somthing I should be looking at getting for further prevention? I really don’t want to get tear trough filler, my mom had it and doesn’t like it. i’ve been looking at fraxel Co2 or vbeam for my under eyes as well for my dark circles, will any of those treatments stimulate collagen?

      There is not strong scientific data either way but would not thing that microcurrent can cause fat loss although I cannot say for sure. For anyone else reading this who uses microcurrent, have you seen any issues with volume loss? Please share.

      I unfortunately experienced facial fat loss from using an at home microcurrent device (Foreo Bear). I was quite shocked and upset because all sources said this was not a risk 😢.

      I have a NuFace mini that I've been using 8 months about 5 times a week and I don't feel there is fat loss with it.

      Micro current treatments shouldn’t cause fat loss. Micro current tones and lifts muscles, therefore the face will look more taught, sculpted and slimmer with more treatments. Kinda like working out. If you stop the micro current, the face will gradually goes back to how it was.
