Lake LA-4 crashes on takeoff from Lake Isabell, California

The pilot of the amphibious airplane reported that, after landing on the water he taxied to a boat ramp where he drained water out of the airplane before he took off from the top of the boat ramp. Once airborne, he established a climb attitude, but the airplane was not able to maintain a climb. He further added that there was insufficient time to raise the landing gear before the airplane landed on the water. The airplane sustained substantial damage to the right wing and aft fuselage and empennage.
A witness reported that he assisted the pilot in pulling the airplane out of the water and onto the boat ramp. The water was drained out of the airplane, and then the pilot commenced with the takeoff. The witness further reported that the airplane struck a tree branch before it impacted the water.
Review of recorded video provided by the witness showed that the airplane departed from the top of a declining concrete boat ramp and became airborne just before the waterline at the bottom of the ramp. Throughout the takeoff sequence, the engine could be heard with no noticeable changes in sound. The video showed the airplane briefly remained airborne at a low altitude before it descended into the water.
A Federal Aviation Administration inspector interviewed the pilot. The pilot reported that there were no preaccident mechanical malfunctions or failures with the airplane that would have precluded normal operation.
Probable Cause: The pilot's failure to establish a positive climb rate after takeoff, which resulted in an impact with an object.

At that density altitude sucking that gear up immediately is crucial. The whole thing looked like it was poorly thought out. Even looked like he didn't use all the ramp.
Any less pixels and that'd be radio...

The NTSB final report concludes the following: "The pilot’s failure to establish a positive climb rate after takeoff, which resulted in an impact with an object." I'm saying this because the NTSB often gives simplistic conclusions that miss the real cause. If we are to learn from accidents, this sort of reporting is of little use. A better conclusion would be: "The pilot's failure to allow an adequate takeoff distance." Said that way, there's a possibility that something could be learned.
I'm not a pilot but that seemed like pretty sketchy takeoff plan.

That was probably an illegal takeoff. A takeoff from the water would be legal, assuming that it's allowed on that body of water, but that boat ramp makes it an off airport operation. Comments?

The dummy panicked and didnt pop out more flaps at Vr. I did that many times as Bush pilot and CFI.

Is it just me, or did none of the inspectors notice the elevator trim wasn't set for takeoff. The last outboard foot of the elevators are usually up at about a 45°angle for takeoff because of the high thrust line. The pilot probably could raise the gear because he had both hands on the yoke trying to keep it from nosing in.
First thing I spotted as he went down the ramp, basically had full down trim. Must have been pulling hard on that yoke to stay airborne, which is why he couldn't put the gear up.
Yeah, these planes are so trim dependant they all came with electric trim with with a yoke mounted switch. I've never seen what looks like full nose down like this before.
This is good to know. I never flew one of those but assumed the “runway” wasn’t long enough

You've got to wonder what power that thing was really generating. Perhaps the power checks, mag drops etc were all performed properly before videoing started, and it wasn't just the "kick the tyres and light the fires" buckaroo shitshow it looks and sounds like.
