NONSURGICAL BUCCAL FAT REMOVAL | MINIMALLY INVASIVE | Is it possible? Safe? Dr. Zelken says NO WAY!

  • Buccal fat pad removal is gaining a lot of popularity. It is surgery, though, and this scares away a lot of people both in terms of price and risk. Dependable plastic surgeons who perform this operation are few and far between, and the results of a successful operation are inherently subtle. Therefore, it is understandable that minimally invasive or even nonsurgical options for the same procedure would be very desirable. Especially if the procedure could be performed by non-physicians, such as nurses, physician assistants, and nurse practitioners. Some people are promoting the injection of Kybella, which is an injectable acid, to the fat pad. This is unsafe and unlikely to work. Another CZcams video shows a physician provider using radiofrequency assisted lipolysis to "remove" the buccal fat pad. In that video, the provider is simply treating the subcutaneous tissues overlying the approximate location of the buccal fat pad.
    Here's the story:
    Bichectomy, or buccal fat pad removal, tends to be relatively inexpensive and takes less time than most plastic surgical procedures. The operation can take as little as 10-15 minutes, and as long as 30 minutes. There is minimal blood loss for the procedure, and most patients choose to do this operation awake. Alternatively, injections of acid into this space would put a lot of important structures such as nerves, salivary structures, and muscles at unnecessary risk. Multiple treatments may be required, results would probably not be seen, and the series may cost more than surgery itself. For those of you who wonder whether or not Facetite or Accutite is an option, this video's for you.
    In this video, Dr. Zelken discusses the possibility of minimally invasive surgical buccal fat pad removal. He demonstrates the act of Facetite versus an intact, freshly removed buccal fat pad. While the treatment causes a one third reduction in the size of the fat pad, it is impossible to apply that type of energy on a living patient with the buccal fat pad in situ. Not only that, a minimally invasive procedure would entail that energy travels from deep to the buccal fat pad to the skin surface. Invariably, this would place motor nerves, sensory nerves, salivary structures, muscles, and skin at risk of injury. As it worth it? Is it possible? For the time being, absolutely not.

    Does the skin contract enough after the Buccal fat pad removal, or it becomes saggy due to having an empty space, like a deflated balloon?

    Hey doctor,
    How would a buccal fat removal affect a face when smiling? For instance, the beautiful Taylor swift doesn’t necessarily have “big cheeks” when she’s not smiling but just more a round face/shorter face-at least from my untrained eye. But how would a surgery, say on someone like Taylor Swift, affect the look of the face when smiling? Would the removal be more apparent? Right now she has a cute small smile, would it maybe look her smile look wider in comparison?
    I ask because I dislike how my cheeks look like only when smile, they’re asymmetrical and just wider than I’d prefer, could this be a good option for me? Or something else?

      You’d really need to be examined to answer this question but in many cases removing the fat pads is subtle at rest but helps with smile. Sometimes, during a smile, the pads are more apparent than at rest.

    I feel like non-surgical alternatives are almost never a better option. Another good example is it’s so much better to get a one time rhinoplasty from a good doctor rather than multiple injections every few months. It’s crazy the lengths ppl go to (probably with misconceptions) to avoid surgery just because they have some weird connotation surgery is bad. Like getting neck fat dissolved with a hundred injections instead of just getting liposuction.

      non surgical rhinoplasty is a cheaper option and also a way for patients to see the difference, the recovery time is also minimal. I am thinking about doing it however if i can just get filler to sort out my asymmetrical nose then i will try filler first.

    i have more facial fat on one side of the fact apposed to the other, would buccal fat removal be a good option for the bigger side to remove the fat? my face is asymmetrical and bigger on the one side

    If I get face thighting first because my cheeks are kind of loose but I do get some improvements some not enough can 6 mths after can I get it done again and also get the buccal fat removal..Example ;; if I see better improvement after healing 1 year later or 2 years if gain weight or get pregnant can I do face thighting again???

    How much time does it take to be in normal shape after removing buccal fat??

      Depends, but many people go back to work the next day and look normal “enough” by a week or so.

    Can Kybella injections target buccal fat pads? If not, are Kybella injections effective for subcutaneous cheek fat?

      This method doesn't work. I wasted money doing this. Only thing Kybella can do is reduce some fat in the face but not the buccal fat.

      I do not have experience with Vaser in the face but in general, energy-based treatments should not go deeper than subcutaneous fat to avoid collateral damage to nearby structures. I also favor bipolar energy devices because you can control the direction of energy transmission.
