The pilot of the coast guard Dash 8 was 100% completely at fault for not stopping at holding point C5, he just proceeded to the runway, although TC should have paid attention to this. Miracle all 367 passengers got out safely thank god over a stupid mistake by the pilot of the Dash 8, he should have NEVER been on the runway in the first place.

That’s a bit harsh, at least at this stage. The accident could have been partly due to the DASH’s pilot’s “stupid mistake” or a true misunderstanding of what was communicated from the tower. And then there’s the issue of traffic warning lights being inoperative; clearly a contributing factor. So yes, the DASH’s pilot appears to bear much responsibility for the accident but as is usually the case, there is likely to be a chain of events that led to this tragedy. It’s amazing that the loss of life was not worse.

This is very similar to accident on KLAX airport when controller simply forgot that there is regional plane very similar to Dash-8 on runway, due to high workload. Having lights which were supposed to light the apron also glaring through the control tower windows didnt helped as well.

I find it interesting that Japan has already implemented changes to help prevent another such disaster.
The NTSB takes three years, then tells us stuff we already know. Their favorite is: “The plane stalled because the critical angle of attack was exceeded.” Wow, that’s insightful.
Then in the unlikely event that the NTSB issues recommendations the FAA will likely ignore them.

Time 9:53 to 9:59 is useless so delete& replace with more detailed view of both planes moving down the runway. Lastly, do not end at10:19; there is more that can be shown regarding the arrival of firefighters etc.

Did Japan released the CVR conversations in the Dash8?
Did CRM played a role in the mistake inside the Dash8 flight deck?
Did Haneda had ARIWS in place?
If they do, was it operational at the time?
If it was, Did Tower ATC failed to see that runway incursion has occurred and an accident is about to happened?
Japan has so much to explain why something like this happened on a premiere aerodrome like Haneda.

Japanese culture will impact the investigation smoothing over some details in order to save face, focusing on the process, not individuals. Americans on the other hand do not spare any opportunity to throw the involved people under the bus. The favorite target of the NTSB is the deceased pilots who cannot defend themselves.

A little correction for this video, during all take off and landings the cabin lights will be dimmed, this is so important factor in this accident, as far as i know all passengers get out of the aircraft safe, all 300+ passengers evacuate into the darkness in under 90sec, had the cabin lights not dimmed some of the passengers might not have made it, because human eyes need to adjust to the darkness outside an lost the life saving seconds. Thank you for this excellent re-creation of this horrible and sad event. I'm sure certain things will change or let's hope so i.e. the plane on the runway couldn't be seen from the approaching Airbus WHY? MAYBE PLANES NEED LIGHTS ON TOP THAT STROBE WHEN THEY ARE ON THE RUNWAY. AND PLANES SUCH AS BOEING AND AIRBUS PASSENGER PLANES SHOULD BE EQUIPPED WITH DETECTION SOFTWARE SOMEWHAT LIKE THE MODERN CARS HAVE WITH CRASH AVOIDANCE AND CAR IN NEXT LANE ALERT. BESIDES THE HUMAN LIVES LOST 😢 THE AIRBUS WAS ONLY 2 YEARS OLD AND THE OTHER WAS ONLY 7. ALL LUGGAGE LOST AND AID SUPPLIES FOR EARTHQUAKE RELIEF LOST SO THIS AFFECTED MORE THAN JUST THE IMMEDIATE PERSON'S ON THE PLANES, I PRAY NO ANIMALS WERE ON THE AIRBUS 🙏

There is something called ground radar but apparently the ATC people were short staffed for the workload involved. The Japanese are looking at this issue as part of the investigation. From accident to complete evacuation, yes. But first the plane had to stop. then, they had to overcome the fact that the intercom was down. so pilots were unable to give the evacuation order. and they had only very few slides available. given the totality of circumstances, this was a good evacuation. and yes, it's likely the a350 carbon fiber fuselage may have helped the success, as it seems it's more fire resistant, than aluminium. not so much luck, as good airplane, good crew and good passengers (following instructions).
