Very nice guide, but your collectables section is a bit confusing. You don't really make clear where exactly some of the more well-hidden documents are. For example at 9:02 you say "over this fence, then drop down this window and into this door", but don't actually show what you are talking about. This happens a couple of times. Since many of these areas look very alike, this just tells me that a collectible is there but not really where to find it.

For some reason i played the game on nightmare and i didn't get the hard and nightmare achievement....is this because i died too many times?

If you ever went to chapter select and then loaded up a chapter that way is the only thing I can think of that might cause that. Dying a few times shouldn't affect those achievements.
