Thanks for the help, tried everything and it's only let me get to a pre 11.52 state still requiring an update even after rebuilding the database. It no longer locks me at the error screen every time I turn it back on. Even from save mode via internet or the USB instructions followed on the ps4 website, it's been trouble. Maybe it's just stuck offline forever lol.
Update, I'd been playing around with trying steps in different order ect. I did the thing with the USB, and it was still giving me the same error, I tried doing safe mode installs and stuff, and got it to a point where it was functional if I didn't install the next system patch, which let me use it for streaming at least.
I took it apart and checked the cord the website suggested checking but it was fine, Still got the same error. So color me confused but today, maybe through updating other streaming applications or an update on Sony's end, it just worked. From the normal notifications menu even though it was failing before.
A maybe relevant mportant note is that when the install worked today, when it was done and restarting it told me it was rebuilding the database 1/2 for a couple seconds, (this is after the point where the install failed before.)
So I rebuilt my database from safe mode, tried the usb update and it didn't work. Got to the normal system screen and it wasn't working. I used it for streaming for a week and a half or two, just kinda leaving my system on as a charger and ignoring the available update. Other updates happened possible to specific applications. Then today I just tried it again, no error, and told me about rebuilding databases after the restart. Sorry I wish I kept a log of it or something and will in the future so I have a better record of what worked.

If your disc drive is faulty then the 11.50 update and onward will not work, the motherboard has to communicate with the Blue Ray disc drive, replacing it isn't so easy either as I believe the sisterboard on the BRDD is tied to the motherboard
Where did you hear that? My disc drive stopped working for no good reason a little while ago, so what you're saying checks out. So that means I can no longer download any of my purchased games or play online anymore, that's ridiculous.

Mine will only start in safe mode and requires a manual update with a USB. It asked for my ps4 to be formatted to Exfat and even after i reformatted it to exfat the ps4 is still saying that it need to be formatted to exfat. Wtf

I have literally done everything. There's nothing left to do and the most irritating thing is if I don't purchase another PS4 or a PS5 I lose years and years of games and data on my Seagate external
Hang in there, mine wasn't working either but just happened to work today. I did up to the database rebuilding and the USB update and it sitll didn't work a week or so ago.
I kept it on and just used it as a streaming device assuming a hard drive issue was the problem and I'd have to backup and replace it. Tried it today, just normal, from the notifications tab, and it just happened ti work. Best of luck!
All those options did not work for me same error! Luckily I made a backup first, just need to get it sorted first!

Same shit with my PS4 slim…rebuild database but when I go for the last part of the instructions and update via safe mode 😵💫🤬 I get very close to the end..the same message

Mine starts up trying to check the system storage. Stops at about 25, then says it cant start and asks me to reinitialize the ps4(delete my data). Rebuilding database doesnt work because it always tries to check system storage.
