      @@zirzmokealot4600 is the whole conversation out? doesn't look like derek asked for an explanation in this clip, just conclusions. i think he's laughing to be polite instead of aggressive. derek wasn't even giving input he was literally asking questions. "even if x?" "even if y?" "So say xyz and then abc, would you still?" Derek didn't even make any direct statements, although he was pushy which I like, i think you're taking it out of context and attributing malice where there is none. Paul is used to being doubted on his claims but I've seen him back up a lot of his stuff when he has the time to.

      @@zirzmokealot4600 I think he just recognizes that point of view. I agree we have different viewpoints, don't really need to clarify that. I'm just trying to add some context and defend the guy from allegations based on your interpretation of his "type" of laughter, based on more concrete evidence and context. If you don't like him that's fine I just don't think it's right to cast aspersions based on your bias and a selectively cropped clip. How much are you going to read into a giggle? If you've seen Paul's context you can see that he's very willing to talk about things and have his mind changed. I think he's probably laughing because he's talked about that exact topic a million times and can explain himself but Derek is machine gunning question after question without asking why. It's like Derek is trying to get Paul to say he would use medication in one extreme example, but Paul wants to expand on the opposite point, that the medications are likely not as effective as it seems and has more downsides than meets the eyes. I like both of these guys, clearly different points of view. It's immature to look at a clip like this and make such a broad statement about someone's character because you think you're some kind of body language expert, when you can *actually* find out what he thinks by listening to what he *actually* says instead of trying to psychoanalyze him based on a clip from your favorite youtuber.

    As a board certified family medicine doctor, I'm almost always the first and only guy to prescribe treatments to patients with hyperlipidemia and/or insulin resistance. I can tell you with utter certainty that Paul could not hang in a conversation involving other doctors. Derek does as good as anyone could considering he's not a doctor (although he's a bit tied up in the rarities and very minor details, e.g., ApoB and LPa). Regardless, it is patently obvious that in a room full of doctors Paul would sound like a baboon. He answers hypothetical questions by asking for hypothetical specifics... or he just talks about himself? An informed doc would cite nuances of data, standards, exceptions, NNT and NNH, forks in the decision-making algorithm, etc. They wouldn't be talking about their own LDL levels.

      Yeah this is nonsense- Paul has debated countless MDs and “experts,” they almost always get destroyed by Dr. Saladino when he has prepared studies and asks them about their studies they reference which are almost always not well outlined.
      Also the fact that you hold such a strong bias in your statement without even looking into the above fact just goes to show how lazy and/or ill informed most doctors are these days at keeping up with understanding wellness.
      A large majority doctors get a degree, do little education after and are heavily influenced by pharma to treat symptoms and process their clients, the system is broken and though many are good intentioned it’s backwards and the results ultimately speak for themselves with how sick America is and how many drugs we prescribe people.
      Ironically Paul might sound out there, but there’s a reason 80% of the population is America is sick and a lot of docs just don’t have the resources or incentives to treat their root cause underlying health conditions.

    LDL does not directly cause atherosclerosis. 2 obvious reasons why:
    1. Atherosclerosis occurs soley in arteries, despite the fact that the veins carry the same LDL as the arteries and we don't get atherosclerosis in veins.
    2. Atherosclerotic plaques occur in very predictable regions in the arterial tree (near bifurcations/branches). If LDL was causal, it would develop uniformly across the arterial surface and it doesn’t.

      Statins have some exceptionally bad side effects. They should be used at the absolute last resort. This love affair with statins is gonna make a lot of people look dumb as more research comes out.

    it's probably better to hedge against atherosclerosis with at least a PCSK9i or something else with low sides. After all, most good data we have suggests high LDL is linked to heart disease.

      Correct. But Paul’s shtick is to talk about mechanisms as though they are the answer. Mechanisms mean nothing compared to outcomes. When you look at HRCT the information is fairly clear on this subject.

      I've seen a meta-analysis about statins that shows low doses have almost the same effect as mega doses. That's another way to hedge against ASCVD with low sides, if the $450/month repatha is not something you can afford or have covered by insurance.

      I’m with Paul on this one tho, at least on statins.
      The issue is statins do far more harm than good. I’ve known many ppl who’ve been prescribed them bc they didn’t take care of themselves, so they were put on statins, then ended up with far worse issues.
      Prevention is EASY-diet, exercise, not smoking etc. The listed CAUSES are the accumulation of saturated fats, smoking, high blood pressure and diabetes.
      If you don’t take care of yourself and you look like a hippo, then eventually the doc will put you on them. They’re a drug that’s extremely overprescribed in the South.

I am in the exact camp as what was being described in this video: High apob (125), HIgh LDL, low HDL, very low triglyceride, good thyroid function, metabolically healthy, very health lifestyle.

      It gets views. Derek is subtly showing the guy is a quack. 10mg of atorvastatin could be the difference in having an ascvd or not and it's virtually harmless. Sure there's rare cases a low dose could cause something but the benefit far exceeds the risk. That's what Derek is saying subtly. If he blantly calls the guy out he won't go on the podcast.

    I want to look into it more but I think metformin is one of these drugs that fall into this category that is being explored further as more of a prophylactic drug before the onset of diabetes. As far as lipid lowering drugs go I don’t think it makes sense unless they have a high ascvd risk score.

      Mine was 244 last time it was checked, and my doctor wanted to put me on a statin medication which I declined. I was 39 y.o. (now 40), in good shape, eat similarly to Paul Saladino, and weight train regularly. With all of the conflicting advice in the world, it's difficult to know what to do, but my gut tells me taking cholesterol medication in my situation doesn't seem correct. Edit:I should add mine was done non-fasted.

    I can understand not wanting to take statin, but some people may benefit from them depending on what their situation is. Everyone is DIFFERENT. It's a choice the patient must make if their cholesterol situation is really bad, but, who knows. We are all going to die and nothing actually matters in the sense that people think anyway, so enjoy your day.

    Statins suck. If there were anything other than that which can modulate lipids or other functions better then it should be an option. Paul doesn't seem to think there are any other options than statins. Though I agree in ways that no pharmacology is better than none in a healthy individual to hedge any genetic bets.

    What the hell does the low carb community have against statins? The side effects are so little, and benefits of avoiding sudden death so great. I believed this crap for ten years, I hope I didn't accumulate too much plaque.

      Idk if you have ever taken a statin but it makes you feel like absolute crap everyday. Not saying you shouldn’t take it but it definitely comes with side effects

      I've been taking atorvistatin for years. Before that simvistatin. I've never noticed any side effects. I should say that I only sporadically took the simvistatin. I was very much into low carb and later keto (I've been a type 2 diabetic since about 2004, when I was about 40). I pretty much bought the party line about LDL, saturated fats and statins. I only looked into the actual evidence a couple of years ago, and completely changed my mind.

    Im not totally against statins, but I will say that my grandfather has been on statins since they were invented. He is now 80 with end stage liver disease and he never drank alcohol.

      @@Lucas_Jeffrey I think it makes little sense to take a drug knowing that it will ruin your liver. "Don't have a heart attack, die of liver failure instead" because when you have 7% remaining hepatic function it does indeed look a bit silly.

    Paul is so far ahead of the game. This was great to see. More and more evidence coming out that will prove him right. If you are healthy ie metabolically fit you dont need to worry about cholesterol.

      what about if you're healthy and metabolically fit and a CT angiogram shows your arteries filling up with calcified plaque, even though your lipids are 'normal'? Genetics, bro. Everyone is different.

      @@_________9996 Like Paul said, the best metric would be checking your fasting insulin. There are other bloodwork markers you can check as well but if you follow a animal based diet without processed foods and exercise, get sunlight, good sleep, etc, you are on the right track.

      @CloudCrash absolutely false, I'm not even a vegan and I eat animal products myself. But I know if I ever have any heart issues I'll stop eating them because the research is pretty blatantly obvious that abstaining from meat will reduce all risk of ischemic heart disease.
