    My mom suffered from depression from infancy til her death, at 59 years old. I've never understood why she couldn't just snap out of her panic/anxiety attacks. This pandemic made me understand in the most awful way. It's not a switch. It doesn't just go away. I had to resort to medication several times.

      Unfortunately you’ve absolutely hit it on the head there.
      It can so easily be seen as a case of “just pull yourself together” but that’s like telling someone who can’t breathe, just breathe more
      We have to treat the cause not the effect. In mental health that is VERY difficult, as we need to be able to identify that first. Even then that might be an exceptionally difficult task

      Fiaram, I can't find your comments anymore and I'm sorry you had to go through that. When I said I didn't understand why she couldn't get better, it absolutely never means that I treated her like shit. I was a kid. I saw her on the ground telling me she was going to die of something I couldn't see. She looked fine and I didn't know what to do with her. I went to nursing school because of her. To try to understand what was going on. To monitor her health as I told her to breath and that she was going to be fine, you can't take your own experience and reduce everyone else's because yours was terrible. My mom never went a day without love and understanding, she had all the therapy and treatment and support she needed. My original comment was just to tell that you cannot ever ever understand what is depression or anxiety without experiencing. Even studying it, even living through it with a loved one, you won't know what it's like u til you feel it yourself. You took my comment and completely missed the point, while at the same time offending me and my entire family.

    Wow, this was an amazing video! I have been dealing with anxiety and depression for years and hearing how he talks about getting help has made me rethink whether I should talk to my doctor to see if there are treatments for me to make me feel better. Thank you Dr. Gill 💜

    This was probably one of the best and, especially when Dr. Stein shared his personal history with depression, even touching conversations about this illness I've ever heard. Two of my best friends suffer heavily from it and I'm often heart-wrentched because I'm unable to really help them. They're both in therapy but it's quite the slow process.

      The A-Z series is evolving. I think we made an early mistake of recording them in large batches, meaning we can’t adapt.
      I think we’re learnt a lot from this one, as we intentionally tweaked a few things before recording it - because we’re both very passionate about ensuring this doesn’t get side lined for patients

    Thank you for this video. I’ve suffered with depression for 14 years and I’m also a nurse, I was in massive denial for years about depression, but I would thank you for not just saying that medication is the only answer. Every patient you deal with and everybody is different, which, sadly most GP’s don’t recognise. Thank you Dr. Gill

    I find it really challenging talking about depression as a medical student. It took me a long time (and many setbacks) to even admit to myself it is an issue I need to seek help for. I was always afraid the medical school/colleagues would find out and it might become a problem in my future progression as a doctor.

      That is one thing broadly to be reassured about. Because of the issues with colleagues etc, health care staff tend to be seen in a different stream, and only by certain doctors to improve that reassurance

    Thank you so much doctors. I have suffered with depression for a long time and am transitioning right now from a clinical study medication that didn't work back to a more standard medication, and am in a really tough spot. I am a professional as well and feel that the worry about missing deadlines, etc. combined with the inherent perfectionism of it has been just totally shredding me up. I am in counseling as well and trying my best. I'm in America so there's not as many resources available to us, I wish we had something like the NHS. I found this chat comforting to watch however and will keep watching it and the rest of your videos. It is really important to know that I and others are not alone. Thank you.

      I'm sorry, and I empathize. I am a student, and after dealing with my depression for a few years school feels almost impossible and the progress it feels that I've lost hurts a lot.

    I had a breakdown last year because of lockdown and the realities of it. After that I was on a downhill slope and my family quickly realised that I wasn't okay, and helped me reach out to my GP. Now I've been on Fluoxetine since November, also with counselling sessions. I still get my bad days, but thank you for bringing this topic up since it is very difficult to talk about.
    Keep up the great work! Your videos have inspired me to follow me dream and work to become a psychiatrist and help other people! Stay safe Dr. Gill! 💕

    I have been visiting a psychiatrist here in spain for about 6 months and he diagnosed me from anxiety. Every single time that I spoke to him I was thinking non stop about the source of my feelings, and if i didnt found any reason why I felt a certain way, i asumed it wasnt happening, so I didnt said anything. Thanks to you, now I know that sometimes there is no logical reason why I feel that way, and that doesnt mean that its not important. Thank you so much dr gil. You have helped me sleep for more than a year now with your clinical examinations and now you have helped me a lot with my anxiety without even knowing that I exist. People like you are literally the heroes we need

      I’m sure your psychiatrist has been then source of the slow change, but if I’ve given some insight into how your mind might work, I’m really glad that helped!
      Have you looked at any of the talking therapies like CBT?

      @@DrJamesGill Thjere aren't many resources like that here in spain, but my local university has just started a program where recent graduate psycology students help other uni students for free, and I have signed up for it but it will start in two weeks. I dont know about the UK but here in spain, mental health is REALLY overlooked and slow, even in private services

    Thank you for this, the crisis team absolutely saved my life and the psychiatry team in Honiton, Devon managed to diagnose me - I was diagnosed with cyclothymia and they were so amazing. I’m so thankful.

      The crisis team really are brilliant, and frankly life savers. We do struggle with the less acute cases though, and it is definitely an underfunded area

    This may be off topic, but I have ALWAYS felt it is IMPORTANT for people to share what they are really going through. This whole society trying to make things seem like we are all living in the world of the Brady Bunch is just wrong and makes those of us going through struggles feel like we are substandard individuals. I’m glad it’s gotten more popular to share struggles and I think that makes people like me feel like maybe what I’ve endured isn’t all that different from what a lot of people go through. Kudos to those who share and want to be honest about their lives.

    I lost a classmate to suicide in high school and my best friend was suffering from suicidal thoughts as a result of her yet to be fully treated depression around that same time and it really scared me. Of course I was so wrapped up in my own situation with struggling really hard to achieve falling grades in high school that I didn’t even realize I too was depressed at the time because I didn’t have any active suicidal thoughts. I was chronically tired, had no self esteem, didn’t see myself amounting to anything, and felt trapped in a situation with no way out in which I emotionally ate to cope. It wasn’t until a change of high schools where I did a complete 180° with everything that I realized how bad things had been. Thankfully aside from a bit of a rocky road after my grandmother’s death I haven’t had any other bouts with depression like I did back then but now at least I know what to look for in case it ever happens again. This is so important, and thanks for sharing!

    Thanks for this video, Dr. Gill. For me depression is a constant comorbidity of ASD and ADHD. I did have a true depressive episode of extremely low mood+s*icidal ideation for some months after I was prescribed Latuda and it caused me akathisia. I’m off that medication now and picking up the pieces. I’m not really able to go back to school to work in healthcare, but your videos are just the right amount of information for a layperson to watch for personal interest.

    Patience is key for dealing with people that suffer from depression, though it is hard at times, I’ve realized that even though I don’t know what makes my wife depressed, being patient with her (even if giving her some space) seems to help her a lot. Whatever she needs. 👍

      You are right. Time is the best treatment for most. For most people, depression gets better naturally or can be controlled. It may need help from docs on the way of course. Its great she has you as support. My wife is brill too

    This was very educational. I have struggled with something for the last five years. I haven’t been able to really get help because when I go out in public, I just feel like I’m ok and everything is fine but when I’m alone I realize things that make me happy just don’t anymore, like spending time with friends and family. It’s like half my mind wants help and the other doesn’t.

    I suffer with Schizoaffective Disorder, Panic Disorder and Anxiety with intermittant Depression. I'm also a Type 1 Insulin Dependent Diabetic. This was really informative and even reassuring too. It would be great if you could do a video discussion on Psychosis/Schizophrenia.

    It'd be great if all doctors were this understanding.
    One doctor previously told me "you're young what do you have to be depressed about"
    There are significant amounts of doctors who do not take depression in young people and or men seriously.

      If I can carefully reframe that, it’s not that some doctors don’t understand, it’s that it is very easy for us at ask the wrong question.
      That sounds a strange thing to say, but if we haven’t asked the right, or conversely have poorly phrased our question, it means we don’t help the patient to fully articulate that is going on with them, and thus we can’t appreciate the severity of the situation, and we can appear not to understand, when actually we’ve not grasped the issue
      I know I’m guilty of that.
      This is why I have a mental rule, if a patient comes back two or three times for the same thing, I’ve probably missed something, and need to reconsider either the diagnosis or management.

      That last bit can be important, I may have the CORRECT management plan, but it might not be the right one for the patient - that happens often with mental health patients, so I have a tendency to “over review” at the start of a patient presentation.
      Bringing them back at 2 weeks, then 4, then 8 weeks, then 3 months time and the “as needed” (obviously excluding the yearly review)

      Bizarrely I think this makes me a “worse” doctor than some of my colleagues, as I’ll see comparatively fewer “new” patients.

      Perhaps your right and this particular doctor just poorly phrased his question. It did make me feel he wasn't taking me seriously so I asked to see a different doctor.
      She was wonderful and helped me get the help I need and made me feel I was being listened too.
      So definitely not attempting to tar all doctors with the same brush. We all know that mental health is extremely complex :)

      Actually, I’m really pro people seeing other doctors. This is a GOOD thing
      I know lecturers I’ve LOVED, and some who I hated having lessons with.
      I’ve personally had the same with counsellors in the past, some “get you” others don’t
      There is nothing wrong with this.
      It’s the same as people at work, we were not best friends with EVERYONE in a job, but we know that person A will be the person to chant about problem Y, and person B will have no clue about problem X, but is the master of problem Z.
      It’s just that patient often have struggled to get an appointment, so NEED the fix there and then. Again, struggles of the system

    Been suffering from depression since 2016 the progression of this mental illness is so slow to the point you doubt having depression but when it hits it peak it's really hard to manage.. I've decided to go to the doctor and he prescribed me antidepressant and anti anxiety drugs.. Helped alot but I feel like the way my mind process and my thinking became soo weird that I couldn't explain it to anyone

      often with medications, it is finding the balance between positive effect and the negative side effect. As long as the patient is happy with the balance, that is a win 😊

    I’m actually in the process of trying to become IAPT qualified as a PWP so that I may become a HI CBT therapist. It is nice to see the psychological side of things being mentioned as it can be easy for doctors to only mention them in passing and focus on the medication

      I actually feel talking therapies are MORE important than the medication, which is why I often support patients in stopping ASAP

    That was a great episode! Unfortunately there are some cases like mine that are beyond help. Years of medication and psychotherapy have done little. I still think about killing myself almost everyday. I've accepted this dark cloud above my head will only disappear the moment I draw my last breath. Hopefully I won't have a long life.

    Hi Dr. Gill, I'm glad you are spotlighting mental health in addition to physical health education. However, I'm a little disappointed that this interview was not conducted with a psychiatrist, psychologist, or other mental health professional. Great work discussing this topic in a non-stigmatizing manner.

      As a GP I’m often the primary physician when it comes to a patients mental health issues, referring onwards if we are not able to resolve things in the practice.
      For the A-Z series we are trying to keep things general, and discuss as most patients would experience the health service.
      However I fully agree with your point that finer details would be better discussed with a specialist.

    I've always considered physical exercise just as important as mental exercise, even as a kid. Now that I'm 40, I truly realize how much more it does for us than just burn calories. I hope that my country of the United States begins to learn that our obesity problem is just that, a problem, and if we'd encourage each other to be healthy, people would feel better too.

    Could you do one on body dysmorphia. I was diagnosed with it, yet locum GP was dismissive, didn't understand. From a patients view most medical staff don't seem to understand it. Good one on depression

    My wife went to the doctors about being down and depressed she just got asked if shes suicidal she said no and then he just put her on anti-depressants and sent her on her way until I had enough of seeing her down and saying she was worthless I made sure I sat down with her all the time asking her how she feels and what we can do together to combat this and now she so confident and outgoing took a while to see her like this she still has off days but nothing compared to what she was like before.

      Why they don't talk to young people about depression, anxiety etc early on in school is beyond me.

      It might be a good thing to cover, but there are so many things that it would be great to educate children about, but the teachers have their syllabus set for them, and the focus is the exam scores unfortunately
      My sister is a teacher so I have an idea of her frustrations of knowledge vs fact transfer

    Love this video, there’s going to be a huge impact on mental health for years after this past 18 months.
    This will probably get lost in the ether but it would be good to have some information on seasonal allergies. I did mention hayfever in another video but I’ve had awful cluster/over my left eye headaches because of the humidity recently! I’ve never had them before so it must be bad this year! Gem x

    This topic really hits close to home. I have schizoaffective disorder (schizo+mdd) since my early 20s, and in 2020 my mum got diagnosed with depression and anxiety disorder. Covid19 truly horribly impacts her mental wellbeing. She's gained quite a bit of weight from her meds (mirtazapine + olanzapine). That's such a horrible side effects, but at least she feels better now..rather than without the meds.

      Said carefully, it’s not that the services are rubbish, it’s they are underfunded and understaffed
      As a GP, I get 10 mins for a new patient (don’t get me started on how stupid that is) for mental health services, a new patient can be up to 90 mins.
      That drastically effects the number of patients the nurses/consultants/registrars/counsellors can see
      Hence the huge waiting lists.
      To put into perspective how hard that job can be, it is only recently that the psychiatric services increased their retirement age above 55 as the staff burn out. You CANT have a burnt out mental health team trying to help someone with burn out

      Its your right to ask for 'double appointments (20 mins)' from your GP'. (and the same GP). If you cannot get the right support consider changing your GP and within the next practice find one with an interest in MH). IAPTs can be good too

    I’ve struggled with mental illness for a long time. Mainly OCD and depression. I can say with certainty that ASMR and your videos and your incredibly soothing voice has helped a lot (along with medication lol). Its relaxing and reduces my anxiety and feeling of impending doom. Even when Im working, just putting it on the background helps a lot

    What do you think about the hypothesis that SSRIs influence serotonin levles only as a side effect, but mainly decrease ceramide levels in the hippocampus, that inhibit neurogenesis and therefore cause low mood?

    I can’t remember what it feels like to feel well, I think a lot of older people just learn to make do and get on with it. I don’t think gps have enough time or energy to give the help some need, I am often made to feel like I am being a problem so like so many people I talk to I start to research things myself.

      I’m sorry you feel like that. As doctors we try, but it is often very difficult to help a patient in the narrow 10 minute window - which isn’t actually 10 mins in reality.
      One of the options I give when we are really struggling is “I can see there is a lot more here to unpack, let’s try X as suggested, and continue this conversation in two weeks, but book you a double appointment”
      That tends to work well

      I think a younger gp are better as they seem to have more of an interest in Heath issues, I am over weight with type 2 diabetes and when I went to see my older diabetic doctor it was plane he was like oh here is another take away person who wants fixing, I spoke to my diabetic nurse and she said they get some patents go in who are over weight and have a can of red bull in there hand and say they can’t lose the weight. How many people does a gp see like that before they put everyone who is over weight in the same box, I have told gp what I eat and get a look of oh of cause you do. When I was in the cardiac centre the consultant said I need to lose weight but he understand how hard it will be due to all the meds I am on so I didn’t feel like I was in that box. It’s also very hard to see the same gp each time, I have seen a gp who said why did dr x put you on metformin three times a day I would have said two in the morning and one at night but it’s your gps choice. Is there a code that makes sure all gps treat the same illness the same way. I think the nhs is struggling to the point of breaking, don’t get me wrong I think the nhs is a wonderful thing my wife has been involved in it for over 30 years working in mentally handicapped hospitals that have now all closed down, she worker her way upto a staff nurse, she now teaching nvq in care homes and hospitals. Watching your you tube channel is very uplifting thank you for that James.

      Different people practice medicine - and that is the phrase - practice in different ways, as we don’t KNOW for certain
      As a comparison, I consider myself an interventionist - I hit patients HARD with diabetic medications (I’m a big fan of metformin) to try to stabilise things, then improve things, and then begin to wean patients off them.
      But it is difficult path, and the nature of food, and education about food, can mean that other there are forces working AGAINST the patient.
      Eg. They feel tired, slow and lethargic (is it mental health, diabetes, something else?) so they reach for the energy drink as they FEEL better. So it makes sense on one level, but then sets back the other changes.
      What we lack is time. So I’m hoping with the A-Z series we might be able to explain a few things like that to patients. 😊

      I fully agree James, it is time that is the issue with our healthcare and a lack of doctors nurses ect ect, and the pay don’t help anyone, my neighbour is bed bound and had carers come in twice a day, he stopped them after a while because they didn’t do anything apart from empty his bottle and fill in their paperwork, they are on minimum wage, not paid for the time driving between peoples homes. At least twice a week he would have a carer from provide who was an agency worker and it was their first day on the job, they had no training on using a hoist so in turn he now has bed sores, the NHS needs a good looking at to be honest, there are a lot of people getting rich by owning an agency then paying their staff as little as possible but charging the NHS and the tax payer a fortune then there is no money left to pay people like yourself a wage they deserve and should get. I don’t know how it can be fixed, all most of us can do is hope they get to go to a doctor like you who still cares enough to do their best to help. Don’t get me wrong my GP is very good he has been my GP for over 25 years but it is hard to get an appointment as he is so busy, then covid jumped in and made things a whole lot worse, I admire the work you GPs do, to be honest I don’t understand why most GPs just don’t start up private practises I would not blame them if they did. You must do the job because you really enjoy medicine and helping people. Thank you James I am looking forward to your next video. Sorry for the long messages but I think the NHS is a wonderful thing but being spoilt by the minority.

    Intermittent fasting HELPED me control my calories. Since I was on 1500 calories, eating 500 breakfast, 500 lunch, 500 supper left me feeling hungry later on. But 300 calories at 1pm, then having 1200 for my evening meal left me satiated -- and, to tie in the theme of the video, in a good mood.
    As an addendum, you say "Talk to your doctor". I think many people find that really hard these days; no appointments 😕

    I'm on 100mg of sertraline and haven't had a panic attack in such a long time now but I can still get those anxious feelings in certain situations I also feel that it has made me think clearer as now I ask myself am I sad or am I just bored?

      Quick answer yes
      Longer answer. Yes, but patients should always have regular medication reviews
      As an extension of the plaster of Paris analogy, people heal at different rates. But maybe they need to swap from the first drug, to a different one. Maybe after the plaster has been removed, the patient needs a walking stick in the long term.
      Each person must be viewed as their own case

    I agree with anti depressants BUT as a complimentary tool. You must identify the areas of the patients life with how they arrived to depression - without doing so the depressed state of mind will persistently return. They are coping tools not a fix.

      I do partially agree here
      Antidepressants ARE an adjunct, and are often less effective than talking therapies.
      HOWEVER
      There isn’t always an external reason for depression, although this is comparatively rare

    You didn't mention bupropion, is that one often used in the UK (I'm in the US)? I was prescribed it and felt great, plus I liked the positive weight and libido side-effects. My doctor also suspected undiagnosed ADHD and she said that it can also be used off-label for that too, although I didn't notice any difference there.

    Depression is tough. I heard some who suffer from depression, they like to make others happy like doing comedy like one of my favorites Robin Williams. Another young lady went into the adult films she also suffer and ended her life at 22. What are these shock treatments I been hearing about, are they trying to shock the depression away, I'm no doctor but that sounds dangerous and I could imagine the side effects!

    Hi James. I’m on 60MG duloxetine to treat anxiety. I have tried many different medications many you mentioned in the video. I feel like they all haven’t done much and do you think it would be worth upping the dosage even more? However i don’t want to feel dependent on a drug to make me feel better, whilst i also don’t want it to affect my day to day life.

      I would advise having a discussion with your doctor
      The best analogy for antidepressants, it again, like orthopaedics. Someone people need a plaster of Paris and a walking stick to help them heal.
      Some need the walking stick for a longer term whilst they get extra help (replace the physiotherapist with this psychologist in this analogy)
      Each patient needs what THEY need.
      We wouldn’t feel bad about needing a walking stick, so we have to normalise having treatment for our mental health - in which ever way works best for the patient

      There's nothing wrong with a higher dose, intrinsically. Sometimes scaling up the dose is all it takes. You should discuss it with your doctor, and there are possible risks (seratonin syndrome, side effects you hadn't felt at lower doses), but I just don't want you too feel like increasing your dosage is off the table. It can help.
      You should tell your doctor how you feel about the medications you've tried, also. There may be more you need than just a daily ssri, like lifestyle adjustments or work/school/home interventions and a doctor can refer you to psychiatrists and psychologists who can help you too.

    I have depression. I've had it pretty consistently for about 12 years. I'm currently at one of the lowest points I've ever been at. I want to quit the job that makes me stress and sad so I can have a short work break to look after myself and put my health first. Everyone I tell this to say it's a bad idea. Any advice anyone?

    I was diagnosed with depression only got better off the meds and with a holistic approach with the psychologist and substantial changes in my life.
    If you’re suicidal yes that’s a different thing. Please understand that you are only treating symptoms just like you do with a headache. You’re not treating the depression itself with medication.

      I’d disagree somewhat. This is the issue with sweeping statements, they don’t take account of the grey
      “You are not treating the depression itself with medication” is true for some people - especially those with a significant external component.
      HOWEVER that is not the case with all patients, some will have a largely endogenous depression and may only be helped by medications.
      Medicine only works by treating the patient and not just an algorithm, and each patient may need a slightly different approach.
      We need to normalise and support ALL methods of treating someone, so that people don’t feel disinclined to one method or another based on the view point of a different individual who has their own psychological background 😊

    Fasting goes far beyond purely weight loss. Michael Mosley's work on the subject showed some benefits around diabetes and other areas.

      That’s very difficult. It’s a little like trying to give up smoking, we can encourage people, but if you are in the “precontemplative stage” it can actually make a patient pull back if we push too strongly

      There are large numbers of different medications, but also different approaches that are not medication. For milder depressions, the first line is actually talking therapies.
      But the best approach would be to talk you our doctor, and see what they can do to help 😊

      FAILING THAT YOU CAN *ALWAYS* go to your nearest hospital
