Thats like of the only real good rap songs. I wish they would have managed to make a good english version of it though. The whole song can basically be translated like this:
She shall not see me when I'm down.
All the curtains are there to be shut.
I feel like I'm more persons than Eckbo - (It's a norwegian comedian that often does parodies of several characters).
In the spotlight, I hate be looked at.
Every day is just nerved and drama.
But I still run in front of the camera and wave my arms.
Some things I regret, is this thing karma?
Because the devil on my shoulder is really enraged.
I struggle and struggle with that devil on the shoulder.
He whispers in my ear who is on control of this gig.
I can't do anything but to listen to my devil.
Where he works to destroy this kid.
      Fuck
Don't answer the phone or retrieve the mail.
Don't try the doorbell, my door is locked.
Soon jumping in the ocean, this is getting to rough.
All I have is time, but it heals no wounds.
Feels like I am in hell.
The devil on my shoulder has become my closest friend.
      On my shoulder reminding me.
My fucking life is going in the wrong direction.
Is it any wonder why I am afraid?
When the devil on my shoulder is who talk to the most?
On my shoulder telling me my life is going nowhere.
Like it's a burden enough to carry.
On my shoulder is a cheeky little bastard.
Telling me that I'm far from the guy I should be.
I should look down the barrel of a rifle.
Some lessons in life I should learn.
Me and other people can't share the same air.
The devil on the shoulder har something worse in waiting.
I recommend you to not be close.
The anxiety is too too motherfucking real.
All I can do is to of drugs and drinks.
Everyone blames me and all I can do is to deny.
The devil on my shoulder makes sure to break me down.
My nightmares are so ugly and real.
I start every day in a bathtub of sweat.
My friends won't see me no more.
My own devil won't see me deliver.
      So
Feels like I am in hell.
The devil on my shoulder has become my closest friend.
      On my shoulder reminding me.
My fucking life is going in the wrong direction.
Is it any wonder why I am afraid?
When the devil on my shoulder is the one I talk to the most?
On my shoulder telling me my life is going nowhere.
      Not going anywhere in life.
      Not going anywhere in life.
      Not going anywhere in life.
      Not getting anywhere.
      Not going anywhere in life.
      Not going anywhere in life.
      Not going anywhere in life.
      OnklP, you won't escape alive.

The song with the "storyline" as you called it is called Styggen på ryggen - The ugly on your back is the direct translation. But a more corect translation would be the devil on your shoulder as you said. Its a song about the struggle of dealing with substinance abuse and the hardship of trying to get back to normal when the "devil on your shoulder" is trying to pull you back in and tripping you up every step you take in the right direction. And all the day to day strugles you deal with when you try to get clean as a drug abuser. In my opinion it's a really good song and hit quite close to home as i have a brother that's been an addict for 20 + years. My brother also knows the guy singing this song and they have been in the same "social cirles" for years.

The last one... You are spot on with the devil on his shoulder. That is basically the title of the song "ugly on my back" directly translated, but yea. What makes it special is the lyrics and that the fact that Onkel P is a household name in norwegian rap, who evolved and now do more things with a rock band as you can see and that the guy playing "ugly" is Atle Antonsen, a giant in norwegian comedy, theatre, radio and social commentary. Onkel P has struggled with drugs and some shit, so it’s a personal thing. I get why you can’t relate though, not knowing him and so on.

Styggen på ryggen is a song about anxiety and depression, the lyrics is one of the best Norwegian rap lyrics ever created. "Styggen på ryggen" can be translated to"the devil on your shoulder"
    Feels like I'm in hell
the devil on mye shoulder is now one of my closest
On my shoulder and reminds me of
how fucking terrible my life has gone
Is it weird that I'm afraid
When the devil on my shoulder is the one I'm talking with the most
He's on my shoulder and tells me "you wont get anywhere in this life"
The long part where he repeats "kommer ingen vei her i live"(getting nowhere in this life) ends with:
"OnklP, you wont get out of this alive"
