  • Hard flaccid syndrome is a perplexing and distressing condition affecting men's sexual health, characterized by persistent firmness of the penis in a flaccid state. Typically, this condition arises after events such as prolonged masturbation, jelqing injuries, or intense sexual intercourse. These activities trigger excessive neural activity from the penis through the pelvic and perineum regions, leading to hyperactivation of the sympathetic nervous system. Commonly, patients report symptoms like coldness and numbness, despite normal neuro and ultrasound examinations. In this interesting medical interview on UroChannel, doctors Dr. med. Dr. phil. Stefan Buntrock and prof. Irwin Goldstein discuss this underreported medical condition.
Interestingly, studies suggest that nerve sprouting might be responsible for this syndrome. Once nerve sprouting occurs, the hyperactivity of the sympathetic system perpetuates, making it challenging to halt the condition. Providers often overlook hard flaccid syndrome due to its atypical presentation, not occurring during erection but in the flaccid state. This oversight highlights the need to reconsider the scope of men's sexual dysfunctions.
There is currently no definitive treatment for hard flaccid syndrome. Techniques such as using cold ultrasound gel and the Rigiscan device can help visualize the condition. Some experimental treatments include Botox injections aimed at reducing sympathetic activity within the penile tissue. Shockwave therapy has shown limited success, and ongoing research is vital for developing effective treatments. Despite the challenges, continued efforts and innovative approaches remain crucial in finding a solution for hard flaccid syndrome.

Awesome. Very encouraging to see the optimistic attitudes and willingness to experiment. I truly hope that the politics will stay away for some time so that real research and real therapies can be developed.

Hi doctor, thank you for this interview, it's very helpful although it's a bit frightening. How about mild traction in fully flaccid state? E.g. traction devices, or some mild manual traction?

Would be helpful to know the type of pump the individual used. You can definitely hurt yourself by over pumping, however there are pumps that use water, and are alot safer to use, but one might take this information, and think that pumping causes hard flaccid. Which it might not. My concern is that pumping gets a bad rap because of this, when it might have just been down to the individual overdoing it and injuring himself.

Pudendal neuralgia from trauma in the pelvic floor, such as improper bicycle seat, seems nearly if not impossible to resolve once it's happened. Almost like a switch is turned on; not much warning.

Actually, such a situation happened to me one year ago, while having sex using a small condom, which triggered a mild injury at the side of the penis, then tissues on the side of injury started contracting and getting a king of tingling for one week. In that time, controlling the orgasm was a kind of difficult due to excessive activity of the sympathetic pathway. I used Diclofenac 75 mg for many days until I recovered.
