For the very latest in acne scar revision procedures
The very first step in acne scar treatment is to determine your scar type. Based upon scar types, I then choose the best method to remove that acne scar. For example-
Ice pick scars are best treated with chemical peels such as TCA CROSS or punch excision
Rolling scars are best treated with fractional lasers Fraxel, INFINI Microneedling, Dermarollers, skin needling and subscision
Box car scars are best treated with laser resurfacing using a co2 or erbium laser such as Profractional
Atrophic scars are best treated with fillers such as Juvederm, Restylane, Bellafill, or even fat transfer
Most clinics will by default ‘prescribe’ a laser treatment such as Fraxel, fractional laser, or have an answer such as Derma Roller or microneedling. Whilst these treatments can give some results, they do not offer Gold Standard acne scar treatment solutions.
A bespoke or individualised treatment approach is best, because it is your scars that are being treated and not just a device that is being used for ‘acne scarring.’ If you look at my acne scar before and afters you will notice one trend- everyone will have a mixture of scars and very rarely will a person have only one scar type. Most patients will present with a variety of scars, some patients will have more atrophic acne scarring, whilst others will have deep ice pick scars. Some patients will present with acne -pimples- zits and acne scars as well. The combinations are endless.
So the very first step I do is to ask- what types of acne scars do you have? What gives me the most information is angled lighting. This is how you visualise scars, and it is the same method I use to find acne scars, map them out, and then treat them. Most patients notice their acne scars when lighting conditions changes, classic examples are when they are in the bathroom, or in a dimly lit room, or when light shines at a certain angle. It might be at night in the car, or the reflection of glass or a mirror. I use the same principles that patients see their scars to examine, identify and treat acne scarring.
Any specialist who does scar revision to a high level would use angled lighting to determine scar types. Shining direct and bright surgical lighting reveals very little, but by dimming lights and shining light at different angles reveals a whole new world of acne scarring. In many of my upcoming videos you will see me using these very techniques to find, treat and cure acne scars. I use light to my advantage.
In the majority of my acne scar treatments I combine a variety of techniques in the one sitting. Early scars respond well to eMatrix RF, whilst late scars respond to TCA CROSS, fractional laser skin resurfacing, fractional microneedling, microneedling RF, subcision, dermal grafting, dermal fillers (Juvederm and Bellafill) as well as many other acne scar revision methods.

dr David, i did 2 session of glycolic peel from a dermatologist but didn't really see any result. he has told to do 2 more. last he used 35 % glycolic acid. let me tell you, i dont have acne but my marks are all spreaded over my face which are not noticeable much. but in natural light and it seems like my face is covered with dark spots. what do you suggest will i see any noticeable results after 2 more glycolic peel? and 1 more thing my skin is really tough and thick. please help

I research for decades about how to get rid of acne scars of different types that I have in my face. Ive struggled with acne in my teens and back then I was always so self conscious at anyone looking at it or making comments on it which made me so down and embarrassed that I feared seeing the specialist for help. I delayed seeing a dermatologist well into my mid twenties (which I was then prescribed Roaccutane, it helped clear the bad cyst type acne). I had asked about acne scarring treatments and the response the dermatologist gave me was "we will talk about it later" even after I told him twice that I wanted to know if there was any kind of treatments out there. Because of his reply, it made me feel that there were no treatments out there for acne scarring removal. I should have found another specialist to see and speak to.
Now years later, though I don't pick at my skin I don't know why new acne scars always seem to keep appearing. Why does this happen? It does depress me as I wish I had good skin. Can you tell me if any of the acne scarring treatments covered by Medicare?
I would love to see you for a consultation though I am based in Sydney. Do you have a sister office in NSW?

It’s so rare to find a doctor that specializes in acne scar revision. Most doctors don’t know dittly about it and will just recommend lasers.
I have been looking for years for proper treatment for my daughters acne scarring (she developed cystic acne at 14, tried antibiotics and even accutane- which lasted as long as she was on it)..and have not felt positive about any office/procedure enough to begin treatment. This video is very informative. Do you have any referrals in the state of Florida? Most offices I am researching deal primarily with anti aging and not specifically with acne, and probably don't analyze and take the considerations you do to determine treatment. You must really be changing lives...living with acne scars is a horrible thing. Thank you for this. It has given me useful information.

I called his office weeks ago! about 4 hours away!. We did get some CO2 for her but I did not see any improvement and he had her 10 days on antibiotics after which is not a good thing for acne sufferers since it causes issues. I did got o someone Dr weiner recommended but it seems that INFINITY alone doesn't do that much for the scarring. More for tightening the skin which makes the scars "appear" less. I did go to Dr Pozner in Boca who uses the INFINITY.(Or is it INFINI?)He also wanted almost 5,000.00 for one treatment. It is a huge gamble so we went with CO2. No one that I have spoken to in Florida uses CO2 PLUS Infini which is what you seem to recommend, and not as invasive. Just a few days of redness. We have to decide whether we will do the Co2 again. I dont like the 10 days of antibiotics (oh and also an antifungal) afterwards. It's not easy...
This is incredible. I've been battling these scars for years now so I hope we'll have these treatments in the Philippines.

Thanks for putting this video out there, Dr Lim - very educational and gives me hope for my skin! I've been an acne sufferer for about 15 years and have a mixture of icepick and rolling scars as well as loss of tone on my Fitzpatrick type 4 skin. I've been at my wit's end trying to find a knowledgeable derm who can perform an intelligent analysis like you did and give me a good treatment plan instead of the old "let's Fraxel it and see what happens" approach. I know you practice in Australia but is there any chance you can recommend a good knowledgeable derm / plastic surgeon in the USA (even better in the midwest area around Chicago IL or Milwaukee WI)?
+Jezebel James Thanks for your comment. There are some really good dermatologists in the US- the best for acne scar revision would be Dr Chris Zachary or Dr Sue Kilmer- both I ve learned from. And my "very smart" dermatologist says nothing really works with my mild scars. He only operated Photo Dynamic Therapy which burned a hole in my pocket and did absolutely nothing, then bombarded me with light chemical peels that also don't do the slightest of a progress (because they only target the surface and discoloration not the pits in the epidermis) with brightening serums.Thank you, Dr. Lim for the very informative video. You gave me hope.

I have ice pick scars on my cheeks and have watched your video with the tca cross although I do not have lots of scars but they are enough to want to try to improve their appearance so I would like to try myself the procedure you recommend. How long do you leave the tca on the scars and how often can I repeat the procedure? . I have got the tca acid along with neutralising gel and the moisture cream. Your procedure looks just like what I need for my scars.
Best advice, - see a Specialist, this is not a DIY home procedure, the concentration of acid is so strong it can either worsen scars (wrongly treated or identified scars) or cause third degree burns. I love your methods. You are a Godsend. If only I can travel or move to Aus I would love to be a patient. Is there anyone in the States that can do these methods as well. I've tried looking but I don't think any derms do subcision, not sure y. Or I just haven't found one that will. I have some tethered n rolling scars that if like to fix.

The most informative video on acne scars I ever watched! Thanks Dr Lim. I wish you're in Singapore... The indirect lighting technique that you used to see what type of scar a person has, is the best method to find out. Most of the aesthetic doctors here do not do that, they just based on the direct lighting. That's why there were no results cos they recommended us the wrong lasers and then we are the ones ended up wasting our money!
The best doctor who I also learned from in singapore is Prof CL Goh. See him, and your scars will be much better. He is a genius.

Most patients don't realise that if you have multiple scar types ONE visit won't do, its like orthodontic work...many treatments over 6-12 months to get the BEST results. The only exception is with shallow box car scars ...one session of laser and its done.

I'm from Vietnam, I had a laser treatment (co2 fractional) 6 months ago but the result makes me too disapointed, ances scars remain unchanged although it took me over a month to get rid of severe sweling and redness. Is it possible to have a second treatment doctor? I'm afraid my skin becomes weaker and will be unable to tolarate any other type of treatment.

by far the best video i have watched on the subject. i have recently done an EndyMed 3deep fractional resurfacing radio frequency microneedle. there wasn't much pain honestly, downtime wasn't too bad, it was a week, and i have noticed a lot of improvement. i have large pores, pitted marks, boxed marks, and the redness. my forehead and chin are clear, i only got this on my cheeks and sides of my jaw. i dont have any bumps, and it seems the 3deep treatment has made my collagen thicker. she suggests 2 more treatments. i hope it looks as nice as what you have done!

How do you threat hypertrophic scars on the nose?
+dodooow If they are truely hypertrophic, I use CO2 Fractional and steroid drip, or straight forward injections. If they are soft, these area actually atrophic- some patients and doctors think they are hypertrophic just because they are raised. in this case I use CO2 fractional combined with INFINI RF micro needling.

Don't let it tear you down, it CAN get better! There are soo many things out there nowadays. Try derma rolling and then afterwards apply beyond cp serum from hollywoodbeautysecrets.com. You can even call and speak to Louisa directly, tell her your concerns and see if she thinks what I said will help:)

I live in Singapore where dermatologists push fractional co2 as the gold standard for all acne scarring. So sad to have no access to a true acne scar revision expert.
+Jes Jos Yes, I also learned my trade in Singapore under Prof CL Goh who taught me CO2 and erbium videos. I did one video on TCA and erbium combination. In some cases, lasers do better that other devices, it really depends on the scar type. I spent a lot of time at NSC (National Skin Centre) 13 years ago, and still rate it as one of the best dermatology centres in the world . Dr Davin Lim

How informative! Thank you for all of this. I had a procedure done that is similar to the Spectra Carbon Laser peel and it worked tremendously to help control my acne! Now that I no longer have active acne anymore, I'm just left with scarring. Your informational videos are really helpful and will let me know the next time I visit a dermatologist! Sadly, I'm in the States, so I can't visit your clinic! Hahaha. Cheers!

Specific... 60-70% of my work is on scarring, I don't even see acne anymore, and stopped rashes 6 years ago. It is a very niche field. Dr Philip Bekhor and Assoc Prof Greg Goodman in Melbourne are great, and taught me some of my skills, probably closer to you as well. Davin Lim

I wish lived in Australia so I could see you! A lot of doctors I have encountered do not take the approach you do and therefore results are minimal compared to what they could be. If you know any doctors who take a similar approach that reside in Colorado, U.S.A that would be wonderful! :) If anyone reads this and knows a great doctor in Colorado for acne scarring, please let me know!

For the whole video i was like "Okay he's explaining well,but i want to see the results with angled lighting otherwise i won't trust even him" and then at the end of the video you putted them!! I have a question about the fillers, are they something like botulin? I mean that i won't like to have some kind of plastic or silicon under my skin like berlusconi or others mostly because i'm sure that botulin after some time needs to be refilled otherwise it looks worse than before. By the way i've afraid of the fact that these treatments initially improves the skin but that after some years it ruins the skin. Can someone clear my doubts?

+Zeno Raiser OK, angled lighting changes- patient in one position, but need to move that patient with fixed light to get my shadows, then I move the light to get the other scars. So, now we have two points that needs to be fixed at the correct angle.
Patient- no problem, we have a system that can lock down. Light- big problem. No system that can give the exact 3D angle to a fixed location. Once we have that, then the real results are seen. Better still, I hand the mirror to the patient and they tell the story... thats part 2! Can't hide the truth from that one.

+Reaper Drakulis Once again- depends on your scar type. I have 3 Fraxel lasers but honestly, unless a patient INSISTS on Fraxel, I do believe there are far, far, far better devices. Fraxel is like a tool, good for lots of things but not the best for acne scars.
