    Saw a patient that presented with this exact finding. I didn't know what to make of it but now it strengthens my MS suspicion.

    As a person living with MS, i can only manufacture the lhermittes sign/electric shock feeling in standing position, usually if i am walking or moving an arm around too. Nothing happens if i lean my chin down while in sitting position.
    It is a horribly painful sensation and i wouldn't wish for anyone to experience it, not ever.

    I had this exact sign going on before I had any medical knowledge. Unaware of the possible dangers I went on a mountain warfare course in Norway. After 6 weeks of carrying a 30kg backpack over mountain tops it was gone. Haven't felt it since. Made for quite a good story in physiotherapy school.

      The statistical values are very poor. So it can be very probable that you experience the Lhermitte sign but don't have a pathology of the dorsal column. It's just a screening test to help your decision making when the patient reports clinical symptoms that fit within a pathological pattern.

      @@Physiotutors i just had the Lhermitte sign 6 weeks ago for the first time in my entire life. It coincided with my Modena booster shot. It only lasted for two hours but it was very scary. When I was putting my chin in my chest I was getting electrical shots in my head and my arm. It went away and I thought nothing about it. Now I read about MS and I am very scared I might have it.

      @@PrettiPetty24 no, it never came back. It was probably a side effect from the vaccine. And the shock was in my head and face also. With lhermitte it’s down your spine so I guess it was something else

    I'm pretty positive I have this issue. It did it the 1st time when I was 41 years old and now that I'm almost 43 it's done it about 15 times. It's an intense lightning bolt shock pain that runs down below my ear down the side of my neck into my shoulder it comes and goes within 2 seconds but it is quite painful. After it happens I get anxiety over it and can't stop thinking about it. I also have pretty low grade chronic muscle pain. I am a physical person and I eat a primarily plant based diet

      How primarily is primarily?
      Iirc the RDA for iron is 8mg for men and a cup on spinach is only 0.6mg. But if you get iron from non-heme (plant based) sources you actually need to increase your RDA by 1.8 times.
      This means most plant based diet are iron deficient if you don’t supplement. Iron deficiency can contribute to nerve issues.

    I’ve felt that shock sensation when I do an intense stretch, not by moving my head down. Could that sensation be normal if you are stretching? I am worried I have early signs of MS but I have pretty bad anxiety so I’m probably overreacting

    Great video although I had a doubt. The orthopaedic assessment textbook of Magee suggests the patient to be in long sitting position while both the head and one of the hip joints simultaneously resulting in shock-like pain. Any reason y'all performed the test in standing and without the hip flexion?

      Very much wondering where Magee took this description from as it seems that they are performing something similar to the slump test in the book. The reference they use links to another book instead of an article.

      @@Physiotutors Yeah it looks like the modified slump test (ST4) for spinal cord and nerve roots but that test doesn't involve raising the hip into flexion simultaneously like the way they've shown in the L'hermitte test
