I've got a pair of twisted Cartel Fantom limbs. We first noticed it some time after I'd given the limbs to my daughter for her bow and saw that the string wasn't sitting in the groove. No amount of adjusting or swapping the limbs between top and bottom would sort it. When we took the limbs off the riser it was bad enough that you can just visibly see the tip isn't straight when the limb isn't in the riser. Of course this was a day just before a tournament, got the bow as good as we could get it and she just had to move the string back after every shot. It actually worked as a good mental distraction and she smashed her PB by a country mile. Got her some WNS Axiom Alpha's which adjusted perfectly. She's still trying to better that PB.

I have these exact limbs and yes the top limb showed signs of an odd scraping sign that suggested off alignment... I did figure out eventually that it had to do with aligning them correctly, thank you for this as it has helped confirm the need for proper alignment checks
Just a minor thing those WNS c5 are actually carbon/foam. Doesn't change much though, still a foam core and have fibreglass on top of the carbon probably just makes it harder for them to get twisted. I also had a relative think his fibre/foam limb were twisted, we just put them in my bow and it was perfectly straight. Just a limb alignment issue.

I had one limb that twisted, but different case. It's manufacturer defect as i put on other riser. Found out limb tip grove on the left (the one that hold the Y loop) not aligned with right grove. I take chainsaw file and grind it gently until both grove even. Tested, no more twist. That linb has been sold to my archer team mate. That was 3 years ago. Until now he still using it and so far no twist and still good to shoot
Yes i done same stuff on cheap pair of "Soul Black Flash" and FC-100 limbs. One side of the grove was missing. 7 pairs of limbs from 4 different manufacturers and no "twisted limbtips"... just uncompleted string groves in the manufacturing.
I have seen multiple wood core and bamboo core came new twisted no matter how much time we spend to tune it, swapped handles,strings and check the limb tip. It just uneven after strung. Foam, barely. But there are times it's the handle itself, was lower end and old.
I heard improper seated limbs several times on the shooting line. A loud "crack" and the whole club turns their head with wideopen mouth thinking it was a dryfire. I think most archers has sometime forgot to pull the string a few times so limbs gets seated properly.

A reputable shop in Melbourne found our WNS limbs had a twist in them. When you eyeballed the profile it was unmistakeable.

I had an alignment issue on a Hoyt Satori 50lb, which turned out to be an adjustment issue. No problems after it was adjusted.

Could a misalignment problem be caused by improper stringing of the bow? Around your leg for example?
Yes If you string around your leg you risk twisting and breaking your bow. I assume everyone uses a bow stringer for correct alignment.
