  • Paul explains why he does not believe that elevated LDL is an issue if you are insulin sensitive. He counters the point often made by Western Medicine that we need to have our LDL as low as possible and if it is high, that we should take a Statin. He touches on what he believes is causing damage to the endothelium of our arteries and when that damage becomes an issue.

    One of the main causes of for an elevated LDL-cholesterol is lowered thyroid function, which is why it is often observed in people following keto, carnivore or fasting. A normal thyroid function increases the amount of cholesterol that tissues are able to take in and in turn it turns that cholesterol into steroid hormones.
    In cases of an infection, the elevated cholesterol plays a protective role.

    My doctor is trying to get me on cholesterol meds because I eat a lot of eggs. I told him I don't care what he says Im not taking cholesterol meds.

      Modern medicine is focused on medicating the symptoms, instead of fixing the problem permanently.
      Just stop/lower your eggs and saturated fat consumption, and you wont need to buy statins for the rest of your life or get mutilated by heart attacks, vascular dementia and brain infarcts.
      This is not a joke anymore, think of the pain and misery that would cause to your family and loved ones who care about you. Just test it out for couple of months - you have nothing to lose, but everything to gain. Good luck!

    I'd like to share my own theory, ldl particles are responding to the damaged endothelial but their apoB receptor is damaged from glycation and oxydation and they are stuck. Ldl with a working key are being absorbed in the endothelial wall and used for repair.
    High blood pressure, mechanical damage to the endothelial, moreover damage to the glycocalix from high sugar and wheat proteins. The issue is the pattern Beta ldl which was damaged, ldl pattern Alpha are functional and they work normally.
    There is a ldl particle test to distinguish does a person have lot more damaged ldl which only a macrophage can clean up.

    a possible part of the artherosclerosis proces that i am missing here is the accumulation and per-oxidation of excess linoleic acid in the membrane of low density lipoproteine and the dustruction of the ApoB protein in the per-oxidtion cascade. And as a consequence, falling out of circulation, clearance by macrofages turning into foamcells etc. I thought that Witztum and Steinberg had this figered out in the '90.

      I agree with you, I myself think that the glyxated and oxydized ldl do respond to the signal of endothelial damage vut because their receptor does't work the are stuck there and only a macrophage can clean them up.

    I agree with Saladino. ApoB particles cannot be atherogenic solely on their own, since if they were, plaques would occur in veins. They don't. But if something else damages the endothelium that lines the arteries, then apoB particles can enter the resulting scab and cause problems. It is sort of like throwing salt in a wound: if there is no wound, it's no problem, but if there is one, it makes the damage worse.

    Had bloodwork done 4 months ago. My LDL was 198 now my Cardiologist wants to put me Lipator if my next bloodwork is the same. I've been on the Carnivore diet for 3 weeks.My biggest weakness is sweets. One of the things I've noticed since I've been on Carnovire my arthritis is not painful I cut out all the sugar stuff except one apple in the morning

    According to a book by Linus Pauling I read, atherosclerosis is a process of the body using a lipoprotein to patch arterial lesions when there is inadequate vitamin C for the body to synthesize the collagen necessary to heal the lesions. Depositing plaque to patch the arteries is an evolutionary adaptation to poor vitamin C status in the human population. It is an evolutionary adaptation you want never to kick in, and the only way to prevent it is to take plenty of vitamin C, far more than the RDA.

    So my current medical literature says the following: Endothelial damage is primary. LDL transport proteins can store themselves on these and set processes in motion. BUT: Endothelial damage is the factor, not LDL per se. And endothelial damage arises in particular from glycation of the proteins by high blood sugar levels. Not to forget what obesity also releases to mediators such as TNFa, angiotensinogen (blood pressure increase) and prothrombotic substances. Those are the primary factors, the LDL is just one factor in the cascade and NOT the conditioner. Healthy intact endothelium, normal blood pressure and blood sugar levels and it's okay. LDL is not the culprit, but endothelial damage caused by high blood sugar or blood pressure. In addition, any damage to the endothelium triggers activation of the immune and coagulation systems, completely independent of LDL. For me, this idea of ​​LDL as a "bad" transport protein is very lame. As for the statins, which inhibit HMGCoA reductase (cholesterol biosynthesis), well...fasting has the exact same effect, inhibiting HMGCoA reductase. From my biochemistry book.

    I cant manage to eat fully Keto. I eat low carb but still alot fat in my diet. I see my digestion is not that great and also have alot black fog before my eyes every time I stand up. Doc said my cholestrol is very high someone my age. (Age below 40) All the other marks for cholestrol were great only LDL was bad. It got me insecure im doing my low-carb diet wrong or maybe i dont have enough stomach acid to digest fats and maybe thats a danger cholestrol can get very high? Beside that i keep losing weight. How to stabilize that?

    I've been doing an animal based diet/keto+ with a lot of eggs for almost 4 years straight. A few days ago, my family physician had ordered blood work to check my cholesterol level. The results came back, showing severely high cholesterol. My LDL is 280, and HDL is only 60. Total chol is 360. The cholesterol/HDL ratio is 5.9. I am a lean guy with visible abdominal muscles. Should I check my insulin sensitivity? Idk, but these results look kinda scary. The doctor said that I'm the second guy in his 25 years of medical practice who had this level of cholesterol. Hearing that statement from him kind of made me feel special.

      Quick update. The doc's office contacted me and scheduled an urgent appointment because of my "abnormal" cholesterol level. So I seen him yesterday again. And I was surprised he didn't blame my diet at all, but he rather attributed all that to my... genetics!. He said that any diet wouldn't do so much of a difference, like raising cholesterol THAT high (2.5x times above the max). He said that my liver is pumping that cholesterol like crazy. And of course he prescribed me a statin. A strongest form called Rosuvastatin, which has some nasty side effects. I don't think I want to take it. But they're talking about a possible heart attack at much younger age in case if I don't.

    Indeed true to the spirit of what physicians will say.
    It's exactly what they'll say down to the exact wording. Let me retell my latest experience here in Norway:
    I went carnivore in January, ketovore in February and have gone Animal Based now in March, and will keep that up for as long as I deem it as the most superior diet. I did a blood test this week. LDL was 240, well above the reference range. I expected it to be that high, as I have been consuming a lot of meat and butter. Maybe more butter than I technically need, but this was on purpose for the time being. I received a msg from my doc the next day. She gave me a general recommendation of eating more margarine, whole grains, veggies and fish, and less red meat and butter, and that if the LDL levels were equally high in 3 months, we should consider a statin. That made me laugh. It was as cliché of a response as I could possibly imagine.
    Context: I'm 36, I lift weights regularly and have done so for 4 years, I've been very food conscious for the past 6 years, I eat clean homecooked foods, and all other parameters were normal and healthy and have been so in the past - even on dirtier diets with more body fat. Long term blood sugar was good, liver enzymes were good, Vitamin D was perfect, HDL was good, iron was good, etc. Heck, even the cholesterol to HDL ratio was just within a normal ratio despite a skyhigh cholesterol, as my HDL was as also high as it's ever been. I'm curious if she's gonna order the same barebones blood panel in three months or if she'll at least request a detailed cholesterol panel and fasting glucose etc (they sadly won't measure blood insulin in Norway, to my knowledge) to see exactly what the status is beyond the LDL particle count before putting me on biohacking medications. I mean, the blood test I had didn't even measure triglycerides. 🤷🏻
    We're basically not allowed to do extensive blood panels in Norway, unless we're physically symptomatic of something that warrants it. It's public policy. Some doctors might say "screw it" and "make up" the justification for extensive tests, but you'd be very lucky to find them. I had one, but he retired a couple of years back. So if I want to know my baseline and health status, I need to find an independent lab and pay out of pocket for tests, and best thing: my doctor don't even want to know or be responsible for analyzing the results. That's what she told me, straight up. It's not just her, though. It's policy. She was at least honest with me.

    I agree completely...Dr. Attia did a meta study on biomarkers of patients who had heart attacks and those who did not.
    He showed from the study that those with cardiovascular events had normal LDL but had low HDL and high triglycerides...a marker for metabolic syndrome.
    The non cardiovascular event patients had higher LDL and Higher HDL and low triglycerides.
    He then looked at their LDL-P size and concentration.
    He concluded that LDL-P particle size and concentration was a strong marker to cardiovascular events because the HDL/triglyceride ratio is closely related to LDL-P.

      Sorry to inform you but LDL and ApoB are the same thing. If you have high LDL you have high ApoB. There is no mathematical way around this. So if you are cool with high LDL, you must also be cool with your high ApoB.

    I am very strongly of the opinion that chronic inflammation is the culprit, LDL is made pathogenic by the inflammation, and that is the problem. Which I think makes C-Reactive protein the better test for potential issues. Carbs are inflammatory, polyunsaturated vegetable oils very inflammatory, high body fat also inflammatory. Keto diet advocates to avoid carbs and polyunsaturated oils. Carnivore also will avoid those things and is specifically a non-inflammatory diet program. Good fats and protein = not inflammatory. Add in exercise to build muscle and lose body fat, and those in combination with each other leads to a much less/non-inflammatory state in the body, allowing it to heal.

    After having a massive stroke four years ago, brought on by a dental procedure, I wound up in rehab where my biggest battle was with the head doc.deapite having an LDL of 110, they insisted upon giving me statins. My diet at the time was low sugat ( no refined carbs, no wheat products for over a decade)
    Organic meats, eggs and dairy products).anyway, since I was 71 and a woman, I knew stays were contraindicated. Because I refused, I was sent yoa nursing home and labeled medically noncompliant.
    I do not take meds and currently avoid western medical practitioners. The general attitude of hospitals toward anyone practicing wholistic medicine is hostility, although they may give lip service to organic foods.
    I went from being completely paralyzed on my right side to full mobility now, although i still use a cane. I have been practicing intermittent fasting all summer and eat
    Generally one meal a day in the early afternoon with a very small breakfast and sometimes a snack around sunset. Currently weigh about what I did as a teenager. 😉 Never thought it would be possible to fit into skinny jeans again!

    How about a subset Familial Cholesterolemia polymorphism/prothrombia "inborn coagulation" / fibrinolysis? Can someone still follow a carnivore diet? I'm 52, lean, in shape, low carb, and intermittent fast 4x per week. I'm in the process of finding out if I have FH, let alone the subset but these mainstream western docs seem clueless. They seem to have a protocol based on 30 years ago. My recent numbers seem pretty good too. Triglycerides 96 (well below 150), a1c is 5.5 and fasting glucose is 98 ( not diabetic).

      LDL damage is non linear function over time, the longer you walk around with high LDL the higher the odds of cardiovascular event, any cardiologist knows that, they see patients, most of them, majority walking around with high LDL for years, it is the era of internet that we buy into this nonsense, how many 80 year olds you know eat meat multiple times a day and eggs for decades? without any pharmaceutical intervention? How many 80 year olds you know eating normal balance healthy diet, including meats, veg, fruit, grains and normal cholesterol around? The internet took the fact that meat is good for you which it is, to eat pounds of meat a day, there is virtually zero people in their 80's that did that for decades, even tribes, they eat tubers, and they eat meat not three time a day 365 days... hope that helps, please don't play with your kids health for no good reason, Lebron James, all NBA, NFL, athletes did not get there by doing crazy diets, they just eat balance food

      That combination of blood lips are strong risk factors for heart disease in the case of a standard western diet. Going moderate to low carb will usually improve HDL and trigs. As will exercise, losing fat mass, not drinking and smoking

      @@69zirc I don't eat a standard western diet. I have some health problems that only allows me to eat certain foods. I eat organic butternut squash with pasture raised duck fat, and occasionally I eat grass fed grass finished beef liver, and steak. I can't eat a lot of it though. I react badly to beef tallow, and I cannot find soy free, corn free duck fat, or bacon fat. I have no sign of heart problems with all the blood tests I've done, and my inflammation is lower.
      Obviously other than my lipid profile.
      I also only eat two meals a day.

      @@odious5317 eating unsaturated fat from the duck, they require more volume to move and thus higher ldl particles are needed. Saturated fat from beef or ant other ruminant is more easily packaged in a lower number of ldl packages.
      Try to find a lab to do the ldl pattern test. If you don't have oxydized or glycated ldl then you are fine with a high ldl.

      These *can* be signs of diabetes.
      It's unlikely to be that given your diet, but you should investigate more.

    Could it be that the pressure difference alone explains why high LDL can lead to damage to arteries and not veins? It's not a small difference. Venous pressure is 5 - 10 mm Hg. Peak arterial pressure is 160-200mm Hg.

    LP(a) ,our bodies use it as a patch to repair arterial damage in the inner most lining. Our bodies need Cholesterol and most LDL is protective and useful. Statins lower indescriminantly. Get rid of the inflamation and the damage along with it and LP(a) isn't an issue. Linus Pauling had a lot to say about this. He went from clogged arteries at age 63 to clean arteries and lived to be 93.

    I have tried many 'mainstream' medication to help reduce my glucose levels brought on by diabetes, they made me sick and a shell of the human I am. My family doctor, in my belief, has, though a series of knee-jerk reactions, prescribed said medications and the side affects from them, flattened me with extreme chronic fatigue, nausea, diarrhea, abdominal pain, aching joints, headaches, the list goes on. I started to question my diet and for the last 2-ish years I have been med free, eating more meats, eggs (I cannot eat organ meat), I was eating more veggies, but soon realized that there are certain side-affects to these as well, like nightshades. I am still sitting high with my glucose levels and not seeing the reported changes, but there are some to note, like my body shape. It is changing, despite what my scale might say, so I wonder if my issue is renal, or pancreatic? I really don't care to talk to my GP, ideally, but it would appear I am at an impasse as we are currently experiencing a shortage of doctors. Time to put my 'Big Boy' pants on and go see her.

My vision has improved, my injured knee is 90% better, I no longer wear my glasses, and my resting heart rate has dropped 10 BPM. I'm sure going keto and trying carnivore helped, but extended regular fasts has been the best for me!

    Vitamin C and Lysine and Proline will regenerate the arteries and blood vessels.
    Sufficicent vitamin c is required for healthy collagen synthesis in our body.

      What I've read up on it's actaully the wheat proteins attacking the glycocalyx lining the artheries. The same proteins tha make the wheat dough rise and be fluffy. It's been increasing in last decades in modern wheat and it becomes clear the increase of artherial damage in modern times.

    Id caution you about tallow, as any long or slow cooked animal fat becomes significantly hydrogenated, and causes inflammation and other harm in the body. Many studies back this up…. I have a bad allergy to any slow cooked animal fat, but quickly cooked or fresh animal fat i have no issues. I had to do a full restriction diet to confirm that, along with finding studies. Same goes for store bought lard, or even slow cooked meat/ribs at home.

    If 88% have insulin resistance and a damaged endothelium, then what do you do to reduce plaque formation and a future heart attack? Carb and Sat. fat restriction are important but are inadequate. The only answer is to lower LDL which in turn lowers medium and small LDL particles that create plaque. Unfortunately, a statin is an answer.
