so even on smaller shirts you would still use the same left chest placement? wouldn't it look a little off with a smaller shirt

You are correct it would look off. I usually go off the end of the collar Stright down to the middle of the design. Make sure to give yourself 3" or three to four fingers from neck! Thanks for viewing.
