- Autoimmunity causing B12 deficiency.
Megaloblastic anemia is a form of anemia characterized by very large red blood cells and a decrease in the number of those cells.
1. Acquired deficiency in B12 and Folic Acid/Folate.
**B12 depletion and/or absorption issues with Metformin, Colchicine, PPI, Histamine 2 Receptor antagonist and alcohol.
**Vegans and vegetarians.
Types of B12
Natural forms: methylcobalamin, hydroxycobalamin, adenoslycobalamin
Synthetic form: cyanocobalamin
2. Pernicious anemia is a type of megaloblastic anemia. Autoimmune conditions related to intrinsic factor antibody, parietal cell antibody and receptors for intrinsic factor.
Intrinsic factor and HCL are produced by the parietal cells
Chief Cells produce pepsinogen and pepsinogen is converted to pepsin in the presence of HCL.
B12 can be absorbed by diffusion when there are adequate amounts of B12. Or they can be transported by intrinsic factor to the ileum for absorption.
Different B12 delivery methods for Pernicious Anemia
B12 intramuscular injections
Sublingual B12
Transdermal B12
Testing:
Parietal Cell antibody
Intrinsic Factor antibody
MCV
Homocysteine
Methylmalonic acid

Hi l have been diagnosed with sceloderma, cant take any medications due to severe allergies, allergic to all antibiotics, also have internal bleeding, l use hollistic practice. Could you give me insight on this please doctors have told me they cant help. also have masses growinh in stomach.
I am pretty sure that unlike so many performers on CZcams, he ACTUALLY knows what he’s talking about.
Too kind. Best, DrJin
After watching several of your video I have to say that with the whiteboard and your clear explanations, each topic is easy to understand and very informative 😁 Thank you 👍
Great to hear!
Please keep making these great videos. I’ve been helped over and over. I’m celiac, diagnosed at an advanced age, AND a COVID long haul survivor. Your info has made a huge difference in my life. Two solid thumbs up!
I have been dealing with auto immune issues and B12 deficiency for almost two decades and this is the first time I have heard some of this. Thank you!
Finally, a real doctor! Thank you! Subscribed!
Very informative! Thank you!
When Dr. Sung claps I am all smiles and locked in to learn. It’s always good. 😊✨
I have listened to many videos on this and you have by far explained this the best very clearly. thank you so much.
Great video as always. Very timely too; I've been dealing with GI issues (I suspect colitis) for the last 15 years and I just started supplementing with a small amount of cyanocobalamin / pyridoxine / folate a couple of days ago. I don't want to get too ahead of myself, but wow, I noticed a difference within a day. Hopefully I can stay on the healthy side with this new insight!
Fantastically clear explanation, thanks Doc!
Excellent video!
Thank you very much . . This coverage is thorough and very useful 🌷❤️🙏👍🏼
Awesome video. So clear.👍
Your videos are so informative and helpful. On a slightly related note, I’d love for you to discuss the MTHFR gene mutation. If you take requests 😊
Yes. Same here.

AWSOME VIDEOS THANK YOU !!!!
This is so helpful, thank you.
Thank you so much for your explanation 🙏🏼
Thanks, explained expertly and easy to understand
Thank you so much for your knowledge and explanations where we can understand everything! Been anemic for years and doctors always give you iron. I started the liquid b-12 a few months ago and I can tell the difference when I don't take the b-12.
Wow! Thank you! You are an "ace fighter pilot" in your communication abilities & knowledge. If I could give you a hug in appreciation, I would do it in a heartbeat. We are each born with certain God-given abilities. You found your calling.
Thank you so much for the video! Very informative. I have a family history of spherocytosis (which causes a folic acid deficiency), and always thought I had it. But with spherocytosis the MCV are normal, whereas my MCV is around 111 (so large cell). I've had my folate and B12 tested, but after seeing this video I am wondering if they need to test the intrinsic factor and parietal cells, especially where I have always had digestive issues. Thank you so much!
Thanks doc. You mentioned MMA test, but not a threshold. What does it need to be elevated to for you to be concerned?
Thanks!
Thank you for watching. Much appreciated
Thank you I have many autoimmune diseases and have been having major problems with my nervous system. I did have two strokes seem on mri but in the past year I have been having increased neuropathy with a rapid decline and was told there was nothing that could be done for it. But after researching and finding this video I started on b12 sl with rapid improvement but now how do I get my primary or neurologist to understand that I probably have an autoimmune condition that is inhibiting b12 uptake? Thank you I feel you have helped me from becoming an wheelchair bound invalid.
Never knew about b12 and Iron deficiency! I know some Vegans with this issue .
I have Pernicious Anaemia with gastric parietal cell antibodies, so I have B12 injections 8 weekly and iron infusions PRN (but I need to have Iron sucrose, not Ferric carboxymaltose, because it caused me to have the adverse event FGF23-mediated severe hypophosphataemia, resulting in a phosphate of 0.16 and FGF23 of 560).
I have several other autoimmune diseases: PL-7 antibody positive Antisynthetase Syndrome (which includes Polymyositis), Relapsing Polychondritis, anti-ganglioside GM1 antibody positive neuropathy (unspecified), and fortunately autoimmune progesterone dermatitis is in remission. I have cold agglutins antibodies but normal Coombs & haptoglobin & no signs of haemolysis so the antibodies don't currently have clinical significance (i.e. I don't have Cold Agglutinins Disease, a rare form of autoimmune haemolytic anaemia), but we continue to monitor.
Am curious possible causes for high in ref (labs keep raising ref range) for MCV , but high in B12 labs, feel better taking B12 but ok MMA and homocysteine labs. Guessing another low nutrient
Dr. My stomach doesn’t absorb the B 12, do you recommend ..
Do you know what causes Geographic Tongue?
You should cross reference this with the effect EMF has on red blood cells.
So would it be helpful (if not necessary )to take digestive enzymes or similar if you are not producing HCL???
Are subcutaneous injections of b12 as effective as IM injections?
Help.... allergic to all B vitamins... have allergies to foods, outside allergies including sun and Medicine adverse side effects. I am also anemic. What's going on? I do take Zyrtec for daily allergies
Hi dr can we reverse this condition
Could untreated coeliac disease also cause a B12/folic acid deficiency (poor absorption in the small intestine)?
Can you explain large red blood cells but small ? I’m confused
spleen Staphylococcus aureus (SFD)????
Sir autoimmune gastritis can be diagnosed by doing ana test?
Hi,
Great video! can you inject b12 into the fat? Thanks Julie
B12 injections are available but needs to got through a provider or clinic.
What should your mcv be?
Mine is around 92? I was deficient lst year-
The range in my blood test says 80-95, but he said 90-92 is “elevated” and points to deficiency. Mine is 91 and, along with high Anti Parietal Cell Antibodies being high, a hematologist just diagnosed me with PA.
This is very important. A long term vegetarian diet caused a critically low amount of B12 for me. Health problems arose because of this. I am now on a pescatarian diet. You need regular blood tests if you are vegetarian or vegan. Take B12 supplements at least or eat a little oily fish.
There isn’t as much b12 in the soil since it’s a microorganism. Any water with chlorine will kill the organism and other results of living in a more sanitary world.
Even cows are b12 deficient (they get it from eating some dirt with their grass), so it’s not uncommon to inject b12 into cattle so the meat will still contain some b12.
I prefer skipping the meat and taking a supplement.
But I also became b12 deficient when I used to eat meat due to some absorption issue…so I needed injections and other treatment.
Thankfully I can take a specific form (adeno/hydrox) of oral b12 and it’s much easier to manage.
Hi can all this cause infertility? Or recurrent miscarriages?
Possibly, but it's more complicated. I did a series on infertility. czcams.com/play/PLazgOvvJ4Pf8RcL2F1tK68_pl2yw8fZrx.html
My MCV is 79... Am I B12 deficient?
Your MCV is below the risk but you need other tells to prove everything out.
I've been told twice I have large red blood cells, but neither Doc investigated it . Are there any symptoms for parietal cells being attacked ?
Same here, infact its never even been mentioned, its just on my medical records and I've seen as high as 102. Just recently started taking b vits after a whole host of symptoms pointed towards deficiency.
@@baileysice2715 Now recently one Doc i saying I have too many red blood cells. Wants me to have a phlebotomy. I' not sure I want to do this.
@@carolelerman9686 oh wow?? Its so difficult l no longer trust any of them. I began getting tingling in my hands and feet and I'm not diabetic, countless blood tests and nothing. Eventually l find out about b12 l start taking it and felt better almost instantly, by this time drs treated me as some kind of hypochodriac! I think you should research as much as you can, ask questions and have them spell everything out to you.
@@baileysice2715 You know Baileys I've given up. I figure what ever gets me first.
