    Great video Doc, but I have a question .
    Bone distraction means "the process of generating new bone in a gap between two bone segments in response to the application of graduated tensile stress across the bone gap".
    In all cases of Le Fort I, that of BSSO, Sarpe and MSDO we can speak of bone distraction, or am I wrong? You talked about bone distraction specifically only in the part relating to Sarpe and MSDO, and I was wondering if it was correct to use the term in the other cases.
    THANKS IN ADVANCE DOC !

    You are truely amazing making all of these videos. I’m already part of your patron community. I have been watching all your ortho series videos but I have noticing when you are talking in occasions you said “here” and “there”. I can tell you probably are using your mouse to indicate what you are pointing (Ex 6:24min of this vídeo but I can’t see what you are pointing). Only when you use your light blue marker or when you paint it, it’s when I can follow what you are referring. Otherwise, your mouse doesn’t show what you are pointing. I can see you format of your slides has changed. The presentation is different than your old videos for example pathology series. Again you are incredible I deeply appreciate your work. You make my studies easier. Most of dental community are using your videos as main guide to study for NBDE2. You should feel very proud Dr. I wanted to let you know in case nobody has said this to you. Thanks a lot for what you do again!! 😊👍

      Hi JR! Yes I realized that for whatever reason, the video capture software doesn’t actually capture the paintbrush when it’s not in use, so I am definitely guilty of pointing to things that you can’t see on screen in the last few videos. Going forward I am now aware of that and have found a workaround for it! Thanks for pointing that out though.

    Just a little not, the maxilla during a LefortI cannot be setback due to interferences and is never done!
    Thanks a lot.

    • @mentaldental
      @mentaldental  Před 3 lety +7

      Yes, you are correct! I should have specified the only way to "correct" Class II with maxillary surgery is a segmental osteotomy setting back the premaxilla only. A maxillary "setback" is never done, thank you for pointing that out!

    Great video doctor! Thank you for everything you do.
    I am a little confused in something.
    Why to correct an open bite you do maxillary superior repositioning? Wouldn’t that open more the bite? And the same for the maxillary inferior reposition, wouldn’t that bring the upper incisors to cover even more the deep bite? I am so confused , please heeeelp anyone. Thank you!

    • @mentaldental
      @mentaldental  Před 3 lety +16

      So this involves a concept known as autorotation of the mandible. As you impact or position the maxilla superiorly, there is space between the posterior teeth and the mandible is allowed to rotate closed. Closing 1mm of posterior open bite space in this way results in about 3mm of closing of the bite anteriorly due to the geometry of the mandible. The opposite is true for positioning the maxilla inferiorly, which effectively "jacks open" the mandible. This is a difficult concept to explain over text, but hopefully that helps!

    dr. in the envelop of discrepancy I think there is wrong in first one retraction and protraction numbers is opposite according to diagram even in extrusion and intrusion in lower incisor and upper

    First of all, thank you so much for making this great videos for us. Truly outstanding! Just one thing regarding this video is, “Envelope of Growth Movement”, I think mandibular extrusion and Intrusion should be the opposite according to the diagram presented. Would you please double check? Thank you.

  • @jamesferone9021
    @jamesferone9021 Před 3 lety

    Regarding 9:00. I don't understand. Isn't a movement of 7mm back with orthodontics on the incisor likely to move it out of the maxillary bone?

    • @mentaldental
      @mentaldental  Před 3 lety

      There is certainly variation from patient to patient, but on average, maxillary incisors can be retracted 7mm thanks to the labiolingual thickness of the maxillary basal bone. However, realistically, you are at risk of pushing roots against the palatal cortical plate which is notorious for causing root resorption.
