I never play "the evil guy" role in Fallout. I can't imagine myself being an asshole to wastelanders. I always go to very good karma.
The devs made a good job at giving the NPCs emotion, makes me feel like I'm actually ruining someone's life or fucking with them, hard to do
I still want to play a "evil" playthrough but everytime i go to do so i cant help but devolve into a good one again
So most of my playthroughs end up being the courier is a good guy, just ignore what happened in goodsprings/primm.

I really appreciate the fact that the world reacts to your evil character and obsidian lets you roleplay as a bad guy throught the game. Really cool that there are so many role play elements in the game

I never realized how much of an ass you could end up being in this game! It’s quite remarkable how good the dialogue system and choices are in this game!
This game was truly a gem. They don’t make games like this anymore, you can either be the saviour of the people or convince people to commit suicide, murder, put people into slavery, or send them into a berserk rage.
Nowadays companies are to scared to give you an in-depth bad guy option like this game did.

You're right about Fallout 4 where it's there's few evil choices. But Fallout 3 has just as many if not more evil choices than New Vegas.
