    Dr. Lam I was recently told by a extremely prestigious facial plastic surgeon that the Deep Plane isn't used that much anymore, and it has been replaced by the High SMAS. I was confused by his comment during our consultation, because practically every Doctor including yourself primarily support the Deep Plane procedure. That being said, are there some similarities between the Deep Plane and High SMAS technique?

      So this is very inaccurate at so many levels and I’m hoping you misunderstood him or her. The term high smas is specifically a variation of the classic deep plane. In short whenever high smas has been used it is a type of deep plane. What it means is you make a squared off entry into the deep plane from way up high near the ear. The proponents of high smas argue that you can capture a wider area to elevate. The proponents of a more classic deep plane (which I am) argue that the entry point of the deep plane being so far away limits better elevation of the midface and jawline since the distance to the part of critical elevation is too far. I have seen great results with both high smas and classic deep plane or what I like to call extended because I go forward to the smile line. But if this doctor is suggesting that he does a high smas and does not do a deep plane then that is absolutely unequivocally a wrong understanding of the term. In fact, the trend for high quality surgeons is almost all going to the deep plane. How do I know? 95% of the lectures at academic meetings now are on deep plane whereas when I started it was probably about 20% 20 years ago and maybe about 50% 10 years ago. Keep in mind I’m talking about the top guys in the field. Still I would say 95% of those doing facelifts do not do deep plane because you really need to know what you are doing.

      @@samlammd I appreciate your detailed response and insight Doctor. I believe that this Doctor was describing the same thing that you did. The traditional Deep Plane has been abandoned by top docs and is now modified w/ the High SMAS technique, but it's still Deep Plane. I was confused until you explained this in different terms for me. He is definitely one of the elites in his field and I don't doubt his technique, especially after the way that you explained it. Definitely the process of finding that 5% of top docs who specialize in this, and who are affordable and available at the same time- has not been smooth sailing.

      @@ninjask8ter ok good. I really am not going to argue that high smas is better or worse. I’ve seen fantastic results with both but I would definitely say high smas is not more popular or trending toward it. It’s probably about 15-20% of deep plane people who do it and there’s no trend toward or away from it.

      @@samlammd I think I found you based on your detailed video analysis of what a Deep Plane procedure is. I actually took notes on your description of the ligaments. I took A&P 3 times and perhaps that's why I'm interested in these details. As you may know there are dozens of videos based on the Deep Plane procedure. What I haven't been able to find is a comparison between the Deep Plane and High SMAS procedure (not conventional SMAS procedure). Perhaps if you would consider making one of these videos in the future, it would be helpful to a lot of other people as well.

    I had a smas face-lift done in Dec 2022 but I can see my jowls showing slightly. I think not many surgeons in South Africa perform deep plane fl. It appears more dangerous though in the wrong hands.
